Apparent or true neonatal hip dislocation? Radiologic differential diagnosis.

A case of neonatal separation of the proximal femoral epiphysis secondary to obstetric trauma is presented. The radiologic differential diagnosis, as in other cases of neonatal dislocation of the hip, included congenital and septic dislocation of the hip and epiphyseal separation - so-called apparent dislocation. When clinical and laboratory signs are minimal or equivocal, x-ray films and, in difficult cases, contrast arthrograms are needed for an accurate diagnosis of neonatal dislocation of the hip.

Hematuria, rectal bleeding and pelvic phleboliths in children with the Klippel-Trenaunay syndrome.

Three young patients with Klippel-Trenaunay Syndrome (KTS), pelvic hemangiomatosis and bleeding are presented. Two children had rectal bleeding and the third presented with hematuria. Radiologic investigation of these patients with complicated angiodysplasia is reviewed. In infants and children calcified phleboliths seen on plain radiographs point toward the correct diagnosis. Angiography shows the vascular malformation which is usually on the venous side. Computed tomography delineates the angiomatous masses and identifies upward extension in the pelvis. Urography or renal ultrasound may show an associated kidney anomaly or tumor.

Radiologic evaluation of osteosarcoma.

Twelve patients with osteosarcoma were evaluated by plain radiographs, radionuclide bone scans and computed tomography (CT). Plain films were the primary radiologic tool for the investigation and follow-up of the skeletal lesion and were particularly helpful for the demonstration of periosteal calcification and bone permeation. The main value of the radiophosphate scan was to detect metastatic or multifocal bone disease. CT was able to show new bone formation in the soft-tissue mass not seen on plain films, to determine proximal intramedullary extension and to assess the response of the bone lesion to preamputation chemotherapy. The lungs were followed at regular intervals with plain chest radiographs and CT scans.

Bowel opacification using "Telebrix 38" for CT scanning.

Prior to abdominal and pelvic computed tomographic (CT) scans in children, bowel opacification was performed with dilute sodium ioxitalamate, meglumine and sodium diatrizoate, or barium sulfate. CT scans of 112 patients using one of these contrast agents were reviewed to compare "image quality." We have obtained the best organ and tissue definition when dilute sodium ioxitalamate was utilized to opacify the gastrointestinal tract.

Computed tomography in bone and joint infections.

Fourteen patients with proven septic arthritis, osteomyelitis or spondylitis were studied retrospectively by both computed tomography (CT) and conventional examinations. CT was performed only when specific problems of diagnosis were unsolved after plain films, standard tomograms or isotope bone scans. In these selected cases, CT was of definite value for: the study of the entire articular surface of bone and periarticular soft tissues; the delineation of extent of medullary and soft-tissue involvement; the demonstration of cavities, serpiginous tracts, sequestra or cloacae in osteomyelitis. It sometimes showed soft-tissue edema or bone destruction not seen on plain films.

[Psoriatic arthritis: analysis of 69 cases and review of the literature].

In a retrospective study 69 cases of psoriatic arthritis evaluated at the Centre hospitalier de l'université Laval (CHUL) between 1968 and 1977 were studied. Certain clinical and laboratory features (age, sex, nail involvement, date of onset of the cutaneous psoriasis, date of onset of the arthritis, and the presence or absence of rheumatoid factor and the HLA-B27 antigen) and the radiologic data were compared with findings previously described in the literature. With three exceptions the findings compared well: at CHUL (a) the frequency of the HLA-B27 antigen in patients with axial psoriatic arthritis was very low, (b) patients with synchronous psoriasis and arthritis were not more severely affected, and (c) certain radiologic signs that seem to be rare and have probably been overemphasized in the literature were not found.

Radiographic gradation of lumbosacral dysgenesis.

The caudal dysgenesis syndrome comprises partial or complete agenesis of the lumbosacral vertebrae, as well as associated musculoskeletal, neurologic and visceral abnormalities. Maternal diabetes mellitus appears to be the most important etiologic factor, but for most patients the etiology is unknown. In the majority of cases, radiologic diagnosis is established readily, but minor degrees often coinciding with a serious urologic disease or a lower limb abnormality may be overlooked.

[Metaphyseal channel and serpiginous tract in osteomyelitis: literature review apropos of 2 cases].

The so-called "metaphyseal channel" and "serpiginous tract" signs are described in detail, emphasizing their value in the roentgen diagnosis and differential diagnosis of some bone lesions. The authors present two cases of osteomyelitis, one of the chronic type and the other subacute in two adult patients, to illustrate the value of the above radiologic signs. These have been previously described in the long bones of infants and children only. The serpiginous tract sign is reported, for the first time, in a flat bone.

Waldeyer's hernia.

One of the rare sites of internal abdominal hernia is Waldeyer's peritoneal fossa. A case report in a young adult male is presented with preoperative roentgen diagnosis on a small bowel follow-through. Anatomic types of paraduodenal fossae are described and some points of surgical importance are stressed.
