[Subcutaneous avulsion of the flexor digitorum profundus and flexor digitorum superficialis tendons of the ring and little fingers caused by blast injury].

The authors report a case of subcutaneous avulsion of flexor profundus and flexor superficialis tendons of the ring and little finger, caused by a blast injury. Treatment consisted of excision of the flexor superficialis tendon with transosseous reinsertion of the flexor profundus tendon. The functional result was good. Subcutaneous traumatic rupture of flexor tendons occurs infrequently, but simultaneous rupture of both flexor tendons is extremely rare. Knowledge of the subject is limited and treatment of this disorder is poorly defined.

[Isolated ulnar nerve lesions. Results and sequelae after treatment at a specialized unit: apropos of 40 cases].

77 patients with an isolated traumatic ulnar nerve lesion were operated on between 1972 and 1982. 40 patients with more than 18 months of follow-up were reviewed with a new clinical quoting. 37.5 p. cent of satisfactory results were achieved with nerve sutures done in emergency, 56.3 p. cent with secondary nerve grafting. Factors influencing the results were age, level of the lesion and use of the microscope. 80 p. cent of the patients recovered a protective sensibility, but only one third discrimination. Patients considered sensory deficit as little cumbersome. Wartenberg's sign was noticed in 60 p. cent of our patients and a claw hand in 46 p. cent. Power grip was only 70 p. cent of the contralateral hand due to poor interosseous muscle recovery. The main disability was an instable thumb metacarpophalangeal joint in 70 p. cent. Subterminal pinch grip was weak and was improved by the transfer of the flexor digitorum superficialis of the ring finger. Cold hypersensitivity was noticed in 73 p. cent of our patients, but its frequency diminished with sensory recovery. Motor deficiencies were the most important and required early tendon transfers in older patients.

[Results of the primary repair of 28 isolated median nerve injuries in the wrist].

Based on the study of a homogeneous series of twenty-eight isolated median nerve injuries at the wrist, sutured as an emergency procedure, the authors determined the course and sequelae of this type of nerve lesion with no associated tendon involvement. Repair was associated with epi-perineural suture performed with an operating microscope, immobilisation in slight flexion of the wrist for three weeks and rehabilitation of sensation. 60.7% of patients obtained a good or very good results with better motor recovery than sensory recovery. Nerve contusion, loss of nerve substance with suture under tension and work accidents were factors of poor prognosis.

[Treatment of lax arthrotic genu valgum using Debeyre's unicondylar osteotomy. A radioclinical study of 55 knees operated on more than 5 years ago].

Seventy-five cases of degenerative arthritis of the knee with laxity (Ahlback grade III, IV and V) were treated by unicondylar osteotomies of the femur performed between 1967 and 1980. 64 have been reviewed after more than five year follow-up. Among them, nine had been re-operated on because of early complications when this technique was set up. The fifty-five remaining knees were reviewed after a nine year average follow-up. Despite the severity of the joint involvement forty-four were still improved by the osteotomy. The tightening of the collateral ligaments had suppressed the joint laxity. The subluxation of the lateral femoro-tibial joint was reduced and the joint space was enlarged. Eleven knees had deteriorated after five years, seven times in the lateral compartment, four times in the medial one. Five had to be re-operated on, due to functional worsening. Good results were observed when the obtained angle was situated between 3 degrees valgus and 10 degrees varus. A persisting valgus more than 4 degrees may lead to a recurrence. Hypercorrection led to a worsening of the medial femoro-tibial joint in two cases. The unicondylar femoral osteotomy is worth-while even in severe degenerative arthritis. The technique must be precise to avoid failures. It is indicated in cases of joint laxity, extra articular osteotomies being indicated when the laxity is absent. It should not be proposed over the age of 70 years to avoid a risk of stiffness.

[Perforations of the small intestine and intestinal parasitic diseases. Apropos of a case of peritonitis caused by the perforation of the small intestine combined with Taenia saginata infection].

A case is reported of peritonitis from perforation of the small intestine found on operation to be due to Taenia saginata. A review of the relevant literature of the last 20 years failed to find many similar cases, parasites, particularly Taenia being an exceptional direct cause of perforation. In the case reported, however, a direct cause/effect relation is highly probable.
