The prognostic value of the noninvasive vascular laboratory in autologous vein bypasses of the lower extremities.

A retrospective study of 34 patients with 29 autologous vein femoropopliteal and 8 femorotibial bypasses was conducted, examining the correlation of angiographic runoff and non-invasive flow determinants as prognostic indicators of early graft occlusion of lower limb bypass grafts. The followup period was 3-27 months, with a mean of 5.8 months. Graft patency and improvement in presenting symptoms were unrelated to preoperative ankle systolic pressure indices. Preoperative and postoperative flow measurements were similar in patients with patent grafts (greater than 12 months) and in those with early graft occlusion (0-6 months, p greater than .05). Preoperative pressure indices did not correlate with calf vessel runoff (p. greater than 05). The data suggest: (1) the noninvasive flow studies are not reliable predictors of future graft patency, (2) vessel runoff is not a reliable predictor of limb flow, and (3) bypass procedures in the lower extremities should not be excluded on the basis of noninvasive flow studies when indicated by other clinical parameters.

[Antral gastritis in chronic alcoholism. Role of cirrhosis and Helicobacter pylori].

OBJECTIVES: Antral gastritis is frequent in alcoholics. The role of H. pylori in the pathogenesis of gastritis in these patients is not well known. The aim of our study was to study the role of H. pylori and cirrhosis in the pathogenesis of antral gastritis in alcoholic patients. METHODS: Seventy-nine patients were included in the study. All underwent upper gastrointestinal tract endoscopy with antral biopsies, independently of the presence of abdominal pain, and had serological examination for H. pylori antibodies. RESULTS: Cirrhosis and gastritis were present in 50 and 40 patients respectively, H. pylori serological assay and histological identification of the bacterium were positive in 35 (44%) and 19 (24%) patients respectively. Discrepancy between the 2 tests were observed more frequently in cirrhotic patients. A positive serology with a negative histologic examination for H. pylori was present for 18 cirrhotic and 4 noncirrhotic patients (p < 0.05). A gastritis without evidence of H. pylori was more frequent in cirrhotic than in noncirrhotic patients. H. pylori was histologically present in 11 of 29 cirrhotic patients and in 8 of the 11 noncirrhotic patients with a gastritis (p < 0.05). CONCLUSIONS: Discrepancies between histological examination and H. pylori serology in patients with cirrhosis might be due to the inhospitable environment for H. pylori in case of portal hypertension; the positive serology could be in relation with a past infection.

NVQs as part of the pre-registration diploma.

National Vocational Qualifications (NVQs) seem to have taken hold of the popular imagination as somehow being more relevant to practical work than many other forms of nurse education. At one time NVQs were seen as having very low status, however, with the opening up of levels 3 and above, the practical benefits of professionals being able to demonstrate competence have started to gain not only popular, but also academic recognition. This paper demonstrates how one college of nursing has made a formal link between the NVQ scheme and higher education by preparing its Project 2000 students to be able to achieve a diploma of higher education and a respected National Vocational Qualification.

[Production of oxygenated free radicals and the role of exogenous antioxidants in liver transplantation in rats].

It has been suggested that, in transplantation organs, the lesions observed after conservation then reperfusion could be related to the formation of oxygenated free radicals. The aim of our work was first to verify the hypothesis that oxygenated free radical are formed after ischaemia-reperfusion of the liver, then to evaluate the effects of the allopurinol and glutathion, known antagonists of oxygenated free radicals, contained in the University of Wisconsin conservation fluid. The University of Wisconsin solution was compared with a Collins solution which does not contain oxygenated free radical antagonists. An orthotopic liver transplantation model was used in Wistar rats. Three groups were studied. In the control group, 5 rats underwent laparotomy then were closed with no surgery being performed. In the Wisconsin group (n = 6) and the Collins group (n = 6) the livers were washed and conserved in the corresponding solution at 4 degrees C before transplanting. Plasma levels of malonyldialdehyde, measured by high performance liquid chromatography, were used as a marker for the formation of oxygenated free radicals. Impaired liver function was assessed on the basis of mortality and serum transaminases, alkaline phosphatase and total bilirubin levels. Liver biopsy was performed at sacrifice. The level of malonyldialdehyde was significantly higher in the transplanted groups compared with the control group (p < 0.01). There was no difference between the Wisconsin and the Collins groups. Hepatic function was significantly reduced in the transplanted groups compared with the control group (p < 0.05). [Serologic study for detecting anti-Leptospira antibodies in Rattus norvegicus from Duque de Caxias, Rio de Janeiro, Brazil].

Due to the high incidence of leptospirosis human cases in Duque de Caxias, state of Rio de Janeiro, the authors captured 47 rats (Rattus norvegicus) in order to search agglutinins on them. Through serological reactions, 17 positive animals (36.17%) were identified, being 16 to the serovar icterohaemorrhagiae and 1 to the serovar canicola, with titers from 1/100 to 1/400.

A systematic overview of the effectiveness of home visiting as a delivery strategy for public health nursing interventions.

The purpose of this systematic overview was to assess the evidence for the effectiveness of public health nursing interventions when carried out by the strategy of home visiting. A search of published and unpublished literature resulted in retrieval of 108 articles; 77 articles were judged to be relevant. Validity criteria included method of allocation to the study groups, level of agreement to participate in the study, control for confounders, method of data collection (pretesting of data collection tools, blinding of data collectors to group allocation of study participants), quantitative measure of effect and percentage of participants available at follow-up. Using these criteria, 9 articles were judged to be strong, 5 moderate and 63 weak. There were no reported negative effects of home visiting in the 9 strong articles. Positive outcomes included improvement in children's mental development, mental health and physical growth; reduction in mother's anxiety, depression, and tobacco use; improvement in maternal employment, nutrition and other health habits; government cost saving; and reduced level of care required for the elderly.

[Chronobiology in medicine and surgery].

Chronobiology studies predictable variations, rhythms and trends in all forms of life. Each physiologic variable or system in an organism has a time structure which is genetically anchored and capable of being expressed in the absence of external cycles. However, it is normally adjusted by geophysical changes. Chronobiology quantifies rhythms by providing measures of amplitude and timing (phase and period), and a mean more precise than the arithmetic mean. The correct measurement of biological rhythms has led to considerable advances in the understanding of their underlying mechanisms of generation and expression. The use of information on rhythms is opening new opportunities in the prevention, diagnosis and treatment of disease. Chronobiology is also effective in the improvement of the quality of life by teaching self-help to the healthy individual.
