[Gentamycin kinetics when administered into the eyes via soft contact lenses].

Pharmacokinetics of gentamicin administered into the eye by means of instillation or hydrophilic contact lenses was studied comparatively. The results of the study showed that gentamicin levels in the humor of the eye anterior chamber after the antibiotic administration with soft contact lenses were higher at all periods than those after instillation. Higher retention time of gentamicin therapeutic doses in the humor of the eye anterior chamber after the antibiotic administration by means of soft contact lenses is also an advantage of this administration route.

Open-ended vasectomy, sperm granuloma, and postvasectomy orchialgia.

The presence of a sperm granuloma at the vasectomy site prevents epididymal pressure build-up, perforation, and the formation of an epididymal sperm granuloma. It thus enhances reversibility of the vasectomy and lessens the likelihood of epididymal discomfort. In two prospective vasectomy series, a sperm granuloma was intentionally allowed to form by not sealing the testicular end of the vas. The sperm granuloma resulted in no instance of orchialgia, but created a greater risk of spontaneous recanalization. This latter problem could only be solved by more careful sealing of the upper end of the vas. In a separate series of nine patients vasectomized elsewhere and specifically referred to us for chronic and persistent postvasectomy orchialgia, seven had no sperm granuloma at the vasectomy site. Pain in these cases was localized in the epididymis and was relieved by vasovasotomy. Any technique of vasectomy carries a very small risk of orchialgia, whether due to the presence of a sperm granuloma at the vasectomy site or to increased epididymal pressure.

[Use of contact lenses in progressive myopia].

Effects of contact lenses on clinical refraction and vision function were examined in patients with progressive myopia. A total of 535 patients were examined; contact lens correction was employed in 385 of these, 150 ones wearing spectacles have made up the reference group; the follow-up period was up to 5 years. Refraction remained unchanged by the end of the follow-up in 73.2 percent of patients using contact lenses, whereas in the reference group this value has made up 10.8 percent. The stabilizing action of contact lenses is explained mainly by improved function of the accommodation system of the eye, weakened in progressive myopia, and by improved quality of the retinal image.

[Significance of the study of the pachymetric index in the evaluation of the process of eye adaptation to contact lenses].

The authors have examined the thickness of the cornea in 270 patients with weak and moderate myopia during adaptation of the eye to soft or rigid contact lenses. They have revealed a linear correlation between the period of wearing the lens and the degree of the pachometric index change during adaptation of the eye to contact lenses. The maximum permissible period for daily wearing of contact lenses have been determined, not involving essential shifts in water-salt metabolism of the cornea. Optimal schedules for managing the adaptation process with rigid and soft contact lenses have been developed, based on the regularities of the corneal thickness changes related to the length of the lens wearing.
