A survey of the Czechoslovak follow-up of lung cancer mortality in uranium miners.

The major Czechoslovak cohort of uranium miners (S-cohort) is surveyed in terms of diagrams illustrating dependences on calendar year, age, and exposure to radon and radon progeny. An analysis of the dose dependence of lung cancer mortality is performed by nonparametric and, subsequently, by parametric methods. In the first step, two-dimensional isotonic regression is employed to derive the lung cancer mortality rate and the relative excess risk as functions of age attained and of lagged cumulated exposure. In a second step, analytical fits in terms of relative risk models are derived. The treatment is largely analogous to the methods applied by the BEIR IV Committee to other major cohorts of uranium miners. There is a marked dependence of the excess risk on age attained and on time since exposure. A specific characteristic of the Czechoslovak data is the nonlinearity of the dependence of the lung cancer excess risk on the cumulated exposure; exposures on the order of 100 working level months or less appear to be more effective per working level month than larger exposures but, in the absence of an internal control group, this cannot be excluded to be due to confounders such as smoking or environmental exposures. A further notable observation is the association of larger excess risks with longer protraction of the exposures.

[Atraumatic Clostridium septicum infection in granulocytopenia].

A fatal Clostridium septicum infection occurred in three patients. Case 1. A 55-year-old man died of septicaemia resulting from granulocytopenia of uncertain aetiology; it was associated with perforation of ileal mucosal ulcers. Autopsy revealed neutropenic enterocolitis and diffuse gas formation, especially in the brain, caused by Clostridium septicum. Case 2. A 18-year-old boy developed a caecal invagination during imipenem-induced granulocytopenia. A fulminant postoperative Clostridium septicum infection ended fatally. At autopsy many ulcers were found at the site of invagination with gas formation involving all organs. Case 3. Myonecrosis of the left arm, caused by Clostridium septicum, developed without external cause in a 12-year-old girl with congenital neutropenia. Despite aggressive surgical intervention she died of toxic shock. Autopsy revealed caecal mucosal ulcers as the portal of entry of Clostridium septicum.

[Subjective sequelae of splenectomy].

111 of 806 former patients splenectomized at the Würzburg university hospital during the years 1968-1983 were interviewed for their complaints since their operation. Investigations included the use of Beck's inventory for measuring depression and the Giessen questionnaire for the evaluation of general complaints used in psychosomatic medicine. There was a significant increase of symptoms after splenectomy as compared to the normal population. A special list of 18 questions to investigate typical postsplenectomy complaints was answered by 95 of these 111 persons and by a control group of "statistical twins" with similar upper abdominal surgery without splenectomy. The leading difference was the highly significant increase of the susceptibility to infections after splenic loss followed by accelerated exhaustion and increased physical and mental weakness. The distribution of other complaints like for instance the intolerance to alcohol hitherto judged as typical sequelae of splenectomy was not statistically different in both splenectomy was not statistically different in both groups. Since the symptoms listed above as significantly increased in the splenectomized patient were closely correlated with the susceptibility to infection they seem to be rather the expression of the decreased resistance than direct consequences of the loss of the spleen.

Radiation-induced activation of transcription factors in mammalian cells.

In mammalian cells radiation induces the enhanced transcription of several genes. The cis acting elements in the control region of inducible genes have been delimited by site directed mutagenesis. Several different elements have been found in different genes. They do not only activate gene transcription in response to radiation but also in response to growth factors and to tumor promoter phorbol esters. The transcription factors binding to these elements are present also in non-irradiated cells, but their DNA binding activity and their transactivating capability is increased upon irradiation. The signal chain linking the primary radiation-induced signal (damaged DNA) to the activation of transcription factors involves the action of (a) protein kinase(s).

[Alloplastic ligament replacement. A study of the biological fixation of 5 non-resorbable materials].

The long-term stability of alloplastic ligament prostheses depends on various parameters, on including the quality of the biologic fixation in the bone tissue. Biomechanical and histological tests were therefore performed to examine the stability obtained with five different alloplastic and non-resorbable ligament substitute materials following stress-free implantation into the distal femur and proximal tibia of Wistar rats. The materials were: a filament of the PTFE ligament prosthesis, PTFE suture material (Goretex), polyethylene terephthalate (Mersilene), polyethylene (Braun-Dexon) and polybutylene terephthalate (Miralene). The materials used were all of USP-1 quality. Extraction trials of the implants 32 weeks later did not show any differences between the PTFE materials, such as polyethylene and polyethylene terephthalate, whereas a significantly lower value was found for polybutylene terephthalate. The fixation involved connective tissue exclusively with all the materials tested except for PTFE. PTFE not only induced tight and direct bony contact externally, within the material itself extensive formation of new bone was also found. PTFE was found to be the most histocompatible material, while polyethylene terephthalate led to the most pronounced foreign-body-type reactions.

Cancer in man after exposure to Rn daughters.

Results are reported of epidemiological studies in six groups of miners, who work in U mines, Fe mines and shale clay mines. A significant excess of lung cancer was proved in exposure categories below 50 WLM, the first significant excess of lung cancer rate was found in the sixth year following the start of exposure, and a significant difference between the observed and expected rate was found in miners even before the fortieth year of age. The mean attributable annual cancer risk after about 30 y of observation in the whole study was approximately 20.0 and in persons starting exposure after 30 y of age the risk was approximately 30.0 per year per 1 WLM per 10(6) persons. The dose-effect relationship and the attributable lung cancer risk per 1 WLM were significantly influenced by the age at the first exposure by total accumulated exposure and by the character of the accumulation of exposure. The observed effects of smoking and exposure to alpha radiation from Rn daughters were nearly additive. The lung cancer risk per 1 WLM at low levels of exposure (not including the contribution from natural sources in the living environment) in U as well as Fe mines indicated a certain elevation compared with the risk at higher accumulated exposure.

[Peroral nickel provocation in nondyshidrosiform and dyshidrosiform nickel eczema].

The flare-up of pompholyx-type nickel eczema induced by oral nickel challenge has been dealt with in several publications. To compare the reactivity of non-pompholyx and pompholyx-type nickel eczema, we challenged three groups of female volunteers (7 with non-pompholyx-type and 12 with pompholyx-type nickel eczema plus 10 control subjects without nickel allergy) with an oral dose of 2.5 mg nickel. By patch testing we additionally determined the degree of cutaneous nickel sensitivity using a series of nickel sulfate dilutions (5.0-0.00001%) and an possible cobalt allergy using 1% cobalt chloride. In 8 of the 19 patients with nickel allergy, the oral nickel challenge was positive (rash, flare-up at sites of healed eczema and former nickel patch tests, and worsening of preexisting eczema; acute pompholyx lesions only in patients with pompholyx-type eczema). The positive results showed the same frequency in non-pompholyx eczema (43%) as in the pompholyx type (42%). Moreover, no substantial difference was found in the degree of cutaneous nickel sensitivity between the two groups of eczema, and there was no correlation between these results and the outcome of the oral nickel challenge. An additional cobalt allergy was recorded significantly more often in pompholyx-type eczema (P = 0.048), yet there was no influence on the oral nickel challenge. The laboratory parameters examined (differential white cell count and total IgE) did not differ in either of the two eczema groups and did not change substantially during nickel challenge reactions.

[Macular edema following implantation of Binkhorst lenses].

The incidence of cystoid mascular edema following intracapsular or extracapsular cataract extraction with implantation of a Binkhorst 4-loop lens is reported. The results, showing an incidence of macular edema of 13% after intracapsular operation and 2.1% after extracapsular operation, correlate well with reports published elsewhere. Most of the cases were recognized at the earliest three months after operation. A correlation with intraocular pressure was found. Examples of fluoresceinangiographic findings are presented.

[Differential diagnosis of hereditary optic atrophy (author's transl)].

The problem of differential diagnosis is discussed with regard to Leber's optic atrophy and dominant optic atrophy occurring in childhood. The discussion is based on case reports on four members of the same family who were affected at very different ages and in whom the disease developed differently. Various symptoms are compared with data already published. It is impossible to classify the members of this family according to any particular group on the basis of these data.

[The heart of the old hemiplegic patient (author's transl)].

The heart of the old hemiplegic patient is examined as follows: 1. The preapoplectic situation of the heart which often induces in a conclusive manner the beginning of a stroke, 2. the occurrence of heart troubles with the stroke itself and 3. the influence of rehabilitation measures and the heart function. Preexistent heart troubles are very frequent (in ca. 80%). Thereby the hypertension with a left ventricular hypertrophy and later with heart failure play an important role. The stroke itself especially in subarachnoidal bleedings can cause severe electrocardiographic anomalies. The telemetric controlled heart shows specially while rehabilitation more extrasystoles and alterations of repolarisation but usually do not impair the rehabilitation. With a systematic rehabilitation (training) the heart is most favourably influenced.
