Regional cerebral blood flow (rCBF) pattern analysis corresponds with albumin extravasation in experimental cerebral ischaemia.

The possibility of the evaluation of ischemia solely by the momentary values of regional cerebral blood flow (rCBF) was investigated. Ischaemic damage was evaluated histopathologically and by observing albumin extravasation. Unilateral carotid clippings were performed for 1 h and the gerbils evaluated with regard to the length of recirculation time. In animals without a recirculation period, the degree of damage could be explained by the rCBF reduction ratio during clipping. However, in cases with reperfusion periods, 5 animals showed an almost complete disappearance of rCBF during clipping, but albumin extravasation was seen in 1 case only. Thus, the discordance between the rCBF reduction ratio and the abnormal permeability was considerable in reperfused animals. The post-ischaemic rCBF pattern was classified into 3 types, the 'post-ischaemic hyperperfusion type', the 'post-ischaemic hypoperfusion type' and the 'post-ischaemic normoperfusion type'. In the first two types, the albumin extravasation was positive in 10/10 cases, and in the third type, in only 1/20. Therefore, the post-ischaemic rCBF pattern was thought to correlate well with ischaemic damage in the reperfused cases. The rCBF changes momentarily and the pattern analysis of the rCBF, particularly in the post-ischaemic phase, appeared to be important.

Lectin (UEA-1) reaction of capillary endothelium with reference to permeability in autopsied cases of cerebral infarction.

The relationship between endothelial reactivity to Ulex europaeus agglutinin-1 (UEA-1) and the permeability of the vascular wall in human autopsied cases of cerebral infarction was studied. Sections from the cerebral cortex were reacted with horseradish peroxidase UEA-1 to demonstrate the surface membrane of endothelial cells. Albumin in the neuropil of sections was demonstrated for the estimation of increased vascular permeability. The results showed that endothelial reactivity to UEA-1 was reduced in cases where death had occurred 3 to 5 days after onset of cerebral infarction. Reactivity was also diminished in cases where death had occurred after 13 and 25 days; these cases showed fresh ischemic lesions caused by re-attacks of infarction. Albumin extravasation into the neuropil was demonstrated in these intermediate cases. Chronic cases, dying after more than 52 days, showed no reduction of endothelial reactivity to UEA-1 and no albumin extravasation was proved. It was concluded that UEA-1 can be employed as a useful morphological marker for evaluation of endothelial function and vascular permeability.

Semiquantitative evaluation of ophthalmic collateral flow in carotid artery occlusion: ultrasonic doppler study.

The reliability of quantitative evaluation by doppler ultrasound with regard to the ophthalmic collateral blood flow in patients with carotid artery occlusion was estimated. The ultrasonic doppler flow signals of the ophthalmic collateral flow of 54 carotid occlusions were classified into four types and three degrees--high, moderate, and low reversed flow patterns--and were compared with the angiographic findings of the collateral flow classified into three grades--good, poor, and none. With the exception of three cases, the ultrasonic doppler flow patterns of the collateral flow correlated well with the angiographic findings in 54 occluded carotid arteries.

Tissue platinum after clinical treatment with cisplatin or carboplatin in tumor-bearing patients.

Tissue platinum (Pt) levels were measured in tumor-bearing patients treated with either cisplatin or carboplatin. Cisplatin was given by intra-arterial, intraperitoneal, and intravenous (iv) administrations. After death, vertebrae and intervertebral disks were removed from eight human subjects, and livers and kidneys were removed from the half of them. When cisplatin was administered intraperitoneally, Pt of the liver was higher than that of the kidney, and a high content of Pt was detected in the vertebra by comparing with the other administration methods. At the intra-arterial administration of cisplatin, Pt was mainly accumulated in the kidney. At the iv administration of cisplatin, a high level of Pt was found in the vertebra and intervertebral disk, especially at the highest value at 10.31 micrograms/g in the intervertebral disk of one case, whereas a low level of Pt was detected in the liver. On the contrary, it was found that the iv administration of carboplatin did not result in high accumulations of Pt in the liver, kidney, intervertebral disk, and vertebra. Therefore, Pt is accumulated in different organs, depending on the way cisplatin is administered, but Pt is accumulated least in them by the administration of carboplatin.

Pattern difference of reversed ophthalmic blood flow between occlusion and stenosis of the internal carotid artery. An ultrasonic Doppler study.

Reversed ophthalmic blood flow in the occlusions (33 patients) and stenoses (11 patients) of the internal carotid artery (ICA) was examined using the ultrasonic Doppler technique. The Doppler shift frequencies of the blood flow signal were analyzed to obtain their sound spectrogram. In stenosis of the ICA, "presystolic notch" was more frequently observed and "d/S" value (S, d; maximum blood flow velocity at systolic and diastolic) was smaller than in occlusion. These two characteristics of stenosis distinguish it from occlusion with 89% accuracy although this method is applicable only for the patients with reversed ophthalmic blood flow.
