Treatment of advanced endometrial adenocarcinoma with a combined cytotoxic therapy. Predictive value of cytosol estrogen and progestin receptor levels.

Twenty patients with advanced or recurrent endometrial adenocarcinoma were treated with combination chemotherapy consisting of Adriamycin, cyclophosphamide, 5-fluorouracil, and vincristine at three-week intervals. A minimum of four treatment courses was given in each case. There were five total and five partial responses (50% favorable response rate); progression of the disease was evident in seven cases (35%). Lung metastases responded significantly better (P < 0.01) than other lesions: seven out of nine lung metastases showed an objective remission, whereas only two out of 11 tumors in pelvic, abdominal, or retroperitoneal space responded. The response rate did not correlate with histologic grade of tumor differentiation, or the performance state and age of the patient. Cytosol estrogen and progestin receptor levels were measured in 15 cases from the carcinomatous endometrial tissue prior to therapy. Ten patients with low receptor values (estrogen and/or progestin receptors below 30 fmol/mg cytosol protein) had a significantly (P < 0.025) greater response rate (70%) than did patients with higher receptor values (both receptors above 30 fmol/mg protein, response rate 20%). Determination of only one of the two receptors did not differentiate the patients equally well, although the response rate tended to be better (0.05 < P < 0.1) in patients with a low level of either estrogen or progestin receptor (67% response rate) when compared with a 33% response rate in patients with a high level of the corresponding receptor. Our results suggest that the measurement of cytosol steroid hormone receptors has the potential to serve as a suitable indicator for selection of endocrine or nonhormonal chemotherapy for patients with advanced endometrial adenocarcinoma.

Doppler ultrasound findings in tubo-ovarian infectious complex.

Ten women with tubo-ovarian infectious complex caused by pelvic inflammatory disease were investigated with vaginal Doppler sonography during the acute and healing phase of the infection. Doppler velocity waveforms were quantitated by the resistance index (RI) and pulsatility index (PI). A low resistance blood flow was found at the margin of the infectious complex. An RI' value < 0.5 was detected in 6 patients in the acute or subacute phase of infection. The severity of the infection as determined by C-reactive protein values was inversely correlated to RI and PI. The process of angioneogenesis is speculated to be responsible for this increased flow. The tubo-ovarian infectious process must be remembered in the differential diagnosis, when an adnexal mass with low resistance blood flow is found in the ultrasound examination.

Follicular fluid cyclic nucleotides, prostaglandin E2, leukotriene B4, oestradiol, progesterone and luteinizing hormone in women stimulated by clomiphene and gonadotropins.

The concentrations of cyclic AMP, cyclic GMP, prostaglandin E2 (PGE2), leukotriene B4 (LTB4) oestradiol, progesterone and luteinizing hormone (LH) were determined in 30 follicular fluid samples from 25 women in an IVF programme. Multiple follicular development was induced by clomiphene citrate and human menopausal gonadotropin. Laparoscopic aspiration of follicular fluid was performed 36 hours after injection of human chorionic gonadotropin. The mean concentration of LH was significantly lower (p less than 0.05) in the follicular fluid with ova developing to embryos that cleaved than in those without cleavage. The mean concentrations of cAMP, cGMP, PGE2, LTB4, oestradiol and progesterone were not significantly different. PGE2 and LH concentrations in the follicular fluids showed significant correlation (r = 0.56; p = 0.004). Significant correlation was also seen between cAMP and progesterone concentrations (r = 0.50; p = 0.006). The presence of cyclic nucleotides, PGE2 and LTB4 in human follicular fluid suggests that these factors may have a role in the function of the follicular fluid.
