Cowden's disease in three siblings: electron-microscope and immunological studies.

Cowden's disease was diagnosed in three siblings (two sisters and a brother) in a Jewish Israeli family of Yemenite origin. The typical mucocutaneous lesions of the disease were present in all three cases. There were hamartomas involving other body systems, including euthyroid multinodular goiter (in all 3 cases), gastrointestinal polyposis (in 2 cases) and hemangioma (in one case). Developmental anomalies were found in all 3 cases. Histological examination of mucocutaneous lesions was in accordance with previous descriptions, including the findings compatible with trichilemmoma observed in cutaneous facial papules. Extensive electron-microscope studies of these facial lesions yielded no evidence of viral particles. Immunological studies, carried out mainly in two cases, revealed a decrease in complement level in the serum and impairment of T cell function.

Tinea capitis due to Microsporum canis in infants.

Three cases of scalp involvement by M. canis in infants are briefly reported. The first was a 2-month-old female with a few erythematous spots on the scalp, the second an 8-month-old male with lesions resembling seborrhoeic dermatitis and the third an 11-month-old male having areas of folliculitis of a kerion type. The father of the second case was found to have a tinea barbae infection due to the same fungus. Stray kittens were the most probable source of infection in all these cases.

Unusual aspects of granulomatous dermatophytosis.

Described are three male patients showing bizarre manifestations of granulomatous dermatophytosis, all in the hand region. One had a small infiltrated nodular lesion near the thumb which was mistaken for a common insect bite. The second had a violaceous-yellowish plaque, also on the thumb, composed of 2-3 small subcutaneous nodules. The third case had five reddish intracutaneous nodules on the dorsum of the hand. Puncture of these nodules revealed the presence of fungal elements and culture yielded T. rubrum in all cases. Each patient was also found to have a fingernail affected by the same agent. Histologic sections in each case showed granulation tissue in the dermis and hypodermis, mainly in the vicinity of hair fragments with spores. These lesions appeared to be variants of Majocchi's granuloma trichophyticum.

Reduced graft-versus-host reaction of T cells in Kaposi's sarcoma: a possible prognostic indicator.

In 20 patients with Kaposi's sarcoma (KS) the level of B and T lymphocytes in the peripheral blood, as well as the function of T cells measured by the graft-versus-host reaction (GVHR), were investigated. Analysis of the results regarding the distribution of skin lesions showed that the mean GVHR was within normal range in 13 patients in whom the lesions were confined to the distal portions of the extremities. In 7 patients, who demonstrated generalized distribution of the eruption, the mean GVHR was significantly lower in comparison with the control group. In view of our results we may conclude that cellular immunity is impaired in KS patients with generalized distribution. Testing T cell function is recommended as a possible prognostic indicator.

Solcoderm in the treatment of solar and seborrheic keratoses.

Solcoderm treatment was applied to 46 patients with solar keratosis and 40 with seborrheic keratosis. Evaluation at regular follow-up over a 10-week period showed disappearance of the lesions without any remnant in most cases. Treatment was found to be effective, convenient, and with essentially no side effects.

New data on the epidemiology of Tinea cruris in Israel.

Among 250 patients with tinea cruris (160 males, 90 females) lesions of both tinea pedis and tinea unguium were found in 221 and lesions of only tinea pedis in another 25. The four children were free of associated dermatophytic lesions. The predominant causative agent was T. rubrum, which was also found to be responsible for the hairy area invasion of the beard and pubis in 12 patients and for the widespread lesions of tinea corporis in 10 patients with lymphoma. None of these patients had noticed the discrete nail and foot involvement. It is therefore recommended that all patients with tinea cruris be subjected to careful examination of all possible mycotic foci, particularly the feet and nails.

[Squamous cell carcinoma in discoid lupus erythematosus foci].

Development of squamous cell carcinomas in lesions of discoid lupus erythematosus (DLE) has been observed extremely rarely. This paper reports on two patients with squamous cell carcinomas in DLE-lesions that had preexisted for decades. Because this complication is observed so rarely, individual predisposing factors are discussed.

[Perianal condylomata acuminata in a 13-month-old girl].

We report on the occurrence of condylomata acuminata in the perianal region of a 13 months old girl. This observation seems to be remarkable because of the early onset as well as the fact that all other published case-reports of condylomata acuminata of this age group are concerned with boys.
