"Flip-flap" mastopexy.
The "flip-flap" mastopexy modifies the McKissock vertical bipedicle design by creating a wide superiorly based flap of breast tissue deep to the vertical bipedicle. Transposition of this flap up and beneath the upper breast, with suture anchoring to the pectoralis fascia at the level of the second rib, restores upper breast fullness, decreases mastopexy-wrecking lower breast bulk, and provides a pleasant forward thrust of the nipple-areolar complex. The "flip-flap" is effective for improving the long-term aesthetic outcome for both reduction mammaplasty and mastopexies of moderate- to full-sized breasts.
Technique for correction of the retracted columella, acute columellar-labial angle, and long upper lip.
The excess tissue from upper lip vertical length reduction provides bulk for opening the columella-labial angle and increasing the visibility of the columella and upper lip vermillion. Decorticated centrally based transverse flaps from the lip and nostril floor, when transposed into a columellar pocket, correct commonly combined degenerative or developmental deformities. The technique delivers premaxilla, columella, and columella-labial angle mass, which simultaneously corrects the retracted columella, opens the columella-labial angle, shortens the upper lip length, and enhances vermillion visibility.
Psychosocial consequences of sterilization: a review of the literature and preliminary findings.
Tubal ligation continues as a frequently used method of birth control. Nonetheless, psychosocial consequences, sexual satisfaction, and later regret regarding the procedure and the loss of childbearing potential remain as concerns. This is particularly true for the population of childless women requesting the procedure. The authors review all available recent literature on regret, psychosocial correlates, and sexual satisfaction related to tubal sterilization, with particular attention to the literature on childless women. They also present preliminary findings from a prospective controlled 5-year follow-up study of sterilization in childless women.
Familial subtypes of depression: a longitudinal perspective.
An 11-year follow-up of depressed women systematically interviewed and classified by familial subtypes during hospitalization tended to confirm findings of a gradation in severity of illness with depressive spectrum disease being a milder form. Depressive spectrum subjects had shorter subsequent episodes of depression than the other depressives and were less likely to require hospitalization for depression. Depressive spectrum subjects were at greater risk than the others for the development of alcoholism and they experienced more social maladjustment, which suggests implications for treatment strategies and future research.
Corticotropin releasing factor induction of leukocyte-derived immunoreactive ACTH and endorphins.
Human peripheral leukocytes infected by virus or treated with endotoxin will, like unstimulated mouse spleen macrophages, synthesize immunoreactive corticotrophin (ir-ACTH) and endorphins. The ir-ACTH produced appears to be identical with authentic ACTH, while enough of the material has been produced in hypophysectomized mice infected with virus to demonstrate a steroidogenic response. Because the production of ACTH by in vivo pituitary cells and by leukocytes is suppressed by dexamethasone both in vitro and in vitro, suggesting that the production of ACTH and endorphins by leukocytes is indeed controlled, we have investigated the effects of corticotropin releasing-factor (CRF), which is known to regulate the pituitary production of both ACTH and beta-endorphin. We now report that the production of ACTH and endorphins by leukocytes is indeed induced by synthetic CRF and, in turn, suppressed by dexamethasone, suggesting that, as in pituitary cells, the proopiomelanocortin (POMC) gene may be expressed and similarly controlled in leukocytes.
