The effect of PGE1 and temperature on lung function following preservation.

We studied the effect of a vasodilator (prostaglandin E1) as well as flush (F) and storage (S) temperatures (4 degrees C or 10 degrees C) on lung preservation in an isolated rabbit lung perfusion model. Low-potassium dextran (LPD) or Euro-Collins (E-C) solution was used as flush solution. Six groups of six animals were studied: group 1 (LPD, 4 degrees C F-S), group 2 (LPD with PGE1, 4 degrees C F-S), group 3 (E-C with PGE1, 4 degrees C F-S), group 4 (LPD, 10 degrees C F-S), group 5 (LPD with PGE1, 10 degrees C F-S), group 6 (E-C with PGE1, 10 degrees C F-S). After 18-hr preservation, left lungs alone were ventilated, and reperfused with fresh venous blood. PaO2, PaCO2, pulmonary artery pressure (PAP), tracheal pressure (Pt) during reperfusion, and wet/dry weight (W/D) ratios were measured. PaO2 after LPD with or without PGE1 was significantly higher than after E-C with PGE1 at 4 degrees C (95.8 +/- 11.5 mmHg in group 1 or 102.7 +/- 8.6 in group 2 vs. 41.8 +/- 10.5 in group 3, P less than 0.01) and at 10 degrees C (119.3 +/- 2.3 in group 4 or 131.1 +/- 6.2 in group 5 vs. 54.6 +/- 5.2 in group 6, P less than 0.01). PaCO2, PAP, Pt, and W/D ratios in the LPD groups were lower than in the E-C groups. LPD/PGE1 and LPD alone produced similar pulmonary preservation. PaO2 of lungs flushed with LPD and preserved at 10 degrees C was higher than that of lungs stored at 4 degrees C. We conclude that LPD solution is superior to E-C solution in this ex vivo rabbit lung preservation model, even when PGE1 is used. A moderate dose of PGE1 did not improve the performance of LPD as a flush solution. Pulmonary preservation with LPD at 10 degrees C is superior to preservation at 4 degrees C.

Low-potassium UW solution for lung preservation. Comparison with regular UW, LPD, and Euro-Collins solutions.

University of Wisconsin solution has been used successfully in clinical kidney and liver preservation. The object of this study was to determine if low-potassium UW (LPUW) solution could be applied to pulmonary preservation. Rabbit lungs were stored after hypothermic pulmonary artery (PA) flush with four different solutions (group 1: low-potassium dextran (LPD) solution, group 2: high-potassium UW (HPUW) solution, group 3: LPUW solution, group 4: modified Euro-Collins (E-C) solution). The lungs were preserved at 10 degrees C for 30 hr and evaluated in an ex vivo ventilation/perfusion apparatus using fresh pooled venous rabbit blood. Mean PA flush pressures (MFP) during harvesting were significantly lower in groups 1 and 3 (8.1 +/- 1.0 mmHg and 7.3 +/- 0.6 mmHg, respectively; mean +/- SEM) than in groups 2 and 4 (15.5 +/- 1.7 mmHg and 12.3 +/- 0.9 mmHg, respectively). Lungs in groups 1 and 3 showed significantly higher PaO2 (103.5 +/- 8.0 mmHg and 89.3 +/- 7.2 mmHg) than groups 2 and 4 (48.3 +/- 7.7 mmHg, 66.7 +/- 4.7 mmHg). Groups 1 and 3 showed significantly lower wet/dry weight (W/D) ratios after reperfusion (6.21 +/- 0.15 and 6.39 +/- 0.23) than groups 2 and 4 (7.70 +/- 0.57 and 7.13 +/- 0.21, respectively). There were no significant differences in MFP, PaO2, PaCO2, mean pulmonary artery pressure, or W/D ratio between groups 1 and 3. These results suggest that LPUW solution may be as beneficial as LPD solution for pulmonary arterial flush and lung preservation.

Development of a biotinylated analog of substance P for use as a receptor probe.

The development of a biotinylated substance P (SP) analog for use as a receptor probe is reported. The lysine in position 3 of SP was substituted by arginine and an amino terminal extension (NTE-SP) was added consisting of Lys-Tyr-Gly-Gly-Gly-Gly-Gly-Gly. Biotinylation of the N-terminal lysine was performed. The biotinylated peptide was purified by high performance liquid chromatography and characterized by mass spectral analysis. Binding studies using human IM-9 lymphoblasts with the biotinylated SP analog (biotin-NTE[Arg3]SP) and native SP yielded dissociation curves which were identical. In addition, the biotinylated SP analog retained functional activity similar to that of native SP in altering intracellular calcium concentration of Fura-2 loaded isolated rabbit colonic myocytes. Applicability of the SP receptor probe was demonstrated by using the streptavidin-peroxidase detection system to identify SP receptors on human IM-9 lymphoblasts. In conclusion, a biotinylated SP analog has been developed which retains the functional characteristics of the native peptide and is a useful and versatile probe for receptor studies.

Use of an isolated in situ canine lung perfusion model to evaluate dopamine clearance by the lung.

An isolated in situ lung perfusion model was used to assess dopamine clearance by the lungs in 12 dogs. The preparation consisted of a closed perfusion system in situ, in which systemic arterial blood supply was interrupted. Blood flow to the lungs was maintained at presurgery flow rates. The inflow was restricted to the lungs by the pulmonary arteries and outflow was limited to the pulmonary veins. Pulmonary artery pressure, temperature and pO2 were maintained at physiological levels. After confirmation of a stable base line, one of 3 doses (1, 2, or 5 micrograms/kg/min) of dopamine was infused over 30 min to achieve steady-state blood concentrations, then blood samples were drawn at specified times during and after the infusion. Dopamine plasma concentrations were analyzed by high performance liquid chromatography with electrochemical detection. Dose-dependent disposition of dopamine was observed in both plasma concentration-time profiles and in clearance (20.3 +/- 9.6 ml/min/kg at 5 micrograms/kg/min vs. 41.6 +/- 19.1 ml/min/kg at 1 micrograms/kg/min, P = 0.038). A sham experiment revealed that the blood in this experimental preparation contributed less than 10% to the total clearance of dopamine. This study revealed that our isolated in situ model is an excellent method to evaluate the role of the lungs in drug removal. Furthermore, it confirmed that the lungs contribute to the clearance of dopamine from the body.

A controlled study of early neurologic abnormalities in men with asymptomatic human immunodeficiency virus infection.

BACKGROUND: Although neurologic complications are frequent in the acquired immunodeficiency syndrome, their incidence and progression in early human immunodeficiency virus (HIV) infection remain controversial. The goal of this study was to assess neurologic manifestations in asymptomatic carriers of HIV. METHODS: We studied 29 HIV-seropositive homosexual men and 33 seronegative homosexual controls by means of neurologic and neuropsychological examinations, magnetic resonance imaging (MRI), and electrophysiologic tests (electroencephalography, multimodal evoked-potential tests, and otoneurologic tests). After six to nine months, the tests were repeated in 27 seropositive men and 30 controls. The investigators were blind to the serologic status of the subjects. RESULTS: The seropositive subjects had a mean CD4+ lymphocyte count of 635 X 10(6) per liter. Neurologic and neuropsychological examination, MRI, and measurements of pattern visual evoked potentials did not show significant differences between the two groups. The latencies of the median-nerve somatosensory evoked potentials were slightly prolonged in the seropositive men. The initial electroencephalogram was judged abnormal in 8 of 27 of the seropositive subjects (30 percent) as compared with none of the controls, with a slowing of fundamental activity, anterior spread [corrected] of alpha rhythm, subnormal reactivity, and unusual anterior theta activities. These findings were confirmed by computerized spectral analysis. The second electroencephalogram was abnormal in 10 of 25 of the seropositive men (40 percent). The otoneurologic evaluation identified abnormalities in the central auditory or vestibulo-ocular pathways in 34 percent of the seropositive men (10 of 29), as compared with 6 percent of the controls (2 of 33), on the first examination and in 44 percent (12 of 27) and 7 percent (2 of 30), respectively, on the second examination. Altogether, electrophysiologic abnormalities were found in 67 percent of the seropositive men (18 of 27) and 10 percent of the controls (3 of 30) (P less than 0.00005). CONCLUSIONS: In persons with asymptomatic HIV infection, electrophysiologic tests may be the most sensitive indicators of subclinical neurologic impairment. Electrophysiologic abnormalities are far more common in asymptomatic carriers of HIV than in controls and tend to progress over time.

Autoradiographic localisation of D1 and D2 dopamine receptors in primordial striatal tissue grafts in rats.

Using in vitro autoradiography the study examined the binding of the dopamine (DA) D1 receptor and D2 receptor radiolabelled ligands, [125I]SCH 23982 and [125I]sulpiride, respectively, to striatal sections of control rats, those with unilateral ibotenic acid (IA) lesions of the striatum and those with grafts of primordial striatal tissue implanted into the IA-lesioned striatum. In animals with IA-lesions, there was almost total degeneration of striatal neurones and a complete disappearance of both D1 and D2 receptors in the lesioned striatum. Within the striatal grafts, dense 'patches' of both D1 and D2 receptors were observed with densities comparable to those in the intact neostriatum. These data indicate that both D1 and D2 receptors are localized on the soma of intrinsic striatal neurones. Moreover, the specific localisation of D1 and D2 receptors in the grafts provides the neurochemical basis for the mechanisms by which the host nigrostriatal DA system can regulate the functional capacity of striatal grafts.

[Effect of the composition of the ration during the dry period on fertility and milk production of Holstein cows in Israel].

In each for of kibbutzin herds located in Israel the dry cows were divided in two feeding groups. The experimental group, in totally 266 cows, obtained only oat, barley or wheat hay ad libitum, whereas the control group (n = 253) was fed additionally mix or silage up to four kg dry matter. The cows in the control group responded in the following lactation with a 1.3 kg higher average FCM per day (p = 0.02) and with a 1.7 kg higher maximum FCM (p = 0.01) in comparison to the experimental group. The differences between the experimental and control group in A. I.-parameters days open, conception rate on first insemination and percent problem cows were rather small. However, the frequency of retained placenta and ovary cysts was significantly (p less than 0.05) higher in the control group than in the experimental group. In one herd the influence of body condition between the end of lactation, dry period and 50-80 days p.p. as well as the change of body weight between the end of lactation and calving on milk production and fertility could be evaluated. A decrease of one body condition score unit between the dry period and 50-80 days p.p. was associated with an increase of FCM by 2.01 kg (p less than 0.01), a decrease of the conception rate by 21% (p less than 0.05) and an elongation of the days open by 23.6 days (p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Regulation of intracellular calcium in epithelial cells.

The role of [Ca2+]i as a second messenger in non-excitable cells has been appreciated for almost 3 decades. The advent of fluorescent Ca2+ indicators has allowed the monitoring of Ca2+ signalling in suspensions of these cells. Agonist mediated changes in [Ca2+]i usually show an initial Ca2+ transient followed by a maintained increase. The former has been shown to be due to Ca2+ release from one or more intracellular stores, the latter due to activation of receptor operated Ca2+ entry (ROCE). More recently it has been recognized that many cells show distinct maintained oscillatory behavior when examined by single cell optical methods. It is proposed here that these oscillations are the consequence of IP3 and Ca2+ stimulation of Ca2+ release and ligand activation of ROCE followed by Ca2+ inhibition of Ca2+ and ROCE as Ca2+ pumps are activated. These oscillations allow more exact regulation of a pump/leak controlled second messenger such as [Ca2+]i.

[Treatment of diabetes with insulin pumps. Current status and insurance medicine aspects].

The continuous subcutaneous insulin infusion (CSII) by means of portable dosing devices ("insulin pumps") represents a new therapeutical principle in the treatment of type I diabetics. Developed in 1978/79, subcutaneous infusion is being used increasingly in today's routine therapy. It achieves an almost physiological distribution of insulin between a basal rate (basic insulin requirement) and additional rates (insulin requirement depending on food-intake), leading to an improvement not only in metabolism but also in the quality of patient's life (increased flexibility in everyday life). The prerequisites for successful therapy are stated and therapy-specific complications are analysed on the basis of the experience of the III. Medical Department of the City Hospital Munich-Schwabing with 104 patients over an accumulated period of 283 treatment years. An outline is given of future advancements in the field of controlled insulin dosage with the help of infusion systems. Important aspects of insurance medicine are pointed out, such as the registration of substandard risks by means of a questionnaire especially developed for that purpose and the prognostic evaluation as compared to forms of therapy applied so far. The reasons for the current classification of this new therapeutical method as prognostic/experimental are indicated and the procedure for selecting favourable risks is described.

Nephrotoxicity screening in rats: a validation study.

A validation of our non-invasive screening test for the detection of renal damage (Zbinden et al. 1988) is presented. The test is based on repetitive, quantitative urine analysis in groups of six female Sprague-Dawley rats treated on 5 consecutive days with low doses of test substances. Higher doses were administered in the following weeks until nephrotoxic effects or signs of general toxicity were observed. Thirteen reference substances (hexachloro-1:3-butadiene [HCBD], cisplatin, carboplatin, suramin, chloroform, neomycin, rifampicin, phenacetin, phenylbutazone, methicilline, sodium oxalate, ethylene glycol and furosemide) were used. The percentage of rats reaching the test criteria, i.e., pathologic values defined on the basis of measured control values, was determined. In the controls, the overall percentage of rats reaching or exceeding the test criteria was 4.48%, a value that is close to the expected 5%. Evidence of nephrotoxicity was found with all reference compounds. Elevated excretion of cells and occurrence of cylinders were the most sensitive indicators of renal damage. Hematuria was the most frequent finding. Of the other urine constituents measured the enzyme malate dehydrogenase (MDH) was frequently increased. Water consumption, urine volume, pH and specific gravity were occasionally, and protein, glucose, electrolytes, amino acids and gamma-glutamyl-transpeptidase (GGT) were only rarely changed. It is concluded that the screening which is based on quantitative and repeated urine analysis is a useful procedure to detect nephrotoxic chemicals acting by a variety of mechanisms. The histopathological examination of the kidneys contributed useful information of the nature of the toxic effects, but as a screening tool it is less sensitive than quantitative urine analysis.

Tactual extinction without anomia: evidence of attentional factors in a patient with a partial callosal disconnection.

Tactual extinction was examined in a 43-yr-old left-handed male patient (MM) with a hematoma in the trunk of the corpus callosum sparing the genu and the ventral part of the splenium as well as the cerebral hemispheres. Experiments I and II showed no deficit in tactual perception nor in the inter-hemispheric transfer of tactual information. In Experiment III, MM palpated stimuli differing in their association value, and responded vocally or manually. There was no difference in performance between the hands in unimanual control conditions, but in a dichhaptic condition an overall right-hand advantage was observed, depending on the kind of stimulus and on the mode of response: such results were not observed in control subjects. Results showed that MM's left-hand performance reached right-hand levels after specific modifications in the allocation of attention between the hands or the load of information in haptic memory (Experiment IV). Experiment V revealed a major deficit in the matching of left- and right-hand information only for the dichhaptic procedure. The results are discussed with a view of cerebral mechanisms as dynamic rather than structurally determined: it is suggested that extinction phenomena are sensitive to cognitive strategies and attentional factors and that the corpus callosum plays a critical role in the lateral control of attention to tactual as well as to auditory and visual information.

Analysis of fertility indices of cows with extended postpartum anestrus and other reproductive disorders compared to normal cows.

An analysis of computerized data on the incidence and consequences of reproductive disorders was performed on 84,818 lactations in more than 100 high producing Kibbutz dairy herds during six semiannual recording periods. The average incidence of primary reproductive disorders, which included retained placenta, abnormal lochia, endometritis, postpartum anestrus, cystic ovaries, negative pregnancy examinations and repeat breeders, was 50.4%. During all recording periods we were able to observe recurrence and repeatability of these reproductive disorders and of the fertility indices consequently seen. Cows with uterine infections diagnosed and treated before Day 20 post partum had better fertility indices than those diagnosed and treated later. The conception rate to first inseminations and the overall conception rate to all inseminations of cows with postpartum anestrus was significantly lower, while the percentage of "problem cows" (not pregnant by Day 150 post partum) was significantly higher, than in normal cows. The incidence of cystic ovaries in cows with a previous diagnosis of reproductive disorders was 2 to 3 times higher when compared to cows without such a diagnosis.

Analysis of fertility indices of cows with reproductive disorders and of normal cows in herds with low and normal fertility.

Semiannual computer printouts of reproductive disorders diagnosed in high producing Israeli dairy herds were evaluated for a comparison of fertility indices of diseased and normal cows in low (LFH) and normal fertility herds (NFH). In 16,910 cows, conception rates to first AI in cows with reproductive disorder were >50% below those in normal cows (P<0.001). For both groups of cows, conception rates were lower in LFH (P<0.0001). A comparison of fertility indices between LFH (5, 171 cows) and NFH (14,573 cows) revealed, in addition a higher overall conception rate and a lower percentage of cows still open on Day 150 post partum (problem cows, (P<0.0001), as well as a higher pregnancy rate for NFH (P<0.001), for cows with reproductive disorder and for normal cows in these herds. In herds with predominantly uterine-related reproductive disorder, a high incidence of postpartum anestrus was seen infrequently; while in herds with a high incidence of postpartum anestrus, postbreeding anestrus was equally high (P<0.05 - 0.001). In general reproductive disorder have been shown to have long-term negative effects on fertility indices in both LFH and NFH, but with more serious consequences in LFH. Since a sizeable percentage of normal cows can be identified, reproducing normally under identical management conditions, cows in both kinds of herds with reproductive disorder must be less resistant to environmental stress. Thus, while cows with reproductive disorders maintained their general health and their high level of productivity, they responded either immediately with uterine disturbances or they refused to cycle properly, and they conceived later. A case can be made that all reproductive disorders represent disease entities influenced by management factors.

Solid phase synthesis and characterization of two canine gut gastrin-releasing peptides.

Two canine gastrin-releasing peptides originally isolated from gut tissue extracts have been synthesized by solid phase methodology and purified by preparative reverse phase high performance liquid chromatography (RP-HPLC). The synthetic gastrin-releasing peptides GRP1-27 and GRP 5-27 were characterized with regard to homogeneity and composition using nine different RP-HPLC systems, mass spectroscopy, amino acid analysis, Edman degradation, methionine oxidation, and peptide mapping with tryptic, Staph. aureus V8 protease and cyanogen bromide cleavage (the latter two systems performed only with GRP 1-27). Although a scarcity of the natural products prevented quantitative biological comparison of the synthetic and natural peptides, they were found to elute identically on RP-HPLC co-chromatography and similar dose dependent biological potencies were observed in canine antral muscle tissue contraction experiments. Indeed, all the peptides containing the bombesin-like carboxyl terminal decapeptide sequence studied to date have similar biological activities.

Prevention of supraventricular tachyarrhythmias post coronary artery bypass surgery.

In a randomized prospective study 32 patients received either alinidine or a placebo for the first five postoperative days after coronary bypass surgery. The purpose of the study was to investigate the prophylactic antiarrhythmic properties of alinidine on supraventricular tachyarrhythmias (svt), which occur with incidence after open heart surgery. There was no significant difference in pretherapeutical parameters between the two groups. Eleven out of sixteen control patients (69%) and none of the patients treated with alinidine had svt. All arrhythmias occurred in the first three postoperative days and required medical treatment. Even after alinidine was stopped, patients in this group did not experience arrhythmias. The mean systolic blood pressure in the treatment group was 113 +/- 13 mmHg, in the control group it was 119 +/- 16 mmHg. The mean heart rate tended to be lower in the alinidine group (82 +/- 12 beats min-1 91 +/- 21 beats min-1. In 1/16 patients the alinidine treatment was stopped due to marked hypotension (less than 90 mmHg) and bradycardia (less than beats min-1). Two other patients in this group had short periods of mild bradycardia (less than 60 beats min-1) which was tolerated well. Additional medical treatment was not needed. In this study prophylactic treatment with alinidine proved to be highly effective in preventing postoperative arrhythmias following myocardial revascularisation.

Adenosine inhibits exocytotic release of endogenous noradrenaline in rat heart: a protective mechanism in early myocardial ischemia.

The effects of exogenous and endogenous adenosine on exocytotic noradrenaline release were studied in rat hearts perfused in situ. Exocytotic release of endogenous noradrenaline (determined by high pressure liquid chromatography) was induced by electrical stimulation of the left cervicothoracic ganglion. Exogenous adenosine significantly reduced noradrenaline overflow from the heart. This suppression of noradrenaline overflow was not influenced by desipramine, indicating a mechanism independent from noradrenaline reuptake. The A1 subtype specific agonists cyclohexyladenosine and R-phenylisopropyladenosine had inhibitory effects at lower concentrations than adenosine and S-phenylisopropyladenosine, suggesting the relevance of presynaptic inhibitory adenosine receptors of the A1 subtype. Short ischemic periods of 3 minutes resulted in a marked coronary venous overflow of adenosine during reperfusion. This was accompanied by an inhibition of noradrenaline release evoked by nerve stimulation during ischemia. The adenosine antagonists theophylline and 8-phenyltheophylline prevented this suppression of noradrenaline release. Blockade of oxidative phosphorylation by cyanide in combination with glucose-free perfusion induced an increased formation of endogenous adenosine and suppression of stimulation-evoked noradrenaline overflow. Again, in the presence of the adenosine antagonists theophylline or 8-phenyltheophylline, this suppression was abolished. These results indicate that adenosine is a potent inhibitor of exocytotic noradrenaline release in the heart with relevance during conditions of increased endogenous adenosine formation such as myocardial ischemia.
