Chondrodermatitis nodularis chronica helicis.

Chondrodermatitis nodularis chronica helicis is a painful nodule of the external ear. These uncommon lesions are most often encountered on the helix in white men older than 40 years, although they also rarely occur on the antihelix in women. The lesions frequently present with exquisite tenderness that interferes with sleep. While the cause of this dermal inflammatory process is not known, long-term trauma or sun damage may play a role. Recurrences often complicate treatment if all sites of inflammation are not eradicated. Surgical treatment is generally recommended, either by wide excision or by deep shave and treatment of the underlying cartilage.

The effects of dietary tryptophan on chronic maxillofacial pain and experimental pain tolerance.

This study investigated the effects of daily administration of three grams of tryptophan in conjunction with a high carbohydrate, low fat, low protein diet on chronic maxillofacial pain, experimental pain thresholds, and anxiety and depression. In a double-blind study, 30 chronic pain patients were randomly assigned to a tryptophan or placebo group. At the initial appointment and 4 weeks later, the patients' subjective ratings of their pain were recorded, electrical tooth pulp stimulation was used to measure pain thresholds, and psychological tests of depression and anxiety were administered. Over the 4 weeks of the study, there was a greater reduction in reported clinical pain and a greater increase in pain tolerance threshold in the tryptophan group than in the placebo group. The group did not differ in anxiety and depression--for all subjects there was lowered depression and anxiety over the 4 weeks of study.

Comparative field evaluation of divided-dosing and reduced feed intake upon treatment efficacy against resistant isolates of Teladorsagia circumcincta in sheep and goats.

The effects of modifying drug administration and food intake upon the efficacy of fenbendazole against resistant isolates of Teladorsagia (Ostertagia) circumcincta were investigated using naturally infected sheep and goats. Administration of the manufacturer's recommended dose (MRD) as two 2.5 mg kg(-1) bodyweight doses divided by a 12-hour interval resulted in an apparent increase in efficacy of over 28 per cent compared with conventionally treated sheep. Withholding feed for 24 hours before treatment at the MRD resulted in respective increases of 39.7 per cent (P<0.05) and 25.2 per cent compared with conventionally treated sheep and goats. A combination of food withdrawal and divided dosing did not further increase treatment efficacy in goats. It is suggested that withholding feed, divided-dosing or combining these methods may extend the useful life of the broad spectrum class of anthelmintics.

Benefit of bicarbonate dialysis during CAVHD.

The effect of bicarbonate dialysate (BD) on acid-base status in six pediatric CAVHD patients was examined during seven episodes of metabolic acidosis. When metabolic acidosis was not corrected with CAVHD, a sterile BD was substituted for either acetate- or lactate-based dialysate. Pre- and post-BD substitution levels of lactate, HCO3, PCO2, anion gap, and pH were recorded, as well as dose of intravenous (i.v.) bicarbonate. Improvements in pH and serum HCO3 were seen in all seven cases. Anion gap decreased in all but one of the patients who were switched from lactate to bicarbonate dialysate, with improvement most marked in those patients with marked elevation of the anion gap. No adverse effect on PCO2 was noted. Lactate dialysate may be less effective when serum lactate levels are high, and may contribute to further elevation of lactate levels and anion gap. These data suggest that bicarbonate dialysate may be preferable to lactate or acetate dialysate in CAVHD patients with persistent metabolic acidosis.

The efficacy of dental sealants for an adult population.

A 1-year pilot study was conducted to determine the retentive capacities of dental sealants on adult teeth. It proposed to utilize the knowledge and experience gained in sealant application, as a means of attacking the problem of the increase in restored teeth at risk in the aging population. Eighty patients aged 30-50 had Delton tinted sealant applied on one to four contralateral nonhomologous, first or second, acceptable molar pairs. Fifty-five patients completed the evaluation at 1 year, with a 68% retention for 250 teeth: teeth listed as sound, 94.1%; IC and IC-amalgam, 93.7%, and amalgam, 44.9%.
