An interactive program for the analysis of ENG tracings.

An interactive program for clinical and research evaluation of vestibular nystagmus has been developed. Because of the interactive structure of the program, the clinician can make immediate decisions on the interpretation of controversial aspects of the nystagmogram; therefore even the more irregular and noisy ENG tracings, frequently obtained from pathological subjects, can be easily processed. In this way, speed and accuracy of computer processing are coupled with intelligent activity and critical judgement of the clinical evaluation. The program can be implemented on a general purpose minicomputer and no specific technical training is required for its use. The structure of the program and the operations performed by main subsystems are discussed in detail. Examples of ENG tracings processed with the program are reported.

Quantification of vestibular compensation in unilateral Meniere's disease.

The main parameters of the vestibulo-ocular reflex (i.e. the gain or reflectivity, the long-time constant of the semicircular canals, the time constant of the mechano-neural transduction, and that of the central leaky integrator) were computed from the post-rotational nystagmic responses of unilateral Meniere's patients. Comparison with normal subjects showed a decrease of the first two parameters both in CW and CCW responses. No significant changes were observed in the remaining parameters. Probably due to a process of central compensation, a high level of symmetry was observed mainly in longstanding patients.

Harmonic versus impulsive acceleration testing of the vestibulo-ocular reflex in normal humans.

Vestibulo-ocular reflex (VOR) was tested in 9 normal humans by using both harmonic (sinusoidal) and impulsive (post-rotatory) angular accelerations. VOR gain and main time constant were 0.49 +/- 0.20 and 15.48 +/- 2.26 s, respectively, when computed from sinusoidal responses, 0.45 +/- 0.17 and 14.28 +/- 3.07 s when computed from impulse responses. Paired comparison could not prove statistically significant differences between the two sets of data. The conclusion is drawn that, in normals, equivalent information about VOR static and dynamic characteristics can be derived from either harmonic acceleration or post-rotatory tests.

Circulating IgG response to stromelysin-3, collagenase-3, galectin-3 and mesothelin in patients with pharynx/larynx squamous cell carcinoma.

With the aim of identifying tumor-associated antigens that could be potential markers and/or targets of diagnostic and/or therapeutic approaches, we studied the occurrence of circulating IgG antibodies to human stromelysin-3, collagenase-3, galectin-3 and mesothelin, by Western blot against their purified recombinant forms, in the sera of 50 patients with pharynx/larynx squamous cell carcinoma (PLSCC), as well as in the sera of 50 healthy blood donors. Overall, antibodies to collagenase-3 were detected in 50% of all the cancer patients and 16% of the blood donors examined; this percentage difference was statistically significant (p = 0.00066). With respect to anti-galectin-3 antibodies, the percentages were 32% and 18%, respectively, but they were not statistically different (p = 0.16). Low levels of antibodies to stromelysin-3 and to mesothelin were detected in sera from only two cancer patients. No significant correlations were found in the present study between the presence of antibodies to these proteins and tumor site, clinical and T stages, lymph node involvement, DNA ploidy and histological grade of differentiation of the primary tumors. To our knowledge, this is the first report on the detection of circulating IgG to collagenase-3 in cancer patients. Some of the percentages found here in certain groups of patients are among the highest reported of circulating antibodies to any tumor component studied so far. The monitoring and the use of human antibodies to collagenase-3 could be of diagnostic and therapeutic interest.

[The definition of a reference protocol for the clinical study of vertigo drugs].

The aim of this paper is to define the problems that arise in the clinical evaluation of drugs for the treatment of vertigo. Among these are the objective criteria used in defining vertigo and those used in evaluating efficacy of the drugs. The resulting protocol for a clinical study of vestibular drugs is a document that clarifies the debated points in the field, and above all furnishes guidelines for establishing uniformity in clinical studies. This, therefore, may become the reference protocol in Italy for clinical evaluations of drugs for the treatment of vertigo.

[Etiopathogenesis and clinical aspects of allergic and vasomotor rhinopathy].

Nasal hyperreactivity is the common physiopathological mechanism of allergic and non-allergic rhinitis. Specific hyperreactivity is characteristic of the IgE-mediated nasal allergy; aspecific hyperreactivity is associated with an amplification of the normal nasal reflexes, due to complex and, as yet, only partially understood factors. Rhinitis or rhinopathy can be classified as allergic, seasonal or perennial, and non-allergic. Perennial non-allergic rhinitis or vasomotor rhinitis is a heterogeneous disorder consisting of at least two subgroups, eosinophilic and non-eosinophilic. The main clinical aspects, the differential diagnosis and some modes of treatment of the different forms of rhinitis are discussed.
