[Endoscopic controlled laser lithotripsy in the treatment of sialolithiasis].

The article describes the first successful application of endoscopically controlled laserlithotripsy in the ENT field in a patient with recurrent purulent sialadenitis of the left submandibular gland due to sialolithiasis. By means of endoscopically controlled laserinduced lithotripsy of salivary gland stones, it was possible to achieve complete stone fragmentation without harming the glandular duct and the gland.

[Incidence of epithelial neoplasms in the head and neck area in non-Hodgkin's lymphomas].

There is an increased risk of malignancy in patients with genetically determined or acquired immunosuppression. The authors report a case of a 70-year-old patient with a lymphoplasmocytic immunocytoma who developed 19 squamous-cell carcinomas (SCC) and nine basal-cell carcinomas (BCC) over a three-year period. This was the reason to review 100 cases of malignant lymphomas for evidence of additional malignancies. Of these patients, 15% had one or more SCC or BCC in the head and neck area. The age range was 59 to 79 years (mean, 71.7 years) and the male:female ratio 11:4. One or more SCC arose in 93% of these patients, 36% developed an additional one or more BCC, and BCC alone occurred in 7%. The usual ratio of BCC:SCC is 10:1; in the authors' patients, by contrast, this ratio was 6:14. In 12 cases, SCC and BCC were located on the skin. The remaining cases of SCC developed in the oral mucosa, the tonsils and the hypopharynx. In 13 cases, low-grade malignant lymphomas were found and in two cases high-grade malignant lymphomas were found. The SCC were clinically aggressive. Thirty-six percent of the patients had recurrent lesions, 43% multiple neoplasms, and 50% metastases. Histologically, the SCC showed moderate to poor differentiation, a high degree of cell polymorphism and mitotic activity, and deep tissue infiltration. There are several explanations for the increased incidence of neoplasms in patients with immunodeficiency disorders. The surveillance function of the immune system may be impaired due to the disease itself or due to the treatment for immunosuppression. Immunosuppressive and cytotoxic agents are potential carcinogens.(ABSTRACT TRUNCATED AT 250 WORDS)

A comparison of class II antigenicity of human tracheal allografts stored in cialit and in merthiolate.

This report deals with some of the immunological aspects of the transplantation and preservation of human tracheal grafts. The immunological behavior of the transplanted graft depends largely on the interaction of the preservatives with the tissue proteins. Using monoclonal antibodies and the immunoperoxidase staining technique we have investigated the effect of Merthiolate, Cialit, formaldehyde/Merthiolate, and formaldehyde/Cialit preservation techniques on monomorphic determinants of class II transplantation antigens in human tracheal allografts. Unpreserved grafts were found to express class II antigens. These antigens were totally destroyed after 7 days in formaldehyde and 42 days in Cialit and Merthiolate. Preservation in Cialit and Merthiolate showed a gradual disintegration of the histological structures. In contrast, buffered formaldehyde did not appear to alter the histological structure of the tracheal graft. Irrespective of the mode of preservation, the cartilaginous tissue appeared to persist virtually unaffected. No essential differences were observed between the immunological staining of tracheal grafts preserved in Cialit and Merthiolate.

[Allogenic cartilage transplantation--immunohistochemical investigations on the antigenicity of the perichondrium].

Perichondrium of human nasal, tracheal, costal and conchal cartilage was analysed by an immunohistological staining technique using monoclonal antibodies against different class II antigens. Our morphological study shows clearly the presence of class II antigens on the perichondrium of nasal, tracheal and conchal cartilage. Therefore the transplantation of cartilage allograft free of perichondrium may be preferred.

[What has changed in esthetic and functional corrective surgery of the nose?].

The septo-rhinoplasty is a standard procedure in head and neck surgery. Only slight modifications have been developed during the last years concerning especially the surgery of the septum, the tip of the nose and the osteotomies. The presently well favored "external rhinoplasty" is not of any advantage and should be restricted to only few indications. In the present paper the different surgical approaches are described for the various nose deformities of adults and especially for those problems related to children.

[Photodynamic laser tumor therapy with a hematoporphyrin derivative in spinocellular carcinoma of the external ear].

Haematoporphyrine-derivative (hpd) selectively photosensitises malignant tumours following intravenous application. 48 hours after injection of hpd an integral laser-light application of the tumour and the surrounding normal tissue was performed. We report on the successful administration of this photodynamic therapy in a patient with a spinocellular carcinoma of the auricle. 16 days after therapy the tumour showed a complete response without damage of the also irradiated surrounding normal tissue. Apart from a temporary sensitivity to light no side effects of the therapy were seen.

[Antibody to lipoid A in the treatment of septic shock].

The protective effect of high-titer anti-lipid A hyperimmune globulin with respect to the course of the disease and the mortality rate was studied in patients with septicemia verified by positive blood cultures. Six patients were treated with anti-lipid A in an open study. Dramatic improvement in fever curves and clinical condition in some of the patients encouraged us to start a randomized double blind study. So far, 17 patients have entered the study, 16 of whom were evaluable. Immediately after a positive blood culture was found, patients received either high doses of anti-lipid A or placebo (saline solution) on two subsequent days. Before and after each infusion blood samples were taken in order to assess serum bactericidal activity and anti-lipid A titers. Because of the still small numbers of patients the results of both studies were summarized. In all patients treated with anti-lipid A clear-cut increases in anti-lipid A titers were shown. Patients with repeated gram-negative infections showed higher median anti-lipid A titers than patients without such a history. The patients treated with anti-lipid A immune globulin ran a significantly milder course than the placebo group. The severe signs of septic shock were reversed in seven of 15 patients on anti-lipid A compared to two of seven patients treated with placebo. In the anti-lipid A-treated group, three of 15 patients died, and in the placebo group two of seven. This difference is not statistically significant.

[Personal catheter technic for intra-arterial chemotherapy in cancers of the head and neck].

Regional intraarterial chemotherapy can display its advantages as augmented concentrations of effective substances within the tumour tissue and lower systemic side effects, only if tumour perfusion can be carried out superselectively. For different reasons, the present techniques have not been accepted for extensive clinical use. A new catheter technique is, therefore, presented that is appropriate for both clinical routine and superselective tumour perfusion.

[Microsurgical reconstruction of large 3-layer defects of the cheek].

For reconstruction of large penetrating defects of the cheek besides pedicled tissue transfer numerous free flaps are available to the head and neck surgeon. The inferior epigastric abdominal wall flap, in particular, has proved a versatile and reliable flap, just as the cutaneous flaps from the dorsum of the foot and from the forearm. While the abdominal wall flap offers both, a cutaneous and a peritoneal surface, the latter is used for replacement of intra-oral lining, the cutaneous flaps have to be folded to obtain two surfaces for reconstruction of skin and intra-oral lining. The surgical technique of the epigastric abdominal wall flap is presented and its use is discussed.

[Indications for venous digital subtraction angiography in otorhinolaryngologic diseases].

Intravenous DSA was performed on 45 patients for diagnosis of otorhinolaryngologic diseases. Hypervascular glomus tumours can be accurately diagnosed. Furthermore, vascular architecture and topography can be analysed to allow the planning of tumour embolization or surgery. Hypovascular tumours are inadequately demonstrated by intravenous DSA. A specific and detailed analysis of thin vessel architecture in malignant tumours is impossible by intravenous DSA.
