[Effect of brietal and gamma-OH on the bioelectric activity of the cerebral cortex and deep temporal structures in SEEG studies in patients with epilepsy].

Eleven SEEG investigations were analysed in patients with temporal epilepsy after separate intravenous injection of Brietal and gamma-OH. The authors discuss a modification of the background activity and pathological seizure discharges in the cortex and deep structures of the temporal lobes. Attention is called to an evident increase of the quantitative and qualitative elements of excitation or manifestation of seizure activity not present previously. The activating effect of the these preparations on the pathological bioelectric activity without presence of clinically observable seizures suggests the possibility of existence of inhibitory mechanisms at the levels below the cortex and limbic system, probably in the reticular system of the brain stem. It was found that in the used doses the administration of both preparations was completely safe. Brietal and gamma-OH are good preparations facilitating more accurate diagnosis of epilepsy.

[Temporal lobe epilepsy during the growth stage treated by amygdalotomy and hippocampotomy].

The authors describe the results of stereotaxis treatment of epileptic patients with behaviour disturbances in developmental age. The indications to surgical treatment were established on the basis of stereo-EEG investigations. For the analysis 20 patients were selected and divided into two groups depending on the state of mental efficiency. Surgical treatment included selective amygdalotomy and hippocampotomy in different variants. The analysis of this material showed that the clinical improvement was greatest in patients with short period of epilepsy duration and with IQ above 90. This suggests that patients with temporal lobe epilepsy with behaviour disturbances in developmental age should be referred for stereo-EEG investions and stereotactic treatment possibly early.

[Effect of electric stimulation of the amygdaloid body and hippocampus on bioelectrical activity recorded from these structures and from skull surface in epilepsy].

The authors discuss the morphology of records of spontaneous and stimulated bioelectric activity in an analysis of selected 15 EEG records using electric stimulation of deep temporal structures using relatively low parameters of stimulation (optimal: 2--3 V, 50 Hz, 1 msec, duration 5 sec). Attention is called also to the fact that seizure activity is absent in leads from skull surface during long-lasting bioelectric seizure activity in deep cerebral structures. It is concluded that induction of afterdischarges and analysis of the directions of their spread may be of practical importance from the point of view of diagnosis and therapy. The authors state also that induction of bioelectric afterdischarges in deep temporal structures, particularly in the hippocampus caused in these patients no disturbances of consciousness and memory.

[Effect od amygdalectomy on memory and learning in patients treated surgically for epilepsy].

The results of neuropsychological investigations of 40 patients with severe epilepsy and behaviour disturbances treated by stereotactic methods are presented. In all cases memory disturbances were found before the operation, in 45% of the cases mental sluggishness was present. After amygdalotomy, together with a reduction of emotional tension and psychomotor agitation, the range of direct memory extended and difficulties in learning verbal material decreased. In some patients a tendency was observed also for a rise in intelligence quotient.

[The effect of stereotaxic injuries on the medial temporal brain structures on memorization of successively or simultaneously presented material].

Effect of stereotaxic injuries to the medial temporal brain structures on memorization of material presented successively or simultaneously was studied. The study aimed at testing whether stereotaxic damage to the hippocampus and amygdala results in a memory deficit and whether functions subserved by those structures depend on type and organization of the memorized material. The results indicate that even small damage to the medial temporal lobe structures may result in a deficit in memorization ability. The greatest impairment was observed for successive presentation of nonverbal stimuli.

[Automatic analysis of frequency spectrum in localization of topography of epileptic discharges at early stages of the disease].

The authors present the value of frequency spectrum analysis in the method of cerebral mapping for the diagnosis and localization of epileptic focus. The patient aged 17 years showed atypical clinical symptoms. The study made possible confirmation of epilepsy, demonstration of two independent epileptogenic foci, and correlation of the atypical clinical syndrome with the location of the foci. The results are presented on topographic maps of the brain.

[Somatosensory evoked potentials in a patient with Parkinson's disease before and after cryothalamotomy].

To document changes in SEPs in PD we have chosen a patient with the idiopathic form of the disease. Because of drug intolerance and progressively deteriorating clinical course she was selected for thalamotomy. Before the operation we recorded oscillations in the range of 150-300 Hz superimposed on SEP over the area of the right hand cortical representation, an a low amplitude of the N20-P25-N34 components. Eight weeks after left thalamotomy the amplitude of oscillations recorded over the left hemisphere diminished. A new wave, P65, appeared and similar SEPs were seen in the right hemisphere ipsilateral to the stimulated median nerve. Following stimulation of the left median nerve unchanged SEPs were observed only in the right hemisphere. Sixteen weeks after the operation, SEPs from the left hemisphere had a similar sequence as before the procedure. There was a clear increase in amplitude of long latency waves recorded from the left scalp. Changes in SEP polarity and amplitude following operation are attributed to plastic reorganization of cortical connections.
