[The urgency of special x-ray control in the conservative treatment of femoral fractures in childhood].

The conservative treatment of fractures of the shaft of the femur by extension according to Weber has been successful with children. It is simple to position the young patient on the so called "Weberbock", the position of the fragments can be always controlled radiologically, and if necessary, it can be corrected. But recent investigations have shown, that in some cases false torsions are corrected spontaneously.

[Primary and secondary arthrodeses following talus fractures].

In 18 cases out of 55 patients, who had been operated on talus fractures in our clinic, a primary or secondary arthrodesis of one or both adjacent joints had been necessary. We carried out a follow-up examination on those 18 patients. In 4 cases a primary or early secondary arthrodesis had been performed; the attempt to reconstruct the joint surfaces after the trauma would have been not promising. The indication of the secondary arthrodesis in the 14 patients is discussed. In all the patients there had been a bony healing of the arthrodesis in an adequate time, the gait pattern of the patients had been improved and they complained less pain. We conclude that in case of a not solvable complication after talus fracture like destroyed joint surfaces, secondary severe arthritis or bone necrosis early secondary arthrodesis should be considered as a measurement to avoid long periods of insufficient conservative treatment.

[Diagnosis of carpal bone dislocations].

The importance of initial diagnosis of luxationes of the carpal bones to prevent severe belated harm is emphasized. For reliable diagnosis instructiones to check the x-ray pictures are discussed and illustrated by demonstrating four clinical-radiological cases.

[Roentgenologic diagnosis of acute posttraumatic osteomyelitis].

The X-ray picture is of outstanding importance for diagnosis and treatment of acute posttraumatic osteomyelitis. Its diagnostic value surpasses that of other costly methods such as scintigraphy. After just 14 days, a typical pathological change of bone structure can be seen. For best possible treatment it is of utmost importance to diagnose the change as early as possible. If the disease develops without clinical signs, diagnosis by x-ray film is sometimes the only possibility.

Posttraumatic pseudarthroses--an analysis of development and treatment.

The results of the analysis of 339 pseudarthroses in 326 patients are reported. We reviewed the aetiology and previous therapy of patients with pseudarthroses, their X-ray, case reports and assessed them clinically. We noticed instability as an important aetiological factor for development of non union, especially in combination with a bone infection. The evidence of failures due to a inadequate implant or due to an inadequate surgical technique is shown. Further aetiological factors are discussed. Our principles of treatment of pseudarthroses are outlined. The first principle is to achieve stability by operative procedure. The second is to indicate correctly bone grafting. The third principle concerns infected non unions in which extensive surgical revision is necessary. By carrying out this therapy we achieved bone healing in 95.4% of non infected pseudarthroses and in 86.8% of infected pseudarthroses.

[Injuries of tendons and menisci in the expert testimony of statutory accident insurance].

Differentiation between damage due to internal deterioration (degeneration) and damage by external causes (trauma) is the cardinal problem of expertizing for the national accident insurance in cases of injuries of the tendons and meniscus. In contrast to recognition of a disease as an occupational disease, recognition of the sequels of an on-the-job accident does not provide any relief from the burden of proof. The onus lies with the insured person in all respects, because German law does not recognize "formulae" representing a set sequence of events occurring during an accident. The article points to the essential medical and legal questions involved in writing an expertize.

[Differential diagnosis of post-traumatic hydrarthrosis of the knee joint].

You can diagnose the post-traumatic effusion in a knee-joint generally without difficulty. The fresh effusion is very often bloody. Reflections concerning differential diagnosis are necessary to recognize heavy injuries of soft-tissue and bony injuries. For the diagnostic special X-rays are useful, the arthroscopy and the arthrography. The joint must immediately be unburdened because the effusion can take a negative effect on the articular.
