Effect of methoxyflurane anesthesia on serum uric acid in man.

Serum uric acid levels were significantly increased on the first and third postoperative days following methoxyflurane anesthesia in subjects undergoing various surgical procedures. The increment on each of these days was significantly correlated with the duration of the anesthesia. No such changes were found following halothane or nitrous oxide-oxygen and relaxant anesthesia.

Electroenterography after cholecystectomy. The role of high epidural analgesia.

The electrical activity of the stomach and intestine was monitored during the postoperative period in 30 patients who underwent cholecystectomy. All patients received standard general anesthesia with artificial ventilation during operation; some received high epidural analgesia during surgery and postoperatively, and others, fentanyl analgesia during surgery and nicomorphine afterwards. Electroenterography (EEnG) showed that electrical activity decreased following surgery and returned to base line on the third or fourth day after operation. A marked increase in amplitude and frequency of EEnG oscillations was recorded in 80% of the patients who received bupivacaine injections into the epidural space. A decrease was almost always recorded after nicomorphine injections. During the postoperative period, eating caused a considerable increase in the amplitude and frequency of the electrical activity of the stomach and intestine in patients treated by epidural analgesia, whereas no observable change was recorded in patients treated by nicomorphine injections. It appears that high epidural analgesia may be useful in the treatment of postoperative adynamic ileus.

Effect of amniotic fluid on bacterial growth.

The bacteriostatic effect of amniotic fluid was studied using four pathogenic organisms which are encountered in the perinatal period, E. coli, S. aureus, L. monocytogenes, and Group B beta hemolytic streptococci. Amniotic fluid inhibited the growth of all four organisms, with a wide degree of individual variation among specimens. The greatest degree of inhibition was noted for L. monocytogenes and the least for E. coli. The time in gestation in which the amniotic fluid sample was obtained did not appear to be related to its bacteriostatic capacity. The ability of amniotic fluid to inhibit the growth of certain pathogenic organisms may be significant in prevention of perinatal infection.

Elevation of serum xanthine oxidase following halothane anesthesia in the rat.

Halothan anesthesia was found to be hapatotoxic in the rat, as demonstrated by a significant elevation of serum xanthine oxidase (SXO) level. SXO appeared to be more sensitive marker of liver damage than serum glutamic oxalacetic transaminase. SXO was found to be elevated also following exposure to relative hypoxia.

Influence of enteral oxygen administration on the slow electrical activity of the intestine and stomach.

Thirty-eight starved cats, anesthetized with pentobarbital (Nembutal) sodium, received enteral administration of oxygen and other gases; the effect on intestinal motility as expressed by electrical activity was measured. Oxygen caused a notable increase in amplitude and frequency of slow electrical waves, while carbon dioxide and nitrogen caused no visible alterations. Phentolamine hydrochloride and propranolol hydrochloride together with atropine sulfate decreased the amplitude and frequency of oscillations to near zero; subsequent administration of enteral oxygen caused a notable increase in electrical activity, while enteral carbon dioxide produced no alterations. On the basis of our previous observations that enteral oxygen enhances oxygen and blood delivery to the gut wall, we suggest that the rise in stomach and intestinal electrical activity during enteral oxygen administration in conditions of surgical stress may be associated with an increase in oxygen and blood delivery to the gut wall.

Designing environments for mentally retarded clients.

An environment that stimulates the auditory, visual, and tactile senses increases appropriate behavior among mentally retarded clients and thereby reinforces their learning activities. The author describes several low-cost deisgn techniques for giving a more stimulating and personalized atmosphere to large, existing institutions; discusses design requirements for group homes for retarded and handicapped clients in the community; and summarizes design ideas to be incorporated in new facilities built for retarded clients. She describes the implementation of her design ideas on a unit for blind retarded clients at the Mansfield Training School in Connecticut.

Continuous low rate enteral feeding.

The original method of continuous low rate enteral feeding is used in cases of nutritious deficiencies and is found to be of value in view of its scientific and technical advantages. Its value is demonstrated by 95% satisfactory results. Furthermore, its low cost, handiness and ease of operation make it accessible to any medical or surgical center where it may have a preferential if not exclusive place.

Some serum protein studies in Crohn's disease and ulcerative colitis.

An auto-immune origin was postulated previously for ulcerative colitis. The present results do not support this hypothesis. Crohn's disease has been compared to sarcoidosis and lymphogranuloma venereum. Crohn's disease may be a virus infection coming from the bowel content. The virus may multiply, for instance, inside the submucosal lymphocytes.
