Urovaginal fistulas: 20 years of experience with 45 cases.

We treated 45 patients with urovaginal fistulas owing to operative gynecological procedures and radiotherapy: 36 had vesicovaginal and urethrovaginal, 6 had ureterovaginal and 3 had rectovesicovaginal fistulas. Reconstruction was performed in 40 patients, mainly via a transvesical approach. There was good success in patients not given radiotherapy: 24 of 26 patients experienced primary healing and no failures were noted. Of the 14 patients treated by irradiation 9 had primary healing and 3 failed therapy. Patients with a urovaginal fistula should be referred to centers with special interest in this type of repair. An alternative surgical technique with the carbon dioxide laser deserves consideration in patients with scarred and irradiated tissue.

Balloon dilatation of bilharzial ureteric strictures.

Balloon dilatation was performed in nine patients with bilharzial ureteric strictures. In six patients a percutaneous, antegrade approach was used and in three a balloon catheter was introduced into the ureter via a cystoscope. In another four patients, negotiation of the strictured area with a guide wire failed. In all dilated patients, symptomatic and urographic relief of the obstruction was obtained during follow-up periods of 3 to 38 months (mean 19). One patient was re-dilated after 24 months and has remained symptom-free for a further 18 months. Balloon dilatation seems to be a promising method of treating this condition.

Studies on renal damage from percutaneous nephrolitholapaxy.

To study in detail the effects of percutaneous nephrolitholapaxy on renal function, a consecutive series of 11 patients were investigated preoperatively by excretory urography, gamma camera renography for determination of individual renal function and computerized tomography of the kidneys. Postoperatively, gamma camera examination, computerized tomography, antegrade nephrostography, renal angiography and excretory urography were performed. With 2 exceptions, percutaneous nephrostomy, dilation of the tract and stone removal were done in 1 stage with the patient under continuous epidural anesthesia. Nephrostomy tract dilation was done with an Olbert type balloon catheter or Alken metallic dilators. Thickening of Gerota's fascia was demonstrated by computerized tomography in most cases, and small to moderate perirenal hematomas were found in several. At gamma camera examination decrease of renal function was noted regularly on postoperative day 1 and returned to near initial levels 2 weeks postoperatively in most cases. Angiography in 10 patients showed discrete parenchymal scarring in some and a peripheral arteriovenous fistula in 1. We conclude that percutaneous renal stone surgery usually is tolerated well by the kidney.

Cardiovascular complications of estrogen therapy for nondisseminated prostatic carcinoma. A preliminary report from a randomized multicenter study.

In a prospective multicenter study, 244 men with highly or moderately differentiated prostatic cancer in stage I, II or III (VACURG) were consecutively randomized to three groups of treatment: Group A (77 patients) received polyestradiol phosphate (Estradurin, Leo) 80 mg i.m. every fourth week + ethinyl estradiol (Etivex, Leo) 150 micrograms daily, group B (72 patients) estramustine phosphate (Estracyt, Leo) 280 mg twice daily, and group C (76 patients) no therapy. Only men without current or previous other malignancy and without cardiovascular disease were admitted to the study. After 4 1/2 years 125 of the 244 patients had left the study, 9 because of cancer progression (stage IV, VACURG). The most serious complications were cardiovascular, including ischemic heart disease, cardiac decompensation, cerebral ischemia and venous thromboembolism, which occurred in 24 patients from group A and 9 from group B as compared to only one patient in group C. The subgroup superficial or deep venous thrombosis comprised 11 group A and 2 group B patients. Estrogens (E + e) offered as palliative treatment to patients with non-generalized prostatic carcinoma is burdened with a high incidence of serious cardiovascular complications.

Visual illusion mimicking dyslexia. A case history.

A 14-year-old girl with reading difficulties was found to have an unusual illusory movement of objects in the central visual field. The movement could be blocked by using a 26 mm Visolette magnifier with an arrow pasted on its lower surface to function as a reference point. After having read with help of this arrangement for 19 months the girl suddenly could see normally and has now been able to read without difficulties for over 3 years. We report the first documented case of visual illusion as the cause of reading difficulty which disappeared at the age of 15 years, thus years after the maturation of visual functions is thought to be completed. Although visual illusion like the one presented here must be rare, it should be diagnosed, because the treatment is totally different from that of usual dyslexia.

Urogenital schistosomiasis: CT evaluation.

Computed tomography (CT) was compared with plain films and intravenous urography in 20 male patients with schistosomiasis. CT better demonstrated the extent of calcifications, which were recorded in the urinary bladder in all cases. The anterior wall of the bladder is first affected. The length and nature of stricture of the ureter can be estimated with CT, and ureteral reflux may be demonstrated when fatty emulsion is used for filling the bladder and the ureter is dilated. Punctate ureteral wall calcifications may be distinguished from intraureteral stones. CT is also helpful in establishing the presence and the staging of bladder tumors.

Quantitative assessment of individual renal function. A comparison between creatinine excretion and a radionuclide method.

Individual renal function was evaluated in 21 patients. Two methods were used in each case, namely gamma camera renography and measurement of creatinine excretion in urine collected via a nephrostomy. The scintigraphic technique utilized measurement of renal parenchymal radionuclide uptake in an early phase. Good agreement was found between the results from both methods in most patients. Gamma camera renography was found to be clinically useful for assessing the function of individual kidneys.

Early visuo-oculomotor development in visually impaired children: two case reports.

The early development of impaired visual functions awaits the serious consideration of clinicians and researchers. Rudimentary vision may be influenced very much by active stimulation during early weeks and years of life. Methods of assessment of visual functions and of visual stimulation/habilitation has to be renewed and properly evaluated. The procedures used at a Centre for Visually Handicapped Children in Sweden, when dealing with small children with very reduced vision, are reported. The influence of various factors on visual and oculomotor functions is discussed.

Transureteroureterostomy with cutaneous ureterostomy for permanent urinary diversion.

Transureteroureterostomy with cutaneous ureterostomy was used for permanent urinary diversion in 18 patients. No early complications occurred, but late complications arose in two patients. Because of its simplicity, this procedure should receive more frequent consideration for permanent urinary diversion. A low complication rate can be achieved by adherence to certain technical principles.

Localized primary amyloidosis of the ureter.

A 57-year-old man with a history of right-sided renal colic had a stricture of the right ureter which was suspected to be caused by a tumour or retroperitoneal periureteric fibrosis. Peroperative frozen section examination revealed tumour-forming amyloidosis of the ureter. The right kidney therefore could be salvaged by an end-to-end ureter anastomosis. Although primary amyloidosis of the ureter is rare, it should be included in the differential diagnosis of ureteric strictures.

A new method for induction of fibrous renal artery stenosis in the pig designed for the study of percutaneous balloon catheter dilatation.

An open surgical method for the creation of experimental renal artery stenosis by ligation with chromic catgut is described. Microscopic examinations at different intervals demonstrated the development of fibrosis throughout the arterial wall. These stenoses seem to provide a suitable model for the study of the effects of balloon catheter dilatation. Previously reported techniques for experimental artery stenosis induction are briefly reviewed.

Balloon catheter dilatation of stenotic renal artery anastomoses. Experiments in the pig.

Stenotic renal artery anastomoses were produced in pigs in order to investigate the effects of transluminal angioplasty. Six weeks after surgery unilateral dilatation with a modern balloon catheter was performed in 8 pigs. These animals were reexamined with angiography and pressure gradient recording 8 to 10 weeks after dilatation and with post-mortem angiography 8 to 10 weeks later. Only 2 arteries had minimal residual stenoses, while in 6 arteries the stenoses were completely eliminated. Microscopic examination showed thinning of the fibrotic vessel wall, with defects in the internal elastic lamina.

Experimental fibrous renal artery stenosis dilated by balloon catheter. Long-term results.

Fibrous renal artery stenosis was induced by subtotal ligation with chromic catgut. Six weeks following unilateral ligation, transluminal balloon dilatation was performed in 8 pigs. Angiography was performed 8 to 10 weeks after dilatation, and at post-mortem 20 weeks after dilatation. Intra-arterial blood pressure and renal venous renin activity were measured before and at the repeat examination after dilatation and the morphologic changes in the arterial wall were evaluated at microscopy. Restenosis developed in 2 renal arteries, while successful dilatation was achieved in 6 cases.
