High prevalence of folic acid and vitamin B12 deficiencies in infants, children, adolescents and pregnant women in Venezuela.

BACKGROUND: There is increased worldwide concern about the consequences of folic acid and vitamin B12 deficiencies on health, which include megaloblastic anemia, neural tube defects and cardiovascular disease. OBJECTIVE: This study intended to determine the prevalence of folic acid and vitamin B12 deficiencies in vulnerable groups in labor and poor socioeconomic strata of the Venezuelan population. METHODS: A total of 5658 serum samples were processed to determine folic acid and vitamin B12 concentrations. The study involved three surveys performed during 2001-2002 and included infants, children, adolescents and pregnant women from labor and poor socioeconomic strata of the population. The method used was a radio immunoassay designed for the simultaneous measurement of serum folic acid and vitamin B12. RESULTS: The prevalence of folic acid deficiency was higher than 30% for all groups studied, reaching 81.79% in adolescents. Vitamin B12 deficiency was 11.4% in samples collected nationwide, but there was also a similar prevalence of high serum levels. The prevalence of folic acid and vitamin B12 deficiencies in pregnant women reached 36.32 and 61.34%, respectively. CONCLUSION: This work shows that there is a high prevalence of folic acid deficiency, especially in women of reproductive age, pregnant adolescents and in the whole population studied in Vargas state. This situation requires immediate intervention as supplementation or food fortification programs.

[Acute confusional syndrome in elderly patients hospitalized due to medical condition].

INTRODUCTION: Delirium or acute confusional syndrome (ACS) is a frequent problem during hospitalization of elderly patients. We study the appearance of delirium and its characteristics in patients admitted to an internal medicine service. METHODS: Prospective study of 148 patients over 64 years admitted due to medical condition (non-surgical) in the internal medicine service of the University Hospital of Bellvitge. Functionality was quantified with the Barthel index (BI) and comorbidity with the Charlson index (CI). The Confusional Assessment Method was used for the diagnosis of the ACS. RESULTS: Seventy-seven (77) (52%) of the 148 patients were women, with a mean age of 78.5 years. The CI was 2.2. Mean of previous BI was 81.7. Mortality during admission was 8% (12 patients). A total of 42.5% of the patients (63) had ACS (30% prevalent). It was hyperactive in 68%, hypoactive in 16% and mixed in 16%. In 38% of the patients, the ACS had morning predominance and 62% it appeared after the evening. Advanced age was the only significant differences between patients with or without ACS (p < 0.001). There were no significant differences in the ACS percentage between patients who survived or who died (p = 0.36). CONCLUSIONS: Frequency of appearance of the confusional picture is high in patients admitted to acute hospitals, it being more frequent in the elderly. Measures to prevent the confusional picture in elderly patients who are hospitalized should be increased.

Sperm characteristics of endurance trained cyclists.

The purpose of this study was to investigate the influence of cycling on sperm characteristics. Twenty subjects volunteered for the study, comprising 10 long distance competitive cyclists (median 25.5 y) and 10 sedentary controls (median 24.5 y). A questionnaire was used to obtain a history of training, health, nutrition and life style. Semen analysis consisted of a detailed evaluation of sperm characteristics that included semen volume, sperm count, viability, motility and morphology. Compared to controls, the cyclists had a significantly lower proportion of spermatozoa with normal morphology (medians: 41.5 % versus 19.5 %; p < 0.01) and a significantly higher proportion of morphologically abnormal tapered forms (medians: 4.5 % versus 22.5 %; p < 0.01). No significant difference in semen volume and sperm motility, viability and count was observed between the two groups. We concluded that endurance cycling appears to be associated with a significant alteration in sperm morphology.

Prior aspirin users with acute non-ST-elevation coronary syndromes are at increased risk of cardiac events and benefit from enoxaparin.

BACKGROUND: The aim of this article was to investigate whether prior aspirin use in patients with acute coronary syndromes affects clinical outcome. The Efficacy Safety Subcutaneous Enoxaparin in Non-Q-Wave Coronary Events Study (ESSENCE) and Thrombolysis in Myocardial Infarction (TIMI) 11B trials have shown superiority of enoxaparin over unfractionated heparin (UFH) in patients with unstable angina and non-ST-segment elevation myocardial infarction (UA/NSTEMI). However, the treatment effect of enoxaparin in the subset of patients reporting prior aspirin use has not been determined. METHODS: The rate of death, myocardial infarction, and urgent revascularization at days 8 and 43 after randomization was compared among patients who received aspirin within the week before randomization with those who did not receive aspirin in the TIMI 11B trial. A total of 3275 patients (84%) were prior aspirin users. RESULTS: The admission diagnosis was similar for prior and nonprior aspirin users. At both day 8 and day 43 the event rate was higher for prior aspirin users than for nonprior aspirin users (odds ratio 1.6 [1.24-2.08], P =.0004 at day 43), even after correction for baseline characteristics. Compared with those prior aspirin users taking UFH, enoxaparin-treated prior aspirin users had a reduced rate of the composite end point of death, myocardial infarction, and urgent revascularization at day 8 (odds ratio 0.82 [0.67-1.00], P =.046) and day 43 (odds ratio 0.83 [0.70-0.98], P =.032). CONCLUSION: Patients with UA/NSTEMI and prior aspirin use had a 60% higher risk of death and cardiac ischemic events compared with nonprior aspirin users. On the basis of this subanalysis, enoxaparin is superior to UFH in all patients. In prior aspirin users the benefit is more clearly demonstrated.

Mutations in LHX3 result in a new syndrome revealed by combined pituitary hormone deficiency.

Combined pituitary hormone deficiency (CPHD) has been linked with rare abnormalities in genes encoding transcription factors necessary for pituitary development. We have isolated LHX3, a gene involved in a new syndrome, using a candidate-gene approach developed on the basis of documented pituitary abnormalities of a recessive lethal mutation in mice generated by targeted disruption of Lhx3 (ref. 2). LHX3, encoding a member of the LIM class of homeodomain proteins, consists of at least six exons located at 9q34. We identified a homozygous LHX3 defect in patients of two unrelated consanguineous families displaying a complete deficit in all but one (adrenocorticotropin) anterior pituitary hormone and a rigid cervical spine leading to limited head rotation. Two of these patients also displayed a severe pituitary hypoplasia, whereas one patient presented secondarily with an enlarged anterior pituitary. These LHX3 mutations consist of a missense mutation (Y116C) in the LIM2 domain at a phylogenetically conserved residue and an intragenic deletion predicting a severely truncated protein lacking the entire homeodomain. These data are consistent with function of LHX3 in the proper development of all anterior pituitary cell types, except corticotropes, and extrapituitary structures.

Kinetics of plasma potassium concentrations during exhausting exercise in trained and untrained men.

The purpose of this study was to examine the time course of changes in plasma potassium concentration during high intensity exercise and recovery in trained and untrained men. The subjects performed two exercise protocols, an incremental test and a sprint, on a cycle ergometer. A polyethylene catheter was inserted into the antecubital vein to obtain blood samples for the analysis of plasma electrolyte concentrations and acid-base parameters, during and after exercise. During both tests, venous plasma sodium, potassium and chloride concentrations increased in all the subjects, although the largest relative increase was detected in potassium concentration--35% and 31% over rest in the progressive test and 61% and 37.7% in the sprint test, for cyclists and controls, respectively. After exercise plasma potassium concentration decreased exponentially to below resting values. There was a linear correlation between the amount of potassium accumulated in plasma during exercise and the amount eliminated from plasma when the exercise ceased. We found that, although plasma potassium accumulation occurred in both forms of exercise in the trained and nontrained subjects, the time constant of potassium decrease following exercise was shorter in the trained subjects. Thus, the trained subjects exhibited a better capacity to recover to resting concentrations of plasma potassium. We propose that the extracellular potassium accumulation acts as a negative feedback signal for sarcolemma excitability depending on the muscle metabolic rate.

Developmental changes in plasma apolipoproteins B and A-I in fetal bovines.

Plasma lipid, apolipoprotein (apo) B and apo A-I concentrations were measured in fetal calves in the age range of 4-9 months. The results were compared with those obtained from preruminant calves and cows. The studied lipids and apolipoproteins underwent quantitative underwent quantitative changes during development. Fetal calves, as compared to preruminant calves and cows, had a particularly low apo A-I concentration and a high apo B/apo A-I ratio. Unlike adult animals which have two forms of apo B, fetal calves had only high molecular weight apo B (apo Bh). During fetal development triglyceride, cholesterol and apo B concentrations decreased, but apo A-I concentrations remained constant.

Serum apolipoproteins B and A-I and naturally occurring fatty liver in dairy cows.

Serum lipids and apolipoprotein (apo) B and A-I concentrations were determined in 164 dairy cows which had undergone liver biopsy in early lactation. The animals were divided into groups according to fatty liver severity on the basis of hepatic triglyceride content. The serum free fatty acid (FFA) concentration was higher in cows that developed fatty livers than in normal cows, and it correlated highly with liver triglycerides. Serum total cholesterol and triglyceride levels did not correlate with hepatic triglycerides. Both apo B and apo A-I levels were significantly decreased in fatty liver cows. In particular, apo B levels showed a strongly negative correlation with liver triglycerides. The present results suggest that hepatic apolipoprotein synthesis is impeded in fatty liver cows.

Plasma lipoproteins in dairy cows with naturally occurring severe fatty liver: evidence of alteration in the distribution of apo A-I-containing lipoproteins.

The relationships between fatty liver in dairy cows and reduced levels of plasma lipoproteins, and particularly of low density lipoproteins (LDL), has been previously described. Since electrophoretic heterogeneity of ultracentrifugally isolated LDL (d, 1.006-1.063 g/ml) has been found, the exact nature of this reduction in cows with fatty liver was investigated. Lipoproteins from control and severely afflicted animals were isolated by ultracentrifugation and affinity chromatography on heparin-Sepharose CL 6 B. Gradient gel electrophoresis of lipoproteins on 4-30% gels and an immunolocalization study of apoprotein A-I (apo A-I) showed that control animals have two subpopulations of apo A-I-containing particles with a mean radius of 6.52 and 5.05 nm. In the fatty liver cows, the former was clearly shifted toward smaller particles. We concluded that the depressed level and compositional modifications of LDL in severe fatty liver cows result from a decrease in the oversized apo A-I-containing lipoproteins which can be isolated in the LDL density range. This could stem from the decreased supply of triglyceride-rich lipoprotein surface components for the production of these lipoproteins. The modifications can be plausibly explained by a reduced synthesis or secretion of very low density lipoproteins (VLDL) by the liver.

Quantitative determination of apolipoprotein B in bovine serum by radial immunodiffusion.

1. In the present study we describe the first quantitative determination of apolipoprotein B (apo B) in bovine serum. 2. Immunosera and standard for radial immunodiffusion assay were raised using a purified apo B-containing lipoprotein obtained by ultracentrifugation and affinity chromatography. The assay working range was linear over the concentration of 4-70 mg/100 ml of protein of apo B standard. 3. Contrary to humans, bovine serum contains a low level of apo B-containing lipoproteins. The mean level of apo B in serum samples from cows at various stages of lactation was 16.0 +/- 0.6 mg/100 ml.

Quantification of apolipoprotein A-1 in cow serum by single radial immunodiffusion.

A single radial immunodiffusion assay was developed to determine the concentration of the major apolipoprotein A-I in bovine serum. Assay was performed using agarose gel plates with monospecific rabbit antiserum. Standards made with bovine high density lipoproteins or serum 10 times diluted were applied in 3-microliters amounts to each well in the gel. The assay working range was 2 to 22 mg of apolipoprotein A-I. The intraassay and interassay CV were 3.7 and 4.9%, respectively. Using protein A-I. The intraassay and interassay CY were 3.7 and 4.9%, respectively. Using radial immunodiffusion assay, 23 serum samples from cows at various stages of lactation were analyzed. Apolipoprotein A-I was significantly increased in midlactation compared with concentrations in early lactation or dry period.

[The characteristics of arterial hypertension in the population of a preventive medicine service].

Of 2,115 persons who were submitted to a voluntary check-up, we found 10.6% suffering from hypertension and 16.2% with borderline hypertension, the latter group having been followed and the condition subsequently confirmed in 45.9% of the case. The prevalence increases with age. There were no prevailing differences found between rural and city populations. The prevalence is greater in self-employed persons that in salary workers and we would bring to the fore the higher prevalence in housewives (p less than 0.0005). 74.8% of the hypertensive patients knew of their situation but only 15.6% of them showed normal blood pressure at the following-up. Rarely 0.85% had severe hypertension and 3% had moderate hypertension.

[Tooth mobility and containment].

Periodontal destruction often leads to tooth hypermobility. In the presence of jiggling forces, traumatic occlusion can provoke a progressive mobility, and in certain cases, accelerate the periodontal destruction in presence of periodontitis. Treatment of infection alone is enough to stop the periodontal destruction. If the mobility becomes progressive, or if the hypermobility impedes the masticatory function of the patient, an occlusal adjustment and/or a splint are valuable to assure the longevity of a tooth. Tooth mobility is the physiological adaptation of the periodontium to the traumatic occlusion, and is, in itself, not an indication for splinting.
