[Levels of mucin-antigen 3EG5 in the blood and bronchoalveolar swabs in patients with tuberculosis, sarcoidosis and fibrosing alveolitis].

The levels of the antigen mucin 3EG5, a marker of the activity of interstitial lung diseases, were studied in the sera and bronchoalveolar swabs of patients with active tuberculosis, sarcoidosis of the respiratory organs, and idiopathic and exogenous allergic alveolitis. Its higher blood levels were detectable in patients with alveolitis (exogenous allergic ones to a greater extent) and absent in active tuberculosis and sarcoidosis. There was a correlation of the levels of the antigen mucin 3EG5 in the blood and bronchoalveolar swabs with the degree of lymphocytosis in the endopulmonal cytogram. The fact that there is a high variability of the blood levels of the antigen mucin 3EG5 in the range of nosological entities calls the differential diagnostic informative value of this parameter to question.

[Modern principles of treating bronchial obstruction in pulmonary tuberculosis patients].

The efficiency of bronchodilating therapy described in the Federal Program for patients with chronic obstructive pulmonary diseases was evaluated in patients with lung tuberculosis with bronchial obstruction, treated in accordance with the Federal Program. Obvious advantages of modern inhalation therapy in comparison with the traditional methods were demonstrated: respiratory symptoms decreased and bronchial patency improved. These results recommend the therapy presented in the Federal Program for wide practical use.

[Experience in long-term use of berodual in the treatment of patients with chronic obstructive bronchitis].

AIM: To study clinical efficiency of 12-month administration of berodual in chronic obstructive bronchitis (COB). MATERIALS AND METHODS: The trial entered 33 patients with at least 2-year history of COB and forced expiratory volume per 1 sec (FEV1) not less than 45% of normal value. RESULTS: The best response of clinical parameters and external respiration was observed throughout 3 weeks of berodual inhalations. Dyspnea and cough were cured by the end of the treatment in 40 and 26% of the patients, respectively. Exercise tolerance has improved, FEV1 has increased for the year from 71.9 to 79.1%. CONCLUSION: Long-term berodual monotherapy is able to inhibit progression of bronchial obstruction and symptoms of COB.

[Blood ozonation in the treatment of patients with progressive pulmonary tuberculosis concurrent with diabetes mellitus].

Twenty nine patients with progressive pulmonary tuberculosis in the presence of diabetes mellitus were examined. Fifteen of them (a main group) had an additional course of intravenous dissolved ozone once 6-8 days (4 mg/ml per 400 ml of isotonic sodium chloride). By the end of month 2, 7 patients stopped massive bacterial expiration, which correlated with clinical improvement while a control group (n = 14) showed no clinical and laboratory changes. Following 3 months, 3 control patients stopped massive bacterial expiration. At month 6, 11 patients of the main group also did this, decay cavities became close in 8 patients while in the controls, these figures were 8 and 7, respectively. The use of systemic blood ozoning eliminated chemotherapy resistance and accelerated abacillation and healing in the decay cavities. The mechanism of action of dissolved ozone is discussed.

[Effectiveness of extracorporeal ultraviolet blood irradiation in patients with chronic obstructive bronchitis].

The use of extracorporeal ultraviolet blood radiation (EUVBR) in the treatment of 67 patients with chronic obstructive bronchitis (COB) showed some specific features of the clinical course of bacterial complications of the basic process. With EUVBR, most patients exhibited normal temperatures, less signs of intoxication and coughing, and rare rale in the lung than in the controls. A most objective criterion for the efficiency of EUVBR was a marked increase in the forced expiratory volume per sec after the first 3 procedures, which improved the patients' condition, diminished exercise-induced dyspnea. The hemogram of the patients undergone EUVBR in combination with antibiotic therapy indicated a substantial drop in erythrocyte sedimentation rates following the first 3 procedures. The growth of pathogenic and opportunistic microbes from the sputum showed a great (10-fold) reduction when EUVBR and antibiotic therapy were concurrently used. The findings suggest that EUVBR has high therapeutical benefits in the treatment of COB.

[Effectiveness of extracorporeal ultraviolet blood irradiation in treatment of chronic obstructive bronchitis in pulmonary tuberculosis].

The use of extracorporeal ultraviolet blood irradiation (EXUVBR) in the complex treatment of patients with chronic forms of pulmonary tuberculosis (cavernous, fibrocavernous) concurrent with chronic obstructive bronchitis (COB) has demonstrated a positive effect of the photo-modified autoblood on the course of COB. The findings have suggested that the magnitude of clinical symptoms of COB was nearly halved, the forced expiratory volume per second increased, the counts of stab neutrophils and lymphocytes and erythrocyte sedimentation rate became normal. Analysing the bacterial isolation rate showed a significant decrease in the number of Mycobacteria tuberculosis detected by luminescence microscopy after a session of EXUVBR. The latter used in pulmonary tuberculosis concurrent with COB promotes the enhanced efficiency of treatment of patients with these combined abnormalities.

[Correction of bronchial obstructive syndrome and antituberculous drugs-induced eosinophilia in patients with pulmonary tuberculosis by using plasmapheresis].

The paper provides the results of a follow-up of 70 patients with active pulmonary tuberculosis in whom the administration of antituberculous drugs induced eosinophilia and bronchial obstructive syndrome. To eliminate the side effects of antituberculous therapy, a plasmapheresis regimen was performed in 44 patients, the remaining patients were given only bronchodilators and antihistamine drugs. Plasmapheresis as a means for correcting drug-induced eosinophilia and bronchial obstructive syndrome was found to be more effective than drug therapy and, in some cases, enabled antituberculous therapy to be continued, without changing a combination of drugs. It is recommended that plasmapheresis should be used in cases of inadequate efficiency of conventional methods for correcting drug intolerance.

[The immunomodulating action of cyclosporin A in vitro and in vivo on the lymphocytes of patients with alveolitis].

The paper presents the first results of using cyclosporin A (CsA) to treat lymphocytes during their extracorporeal immunomodulation (EIL) in patient with fibrotic alveolitis of various etiology. Two-hour lymphocytic incubation in the medium containing 0.1-10 micrograms per ml of CsA was sufficient for CsA to show its in vitro immunosuppressive effect, which resulted in a substantial inhibition of a proliferative lymphocytic response to mitogens and antigens. Administration of CsA-treated lymphocytes induced no profound structural changes in lymphocytic subpopulations (CD3, CD4, CD8), but it was followed by a reduction in the baseline high proliferative lymphocytic response to PHA. The clinical effect, alveolitis alleviation was noted in all patients. It is suggested that clinical effects may be produced by a local concentration of the treated lymphocytes and their transferred CsA as well.

[Use of bronchovaxom in the treatment of patients with chronic obstructive bronchitis and bronchial asthma].

Polyvalent vaccine bronchovaxom (BV) was studied for efficacy against chronic obstructive bronchitis and bronchial asthma exacerbation. Good results were reported: reduction of disability duration, number of recurrences, cough intensity, discharged sputum. Laboratory tests discovered that BV brought about IgE decrease, IgA, T3, T4/T8 increase in bronchoalveolar lavage.

[Impaired free radical oxidation and structural-functional state of erythrocytes in chronic respiratory insufficiency and a method of their correction].

To examine erythrocytic structural and functional changes and lipid peroxidation impairments, which are caused by respiratory failure, and to study whether small-volume extracorporeal blood membrane gas exchange (SVEBMGE) can correct these disorders, a clinical and experimental studies were undertaken to examine 15 patients with tuberculosis complicated by Degree III respiratory failure. Respiratory failure was found to activate lipid peroxidation which was accompanied by lower blood antioxidative activity and correlated with the structural and functional changes of red blood cells and their population composition. The application of SVEBMGE to correct experimental hypoxemia in Degree III respiratory failure complicating pulmonary tuberculosis was demonstrated to normalize the population composition of red blood cells showed the substituting and antioxidant-stimulating effect of oxygen, by contributing to a reduction in the rate of free radical oxidation.

[The use of an ozonized sorbent in treating patients with progressive pulmonary tuberculosis combined with hepatitis].

After analyzing comparative data on various treatments for progressive pulmonary tuberculosis combined with hepatitis, it is inferred that ozonized sorbent hemocarboperfusions are effective modality against the above condition. Ozone does not activate lipid peroxidation in blood, while it might activate electrochemical blood oxidation on the sorbent membrane and produce a direct therapeutic effect on hepatocytes.

[The use of plasmapheresis in treating the broncho-obstructive syndrome in patients with pulmonary tuberculosis].

Examination included 81 patients with the broncho-obstructive syndrome, which in 45 patients was combined with active pulmonary tuberculosis. Plasmapheresis was used in 43 patients. Treatment proved to be effective in patients suffering from bronchial asthma who underwent plasmapheresis; attacks fully discontinued in 70.9 x 8.15 patients. Plasmapheresis had no unfavourable influence on the specific process, it promoted fuller restoration of external respiration parameters and reduction of the level of acute-phase proteins.
