Combined treatment with ranitidine and saline antacids prior to obstetric anaesthesia.

Ranitidine 150 mg was given to 126 patients requiring elective Caesarean section under general anaesthesia: 43 women had ranitidine alone, 43 had this supplemented by a pre-induction dose of sodium citrate and 40 patients had ranitidine plus sodium bicarbonate. All three sub-groups provided satisfactory gastric pH and volume. Ranitidine 150 mg was given orally every 6 hours to women in labour. Of 221 patients requiring general anaesthesia during labour, 103 women received 30 ml 0.3 M sodium citrate and 118 women, 20 ml of 8.4% sodium bicarbonate 10 minutes before induction of anaesthesia. In the citrate sub-group there was one patient with a gastric pH less than 2.5 (mean pH 6.2, SEM 0.13 range 2.1-8.4). In the bicarbonate sub-group the lowest gastric acidity was 3.8 (mean pH 8.3, SEM 0.11 range 3.8-9.83).

Systemic lupus erythematosus and abdominal pain.

Abdominal pain is a common symptom in systemic lupus erythematosus. We report two patients with recurrent severe attacks of abdominal pain, often necessitating hospital admission, which were not diagnosed until laparotomy. In one an active arteritis of the mesenteric arteries was seen, with infarction of the jejunum and ileum, and in the second infarction of the ileum with haemorrhage and conglutination of red cells, but no true arteritis.

Cancer in Crohn's disease--an 'occult' malignancy.

The purpose of this case report of cancer complicating Crohn's disease is to highlight the occult nature of these tumours. From the literature it is apparent that the majority are only discovered by the pathologist. As many are inconspicuous even on macroscopic examination an improved method of dissection is suggested.

Chemotherapy of chemically-induced colorectal tumours.

Benign and malignant tumours were induced in the large bowel of rats by the carcinogen 1,2-dimethylhydrazine (DMH). Benign tumours appeared from week 20 onwards and malignant tumours from week 30. 5-Fluorouracil was administered intraperitoneally in maximally tolerated doses from week 26 onwards and failed to influence significantly the development of either benign or malignant tumours. Tumours induced by DMH are a close model of human colorectal cancer and suitable for testing anticancer drugs. Agents with greater activity against colorectal cancer are required.

Lethal, neonatal, short-limbed platyspondylic dwarfism. A further variant?

A stillborn short-limbed platyspondylic dwarf is described. Detailed analysis of the X-ray and histological characteristics of bone and cartilage suggest that this infant represents a previously unreported variant of short-limbed platyspondylic dwarfism. The designation "Luton variant" is suggested.

Comparison of recommended respirable mass dust sampling devices.

The draft, "Revised Guide for Respirable Mass Sampling," prepared by the AIHA-ACGIH Aerosol Hazards Committee, recommends five respirable mass sampling devices that sample in accordance with the "Los Alamos" respirable dust criterion. The spectrum of recommended sampling rates for these devices ranges from 1.8 to 430. liters per minute. A study was conducted to determine if simultaneous measurements with these devices yielded comparable respirable mass concentrations. The penetration characteristics of the first stage separators were also evaluated and compared.
