The purpose of this study was to test a theory of affective learning and to develop a data-gathering instrument for evaluation of affective learning. Two primary areas of affective learning were conceptualized for study: coping with feelings about professional knowledge and coping with feelings about the necessity for development of professional attitudes toward illness and disability and toward death and dying. This two-year study project (1977-1979) involved a total of 70 first-year medical students. Data were obtained by means of observing students in small group discussions. The observation instrument consisted of a checklist of behaviors. Observer reliability was tested. Study results suggested that students progress through phases of affective learning as described in the theory. Implications of the findings are discussed.

Sixty-three children with seizure disorders receiving phenobarbital and/or diphenylhydantoin for more than 12 months had liver function tests evaluated. All 56 whose serum anticonvulsant concentrations were in the therapeutic range had elevations of their serum gamma glutamyl transpeptidase activity. Of the 11 who had elevated SGOT and SGPT concentrations initially, six had persistent transaminase abnormalities for more than 20 weeks. Liver tissue from these six patients revealed by light microscopy uniform swelling of the hepatocytes without cell necrosis, inflammation, fibrosis, or disturbance of hepatic architecture. Electron microscopy demonstrated proliferation of the smooth endoplasmic reticulum without other ultrastructural alterations. All six patients were maintained on the same dosages of PB and/or DPH, and their transaminase activities returned to normal within eight to 14 months. The clinical well being of these patients, the transient nature of their SGOT and SGPT elevations, and the absence of specific histopathology suggest that chronic treatment with PB and/or DPH does not result in hepatotoxicity but rather in enzyme induction. The data indicate that liver biopsies are not warranted in such children and that PB and DPH may be continued despite mild elevations of SGOT and SGPT, without concern for hepatic damage.

Male pesticide applicators who worked with the nematocide dibromochloropropane (DBCP) were examined to determine the possible testicular toxicity from this exposure. Infertility and azoospermia which were first noted among factory workers exposed to DBCP were not observed among the applicators. Sperm count depression, however, was associated with the duration of exposure in the current year but not with exposure in past years. The extent of exposure to DBCP in the current year was also associated with an elevation of serum follicle stimulating hormone (FSH) but not of luteinizing hormone (LH). Sperm count depression was limited to applicators involved in irrigation setup work and in the calibration of equipment. These results suggest that the testicular toxicity of DBCP for men may occur in a shorter period than was previously reported, that the effect may be reversible in men with mild sperm count depression, and that public health measures might be directed at controlling specific application techniques.
This field study was undertaken to quantitate the exposures of peach pickers to pesticide residues. Five fields were harvested each for three days at decreasing post-application intervals. Aerosol and dermal exposure estimates indicated 98-99% of the workers' dose was dermal, predominantly to the hands and upper extremities. Estimates of dose correlated with measurements of foliar residues. Despite estimated dose rates of up to 14 mg/hr, the RBC cholinesterase in these workers may have been only slightly inhibited (circa 4%) when compared to an unexposed, control population. Comparisons of these results with earlier studies of parathion residues on citrus indicate a consistent pattern of worker exposure. However, a concern is raised over the efficacy of using existing foliar residue sampling methodologies for all other pesticides.
