Bilateral forefoot ischemia as a premonitory symptom of mixed cryoglobulinemia.
Cryoglobulins are composed of cold-sensitive immunoglobulins that precipitate upon cooling. As the cutaneous vasculature of the extremities is commonly exposed to colder temperatures than the body core, this precipitation often occurs in cutaneous, or even digital vessels. Hyperviscosity from the precipitated proteins can incite local thrombosis in otherwise normal vessels, which is manifested clinically as ischemic ulceration. In previously injured vessels, as seen with atherosclerotic occlusive disease, cryoglobulin precipitation can lead to thrombosis of larger vessels, with the consequence being more severe ischemic necrosis. A case of bilateral forefoot ischemia is presented where the precipitating cause of the gangrenous changes appears to be the development of a mixed cryoglobulinemia and a B-cell lymphoma. Tibial angioplasty, plasmaphoresis, and chemotherapy directed at the B-cell lymphoma allowed limb salvage with bilateral transmetatarsal amputations.
Attraction of ethyl ether extracts of 232 botanicals to oriental fruit flies, melon flies, and Mediterranean fruit flies.
The Mediterranean fruit fly, Ceratitis capitata (Wiedemann), the melon fly, Dacus cucurbitae Coquillett, and the oriental fruit fly, D. dorsalis Hendel, three Hawaiian tephritids of economic importance, were exposed to traps each containing one of 232 ethyl ether extracts of air-dried botanicals. The principal concern was to establish which extracts were attractive judged on the basis of an index determined by the number of times more flies were attracted to the trap baited with water and extract than were attracted to traps baited with water only. Female Mediterranean fruit flies were attracted to 61 extracts and melon fly females to 31 extracts. Female oriental fruit flies were not much attracted. Many extracts were attractive to males of the three species. The extract of Coffea robusta Linden (Family Rubiaceae) was the only material attractive to both sexes of all three species.
The treatment of the expectant mother with deep venous thrombosis.
Experience with 12 expectant mothers with DVT from 1978 to 1983 supports heparin therapy. After early experience with four patients with various doses of intravenous and subcutaneous mini-heparin both in and out of the hospital, we have selected a program of outpatient, self-administered adjusted subcutaneous heparin. There has been no maternal or fetal mortality. There have been no serious bleeding or post-phlebitic complications, although two patients have had flare-ups of superficial phlebitis during the follow-up over the last four-and-a-half years. There have been three subsequent pregnancies, two of which were first trimester miscarriages and one uneventful term delivery of a normal infant.
