Long-term follow-up of pseudoexfoliation and the development of elevated intraocular pressure.

A retrospective study was performed to determine the long-term course of pseudoexfoliation (PEX) as it relates to both the development of elevated intraocular pressure and to the initial appearance of exfoliation material in normal fellow eyes of patients with PEX. Six hundred eighty patients seen from 1940 through 1980 were diagnosed as having PEX. Patients were excluded from long-term study if there was a diagnosis of glaucoma prior to examination at our institution (199 patients), if glaucoma was present at the time of diagnosis of PEX (40 patients), if there was a history of prior intraocular surgery or disease (19 patients), or if there was only one ocular examination performed (75 patients). The remaining 347 patients with initially normal intraocular pressures were followed over time and consisted of 225 patients with unilateral PEX and 122 patients with bilateral PEX (a total of 469 PEX eyes). Life table (actuarial) analysis was used for data evaluation. During the course of the study, 32 (14%) of the unilateral cases of PEX became bilateral. The cumulative probability of a normal fellow eye developing PEX increased over time, with a 6.8% cumulative probability in 5 years and a 16.8% cumulative probability in 10 years. Elevated intraocular pressure developed in 35 patients (44 eyes) during the study. The cumulative probability for eyes with PEX to develop elevated intraocular pressure was 5.3% in 5 years and increased to 15.4% in 10 years. These probabilities predict that patients with PEX have a higher incidence of developing elevated intraocular pressure than would be expected in patients without PEX of similar age.

Alteration in cerebral perfusion: clinical concept or nursing diagnosis?

Nursing diagnosis was introduced into the neuroscience intensive care unit (NSICU) of the Medical College of Virginia in 1984 as a framework for naming the problems nurses treat. During this process, there was concern about using nursing diagnosis terminology to describe common NSICU problems. Of particular concern was increased intracranial pressure (ICP). The staff concluded that "alteration in cerebral perfusion" was the nursing diagnosis from the North American Nursing Diagnosis Association (NANDA) list that best described the patient with increased ICP. A descriptive study of patients who received this nursing diagnosis in our NSICU was designed and implemented one year later. This study identified patient characteristics and independent and interdependent nursing interventions.

The adverse effect of benzhexol on memory in Parkinson's disease.

Seventy-eight subjects suffering from idiopathic Parkinson's disease were studied in 2 separate groups to test the hypothesis that benzhexol affects memory. In study A, 54 subjects were tested by means of a free recall technique; in study B, 24 subjects were given a signal detection memory task. Both studies yielded significant correlations between memory impairment and dosage levels of benzhexol. No correlation was found between memory status and dosage of levodopa, duration of illness or Hamilton depression scores. The implications of these findings are discussed.

Hysteria: its nature and explanation.

Hysteria is a concept that has led to much confusion and controversy. It is argued that a phenomenon exists which needs to be explained. The main contemporary approaches to hysteria are considered and all are shown to have serious weaknesses. It is argued that the illness behaviour/sick role model does have at least heuristic value in that it leads to asking a rather different set of questions about hysteria which should prove more amenable to experimental investigation. It also avoids the logical difficulties inherent in most of the alternative theories.

The production and use of monoclonal antibodies to measure apolipoprotein B by a competitive enzyme-linked immunoassay.

The production of monoclonal antibodies to human lipoproteins is described. One of these antibodies, which was shown to be specific to apolipoprotein B, was used to develop a competitive enzyme-linked immunoassay for apolipoprotein B in serum samples. The antibody selected recognises apolipoprotein B in both low density and very low density lipoprotein particles, but there is no cross-reactivity with high density lipoprotein. There is no requirement for labelling of antigen or antibody used in the assay, and results obtained correlate well (r = 0.88) with measurements on serum samples using a radial immunodiffusion assay for apolipoprotein B.

Transoral transclival removal of anteriorly placed meningiomas at the foramen magnum.

Anteriorly placed meningiomas at the level of the foramen magnum are rare, are difficult to diagnose, and present technical problems for a conventional posterior fossa removal. The authors describe the successful transoral transclival excision of two such tumors. Cerebrospinal fluid (CSF) fistula can be avoided by dural repair using a thrombin glue and long term CSF diversion. This modification enables the transoral route to be considered for anteriorly placed intradural lesions.

Effect of left atrial to left femoral artery bypass and renin-angiotensin system blockade on renal blood flow and function during and after thoracic aortic occlusion.

Temporary thoracic aortic occlusion can result in renal insufficiency with or without adjuncts to avoid distal hypoperfusion. In a canine model of thoracic aortic occlusion, left atrial to left femoral bypass was compared with blockade of the renin-angiotensin system. Renin-angiotensin system blockade with the converting enzyme inhibitor, MK422, resulted in restoration of baseline renal blood flow and glomerular filtration 30 minutes after cross-clamp release. Left atrial to left femoral bypass resulted in significant reduction in both renal blood flow and glomerular filtration 30 minutes after cross-clamp release. Stimulation of the renin-angiotensin system plays a significant role in the altered renal hemodynamics and glomerular filtration rates after thoracic aortic occlusion.

A rapid and sensitive enzymatic-radiochemical assay for the determination of triglycerides.

An enzymatic-radiochemical method suitable for the determination of triglyceride levels of cells in culture is described. The method is based on the enzymatic hydrolysis of triglycerides to free fatty acids which then complex with 63Ni. The method is rapid, accurate, and inexpensive. The procedure extends the sensitivity of triglyceride measurement to as low as 0.25 nanomoles.

Optimal time for obtaining peak and trough theophylline levels in 12-hour and 24-hour products in children.

A direct relationship between bronchodilator efficacy and toxicity by the measurement of serum theophylline concentrations is well established. This study evaluated, in children receiving either 12-hour (Theo-Dur) or an experimental 24-hour (Theo-Beads) preparation, the most common time to reach peak and trough levels in the serum. Twenty-three children with chronic asthma between the ages of 7 and 12 years participated in the study. Twelve received theophylline every 12 hours at a dose ranging from 400 to 800 mg per 24 hours. Eleven children received a 24-hour preparation at a dose ranging between 400 and 1000 mg per 24 hours. The results demonstrated no relationship between dosing interval and the appearance of peak or trough levels with Theo-Dur. Theo-Beads reached its peak in 9 to 11 children between six and eight hours and for 10 to 11 children the trough value was at the end of the dosing interval.

Tissue culture of human choroidal melanoma cells obtained by fine-needle aspiration biopsy.

The optimal treatment of choroidal melanoma is controversial. Many patients are now being treated with nonenucleation therapies. Fine-needle aspiration biopsy of intraocular tumors may well prove to be safe; we used a needle biopsy technique on three freshly enucleated globes and established choroidal melanoma cell lines in culture from these specimens. This report describes our needle biopsy and tissue culture techniques.

Effect of copper on insulin release by the intact rat pancreas and the perfused rat pancreas.

In previous studies, we have shown that rats fed a copper-poor diet have an impaired glucose tolerance and insulin response to an oral glucose load. Furthermore, CuCl2 X 2H2O added to the incubation medium stimulated glucose incorporation into the diaphragm glycogen and epididimal fat of rats. The purpose of the present study was to examine the effect of copper on insulin release in vivo and in the perfused pancreas. Copper chloride stimulated the insulin release by the perfused pancreas of rats fed laboratory chow (copper content, 6.7 ppm). Stimulation with 0.5 mg/dl CuCl2 X 2H2O for 20 min resulted in release of 14.7 +/- 3.1 compared with 18.9 +/- 4.3 mu insulin following stimulation with 16.7 mM glucose. The CuCl2 X 2H2O effect depended on the presence of calcium in the medium--as with other insulinotropic agents--but not on the presence of magnesium. Animals fed a copper-poor (1.2 ppm) sucrose diet have a selective impairment to i.v. glucose-induced insulin stimulation, but not to i.v. arginine or aminophylline stimulation when compared with controls fed a sucrose copper-supplemented diet or a laboratory chow diet. This selective beta cell defect is not necessarily genetically inherited and may be acquired. The role of copper on insulin release is discussed.

Detecting hysterical sensory symptoms: an elaboration of the forced choice technique.

Patients with hysterical sensory symptoms can often be detected because they give significantly fewer correct responses with forced choice testing than would be expected on a chance basis. It is always possible that such a patient may see through this strategy and respond within the range associated with chance. This report describes one such subject together with an elaboration of the forced choice technique which strongly indicated that his symptoms had no organic basis.
