Longitudinal study of tolyl-reactive IgE antibodies in workers hypersensitive to TDI.

Three workers with TDI hypersensitivity were evaluated for IgE antibodies to TDI over a period of 13 months. A radioallergosorbent test (RAST) system was employed using p-toly(mono)isocyanate-human serum albumin antigen covalently bound to cyanogen bromide-activated paper discs. IgE antibody titers were consistently elevated in two individuals who experienced several bronchial hypersensitivity responses to TDI during the study period. The responses were either solely asthmatic or asthmatic accompanied by cutaneous hypersensitivity reactions. By contrast, antibody titers in a third subject who had not experienced any hyersensitivity reactions during the study period continually decreased, falling to insignificant levels after 12 months. In the absence of renewed TDI exposure, sensitive workers may have titers indistinguishable from those of workers exposed to TDI but without sensitivity to the chemical.

Hapten-specific respiratory hypersensitivity in guinea pigs.

Respiratory hypersensitivity to small chemical determinants (haptens) was produced in guinea pigs by repeatedly exposing the animals to aerosols of hapten-ovalbumin conjugates. In this way, reactivity toward p-azobenzenearsonate and p-tolyl isocyanate was induced. Carrier (ovalbumin)-specific respiratory reactions were not produced. Hapten-specific respiratory hypersensitivity was accompanied by the production of hapten-specific antibodies. The method for inducing hapten-specific hypersensitivity can be applied to screen various industrial chemicals for their sensitizing abilities toward the respiratory tract.

A controlled study to evaluate directed masturbation in the management of primary orgasmic failure in women.

This paper presents the results of a prospective controlled study evaluating a programme of directed masturbation against a combined sensate focus and supportive psychotherapeutic approach in the management of female primary orgasmic failure. Of the 20 patients who followed the masturbation programme 90 per cent gained orgasmic capacity compared with 53 per cent of 15 patients who were treated conventionally. Eighty-five per cent of the patients treated by the masturbation programme and 47 per cent of the control group of patients became coitally orgasmic on at least 75 per cent of coital occasions. The difference is statistically significant at the 5 per cent level. The results suggest that directed masturbation is an effective adjunct in the management of primary female orgasmic failure.

A histologic and electronic evaluation of electrosurgical currents: nonfiltered full-wave modulated vs. filtered current.

A controlled system for examining the tissue alteration and healing of electrosurgically produced wounds was established. Standardization of width, depth, stroke, amperage, average voltage, and wave form was achieved. A histologic comparison of tissue alteration and healing produced by a conventional blade and two electrosurgical currents was carried out. The need for standarization of instrumentation and clinical procedures is discussed.

Combined effects in toxicology--a rapid systematic testing procedure: cadmium, mercury, and lead.

A testing procedure is described for the assessment of the toxicological response (e.g., acute toxicity or mutagenicity) of any combination and number of chemical, physical, and biological agents, with no more effort for a particular combination than for a single agent. The method provides a simple, sensitive, and quantitative index of synergism, antagonism, and additivity, and it has been demonstrated experimentally in rats by determining the acute lethality of combinations of cadmium, mercury, and lead salts. In a combination of two metal salts, the dose of one metal of the pair was fixed at or near the no-effect level while the dose of the second metal was increased until the entire dose-response curve was obtained. To evaluate interactions of the three metals, the previous pair of metals were kept fixed at their combined extrapolated LD1 level, and the third metal was increased. The statistical treatment of the data employed a computer program that did not involve probit transformations, but rather the approximate linear relationship between the fractional response and the logarithm of the dose. A particular combination could be synergistic, antagonistic, or additive, depending on the relative doses employed. Generally, a combination was synergistic when the most toxic member was present at or near its LD1 dose in the presence of the much less toxic member; the same combination was protective when the least toxic member was present at or near its LD1 dose. The results clarify apparently contradictory reports regarding the biological effects of metal combinations. The application of the testing procedure to combinations of mutagens is described, and an example is cited involving, for a particular bacterial mutagen, a combination of N-methyl-N'-nitro-N-nitrosoguanidine with ethylmethanesulfonate.

Effectiveness of teaching interviewing and communication skills to physiotherapy students.

Recognition of the importance of effective interviewing and communication skills, and the concomitant need to teach such skills to students in the helping professions are both relatively recent phenomena in rehabilitation medicine. The purpose of this study is to evaluate the impact of the model developed at the Institut de réadaptation de Montréal (IRM model) in the teaching of interviewing skills to physiotherapy students in the clinical setting. Twenty-six final-year physiotherapy students were assigned to two groups of 13 each, one of which received structured training in interviewing skills over a four-week period while the other received no specific instruction. Analyses of the appraisals of pre- and post-test student-patient interviews--the appraisals having been done not only by the students themselves, but by the patients and the independent raters--showed that scores on each evaluation improved significantly following the students' training program. The group receiving no training showed no significant improvement. The authors conclude that a short clinical training program using the IRM model can be effective in improving the interviewing skills of physiotherapy students.

Ceramic shade determination: current technique for a direct approach.

A method for selecting the optimal shade of a ceramic restoration is presented. The shade is determined using a direct approach, facilitating communication with the laboratory. The dentist generates a custom shade tab using unfired dental porcelain powder mixed with an organic binder which has a refractive index that is adjusted to the fired material. Hue, chroma, value, and translucency are verified by direct comparison with the tooth to be replicated. Necessary adjustments are ascertained prior to fabricating the restoration, minimizing postfabrication alterations.
