Clinical breast examination for screening of asymptomatic women: the importance of clinical breast examination for breast cancer detection.

Of 489 asymptomatic women who were referred to our institute from other screening clinics, 46 were eventually proven to be breast cancer patients, and this number equated to 8.1% of the 565 breast cancer patients treated in our institute during the period of January 1997 to December 1998. Among the 46 cancer patients of the initial 489 asymptomatic women, twenty-five (54.35%) were detected by mammogram alone, six (13.04%) by clinical breast examination (CBE) alone, and the remaining 15 (32.61%) by both mammogram and CBE. In context with age, the mammographic sensitivity for cancer detection was 100% for women aged over 60, 91% for 50s, 78.9% for 40s, and 75% for 30s, and inversely correlated with the patient's age. Among the 25 cancers detected by mammogram alone, 18 (72%) belonged to DCIS or stage I. In contrast, four (66.7%) of six cancers detected by CBE alone and nine (60%) of 15 cancers by both CBE and mammography were included in stage IIa or IIb. However, the total incidence of early cancers (stages 0 and I) was significantly higher in the screening group than in the symptomatic group (p < 0.01). These results suggest that the role of mammography is important in the detection of cancers in their earlier stage and CBE is helpful in reducing false negative results in breast cancer screening. In conclusion, film mammography is the best tool for the detection of microcalcification and is useful for the detection of earlier lesions, but is not perfect for the detection of breast cancer particularly in young women. A careful CBE is an essential part of breast screening in order to reduce false-negative results.

Ocular deviation after unilateral laser in situ keratomileusis.

Laser keratomileusis and excimer laser photorefractive keratectomy in situ are widely used therapies for treating myopia. The corrections of refractive error by glasses or contact lens result in a relatively equal refractive correction on both eyes. However, refractive surgery on a single eye can cause a focus disparity between both eyes and may result in the impairment of fusion leading to strabismus. This article aims to report a case where diplopia and esotropia occurred 1 month after laser keratomileusis (LASIK) in situ for the correction of myopia.

The mucinous layer of corneal endothelial cells.

PURPOSE: The goal of this study was to characterize the morphology of the mucinous layer on rabbit, bovine, owl, and human corneal endothelial cells. METHODS: Corneoscleral buttons were fixed using cetylpyridinium chloride to stabilize "mucus" and the tissue was prepared for transmission electron microscopy. Photomicrographs were measured to determine the thickness of the endothelial and epithelial mucinous layer in the central cornea. RESULTS: The endothelial mucinous layer was seen as a nearly uniform electrodense region on the apical aspect of the endothelium. It was found to be 0.9 microm, 0.9 microm, 0.9 microm, and 0.5 microm thick in rabbit, bovine, owl, and human, respectively. The owl endothelium had an additional less electrodense layer with a granular appearance and a thickness of about 200 microm. The mucinous layer on the epithelium was similar in appearance to that on the endothelium and across species. CONCLUSIONS: The morphologic similarity of the endothelial and epithelial mucinous layers is a serendipitous finding that should prove valuable in experimental design. Ultimately, it is hoped that studies of the posterior corneal surface will deepen our knowledge of endothelial protection.

Radiologic findings of metastatic signet ring cell carcinoma to the breast from stomach.

Two Korean women (41 and 23-years of age) presented with painful breast enlargement and redness. The involved breast was confirmed as metastatic signet ring cell carcinoma. Although metastatic signet ring cell carcinoma of the breast shows similar clinical symptoms to inflammatory breast cancer, the difference between the two is that this malignancy showed no microcalcifications or mass on radiographic findings (mammograms and US). Therefore, after the confirmation of signet ring cell carcinoma of the breast has been made, metastatic signet ring cell carcinoma should be considered if there are no microcalcifications or masses evident on radiographic findings.

Potential biomarker of coal workers' pneumoconiosis.

It is well known that various cytokines and growth factors secreted from macrophages/monocytes play the key role in the pathogenesis of pneumoconiosis. These can act as biosensors for the prediction of pneumoconiosis. To evaluate which cytokines can be used as sensitive biomarkers in pneumoconiosis, we measured tumor necrosis factor-alpha (TNF-alpha), interleukin-8 (IL-8) and platelet-derived growth factor-AA (PDGF-AA) in supernatant of monocytes with or without coal dust (5 mg/ml) and serum in 42 coal miners with pneumoconiosis and ten healthy control. The coal-stimulated release of TNF-alpha and IL-8 from blood monocytes was significant increased in pneumoconiosis patients compared with controls. The level of TNF-alpha and IL-8 in blood serum was higher in subjects with pneumoconiosis than in controls.

Cloning and characterization of the promoter region of the Wilson disease gene.

Wilson disease (WD), an autosomal recessive disorder of copper transport, is marked by impaired biliary excretion and incorporation of copper into ceruloplasmin. Molecular mechanism regulating the expression of the WD gene was studied. We isolated, sequenced, and characterized approximately 1.3 kb of the 5'-flanking region of the WD gene from the human genomic library. The approximately 1.3 kb of the WD sequence directed high level of luciferase activity in HepG2 cells. Interestingly, the 5'-flanking region contained four metal response elements (MREs) and six MRE-like sequences (MLSs), usually found in the metallothionein genes. It also contained a number of putative regulatory elements such as Sp1, AP-1, AP-2, and E-box, but lacked TATA box. The transcription start site was located at 335 base pairs upstream of the translation initiation site. Successive 5'-deletion analyses suggested that the 159-base pair region from -811 to -653, which includes MLS2 (-802 to -796) and MLS3 (-785 to -779), contained one or more positive regulatory element(s). A negative element was also identified at region -1038 to -812. A protein-MLS complex was identified through electrophoretic mobility shift and competition assay using MLS2/MLS3 and HepG2 cell nuclear proteins.

Expression and distribution of extracellular matrices during corneal wound healing after keratomileusis in rabbits.

We compared the corneal wound healing responses of keratomileusis and keratectomy in rabbits. A single pass of thin lamellar keratectomy was performed with a microkeratome in rabbits. The lenticule was repositioned with a hinge in one group and discarded in the other. With immunofluorescence techniques, we studied the appearance and distribution of fibronectin, fibrinogen and type III collagen at follow-up intervals from 1 h to 3 months. Fibronectin and fibrinogen began to deposit on the periphery of the denuded stroma 3 h after induced injury and diminished once reepithelialization had completed in both groups. In the corneas with attached lenticules, type III collagen was detected 7 days after surgery and lasted for at least 3 months. Type III collagen was detected only in the periphery of the interface and not in its center. These findings suggest that fibronectin and fibrinogen may play roles in epithelial healing after in situ keratomileusis in rabbits. Stromal healing of collagen seems to take place 7 days after keratomileusis and to last for several months. No deposits of new collagens in the center of the interface between lenticule and stromal bed may explain the optical clarity in the keratomileusis.

Contrast enhancement of small hepatocellular carcinoma: usefulness of three successive early image acquisitions during multiphase dynamic MR imaging.

OBJECTIVE: The purpose of this study was to document the usefulness of early triphasic image acquisition during gadolinium-enhanced multiphase dynamic MR imaging for the detection of contrast enhancement in small hepatocellular carcinoma. SUBJECTS AND METHODS: Dynamic MR imaging was performed in 332 patients at 1.5 T using a T1-weighted fast low-angle shot sequence (TR range/TE, 113-130/4.1; flip angle, 80 degrees). With the start of bolus injection of gadolinium dimeglumine (0.1 mmol/kg) into the antecubital vein, three sets of contrast-enhanced images of the entire liver were obtained serially at 10, 35, and 60 sec. Contrast enhancement of the tumor detected on each phase was evaluated in conjunction with the unenhanced T1-weighted images for 52 patients with small hepatocellular carcinomas up to 2 cm in diameter (n = 79). RESULTS: The first phase images depicted a larger number (n = 57) of contrast enhancement of small hepatocellular carcinoma tumors than the second (n = 44) or third phase images (n = 14). Twenty-six lesions were enhanced only during the first phase, and nine, during the second phase. The detection rate of contrast enhancement in the combination of first and second phases was the same as the overall detection rate for all three phases (70/79, 89%) and significantly higher than those of each phase. CONCLUSION: Early multiple image acquisition is helpful for the assessment of small hepatocellular carcinoma and is enabled by the high detection rate of contrast enhancement regardless of individual variations in the timing of enhancement.

Time-dependent expression of ICAM-1 & VCAM-1 on coronaries of the heterotopically transplanted mouse heart.

To investigate the pathogenesis of accelerated graft atherosclerosis after cardiac transplantation, a genetically well-defined and reproducible animal model is required. We performed heterotopic intraabdominal heart transplantation between the two inbred strains of mice. Forty hearts from B10.A mice were transplanted into B10.BR mice. Recipients were sacrificed at 1, 3, 5, 7, 14, 28, and 42 days after implantation. The specimens from both donor and recipient were examined with fluorescent immunohistochemistry and the serial histopathologic changes were evaluated. In the donor hearts, ICAM-1 and VCAM-1 expressions were minimal at day 1 and they gradually increased, reaching their peaks on day 5 or 7 and remained unchanged by day 42. However, there were very little expressions in the recipients' hearts. Mean percent areas of intima in the donor coronaries revealed progressive increase by day 42. However, those in the recipients occupied consistently less than 5% of the lumen. In conclusion, we demonstrated that a heterotopic murine heart transplantation model was a useful tool to produce transplantation coronary artery disease and that adhesion molecules on the cardiac allografts were activated very early and remained elevated at all time-points, nonetheless the arterial lesion was detected after day 28 and its progression was accelerated thereafter.

Effect of angiopeptin and aspirin on accelerated graft atherosclerosis in transplanted mouse heart.

In this study of the inhibitory effects of angiopeptin and aspirin on the development of accelerated graft atherosclerosis (AGAS), 22 B10.BR mice received intra-abdominal heterotopic heart transplants from B10.A mice, without immunosuppression. Group 1 (n = 5) received no pharmacological intervention, Group 2 (n = 6) was treated with angiopeptin, Group 3 (n = 5) with aspirin, and Group 4 (n = 6) with both. There was no significant difference in the incidence of AGAS among these groups. The magnitude of intimal lesion development showed less narrowing of large vessels (> 100 microns in diameter) in groups 2 and 4--i.e. the groups received angiopeptin (Group 1 = 46.9 +/- 9.3%, Group 2 = 28.5 +/- 9.2%, Group 3 = 44.1 +/- 10.9%, Group 4 = 24.2 +/- 5.9%; p < 0.01). Comparison of the fraction of tropomyosin-positive staining cells in the intima revealed a lesser degree of staining in Group 2 (p < 0.01). No intervention was effective in preventing smooth muscle cell proliferation in the media or inflammatory cell infiltration in the adventitia. In conclusion, our data suggest that angiopeptin is effective in the direct inhibition of intimal smooth muscle cell proliferation in relatively large vessels, whereas aspirin exhibits no inhibitory role in the progression of AGAS. Angiopeptin appears to be a potential therapeutic agent for inhibiting the progression of postoperative AGAS in clinical heart transplantation.

Analysis of the factors affecting decentration in photorefractive keratectomy and laser in situ keratomileusis for myopia.

To evaluate the relationship between ablation zone decentration measured by corneal topography and various factors such as sex, age, order of operation, preoperative sedative prescription, ablation diameter and depth, type of procedure (photorefractive keratectomy = PRK, laser in situ keratomileusis = LASIK), and the use of a passive eye tracker, we examined the data of 80 eyes in 50 patients. The patients received PRK (43 eyes in 30 patients) or LASIK (37 eyes in 20 patients), and were followed for 3 months postoperatively. Statistical analysis of the data was performed using t-test, ANOVA and multiple regression analysis. The overall average ablation decentration from the pupil center was 0.43 +/- 0.27 mm, 0.35 +/- 0.22 mm in PRK and 0.47 +/- 0.30 mm in LASIK. Overall 91.3% of patients were decentered less than 0.75 mm and 95.0% were decentered less than 1.00 mm, while 93.9% of patients were decentered less than 0.75 mm in PRK and 88.7% were decentered less than 0.75 mm in LASIK. The most meridional displacement was toward the superonasal quadrant; 46% in PRK and 51% in LASIK. There was less decentration in males, in the 2nd-operated eye, in older age, PRK, in larger ablation diameter, and in shallower ablation depth, but these differences were not statistically significant.

Current status of product for endoscopic surgery in Korea.

Endoscopic surgeries are popularly performed these days because of the advantages in rehabilitation and cosmetic aspects. But some accompanying disadvantages should not be neglected. For surgeons unfamiliar with the endoscopic surgery techniques, the operation takes more time and dangerous complications may occur more frequently. Most endoscopic surgery techniques, also called minimal invasive surgery techniques, were developed and disseminated from Europe and the U.S. They require specialized equipment and instruments that are expensive, and for this reason the total operation cost compared to conventional open surgery is higher. Despite all the advantages of endoscopic surgery, the high cost due to the of imported specialized instruments is holding back its spread in Korea. The development of self-supporting endoscopic instruments in Korea is mandatory for reducing the expense, and in this regard I have designed trocars of various size, specimen retrieval bags, suction and irrigation tubing sets, gasless laparoscopic surgery sets, scissors of various types, and forceps. Instruments used in suture and ligation which are currently under development will also soon be introduced.

Identification of three novel mutations and a high frequency of the Arg778Leu mutation in Korean patients with Wilson disease.

Four mutations--R778L, A874V, L1083F, and 2304delC--in the copper-transporting enzyme, P-type ATPase (ATP7B), were identified in Korean Patients with Wilson disease. Arg778Leu, the most frequently reported mutation of this enzyme, was found in six of eight unrelated patients studied, an allele frequency of 37.5%, which is considerably higher than those in other Asian populations. The novel single nucleotide deletion, 2304delC, was found in one patient. Since a mutation at cDNA nucleotide 2302 (2302insC) had been previously described, this region of the ATP7B gene may be susceptible to gene rearrangements causing Wilson disease.

Synthesis and comparative molecular field analysis (CoMFA) of antitumor 3-arylisoquinoline derivatives.

In this study a series of 3-arylisoquinoline derivatives were synthesized and cytotoxicity against human melanoma tumor cell evaluated, and a three dimensional quantitative structure-activity relationship was investigated using the comparative molecular field analysis (CoMFA). The results suggested that the electrostatic, steric and hydrophobic factors of 3-arylisoquinolines were strongly correlated with the antitumor activity. Considerable predictive ability (cross-validated r2 as high as 0.721) was obtained through CoMFA.

Nutritional status in acute stroke: undernutrition versus overnutrition in different stroke subtypes.

OBJECTIVES: Nutritional status in the acute stage of stroke has not been properly evaluated in different stroke subtypes. The objective of this study was to investigate the nutritional status of different subtypes of stroke patients. SUBJECTS AND METHODS: We studied 88 female patients with first-ever strokes. Strokes were divided into cerebral infarction (CI, n=67) and intracerebral hemorrhage (ICH, n=21). We measured the nutritional status of the patients in the acute stage of stroke with the use of 8 parameters including 3 biochemical and 5 anthropometric ones. These variables were assessed in stroke patients and 120 age-matched controls, and were compared with each other. RESULTS: In the acute stage of stroke, undernourishment was significantly (P=0.000) more prevalent in the ICH group (62%) than in the CI group (25%) or controls (13%). On the other hand obesity was present in 10%, 24% and 17% in patients with ICH, those with CI, and controls, respectively, which was not significantly different (P=0.461). Only abdominal skinfold thickness was significantly greater in patients with CI than in those with ICH or controls. Conclusions - Our results illustrate that undernourishment is prevalent in acute stroke patients, significantly more so in patients with ICH than in those with CI. Stroke patients, especially those with ICH, should receive special nutritional intervention starting immediately after admission.
