[Development of meningioma after x-irradiation of the head].

A case with one-stage removal of pus from the cavities of two abscesses in a 49-year-old female through a supratranstentorial approach is described. The postoperative period was uneventful. A follow-up four months later showed that the woman was healthy and continued working.

[Stereotaxic method of treating extrapyramidal spastic torticollis].

A total of 196 stereotaxic operations were performed on different subcortical structures (ventrooral thalamic nuclei, subthalamic area, Cajal's nucleus) in 152 patients with various forms of extrapyramidal spasmodic torticollis. A classification of the various forms of torticollis is suggested and 4 degrees of severity of the disease are distinguished. Study of the late results showed stable and marked improvement or practical recovery in more than 50 per cent of patients. Bilateral stereotaxic operations produce a good stable effect twice as often as unilateral operations (68% of patients). The choice of the side on which the first operation will be performed should be based on the suggested classification of the forms of the disease with due account for the predominant affection of different groups of muscles on one or both sides.

[Association of persistent primitive trigeminal artery and an aneurysm of the middle cerebral artery].

A case in which the persisting trigeminal artery is combined with arterial aneurysm is described. This is the 26th case with such combination reported in the world literature and the 4th with combination with an aneurysm of the middle cerebral artery. The arterial aneurysm was manifested by two subarachnoid hemorrhages, and the persisting trigeminal artery only by mild dysfunction of the first branch of the trigeminal nerve. The case described is an additional proof in favour of the theory of the congenital origin of arterial aneurysms of the brain.

[Vascular factors in the etiology of hemihyperkinesis].

Pathological conditions of the cerebral vessels and main arteries of the head on the side opposite to the side of the involved limbs are the etiological factors in one third of the patients. Cerebral angiography and computer tomography make it possible to verify vascular lesions of the subcortical structures which could cause the development of hemihyperkinesis . These foci of affection were most frequently located in the striapallidal system of the hemisphere contralateral to the affected limbs.

[Stereotaxic clipping of cerebral arteriovenous aneurysms].

The method of stereotaxic clipping was used in the surgical treatment of 14 patients on whom 16 operations were conducted for clipping of the afferent vessels of arteriovenous aneurysms. A stereotaxic atlas was used for the first time to determine the localization of the aneurysms and the structures they occupied. In 12 patients the aneurysms were found to be unsuitable for radical operation. Besides angiography, examination of the volumetric cerebral blood flow was conducted by the modified Kety and Schmidt method to appraise the eficacy of the operation. The examination of 5 patients showed marked (approximately by half) decrease in the bloodflow after operation in 3 patients and its normalization in 2 patients.

[Stereotaxic biopsy of brain tumor using computer tomography].

The work discusses the results of the clinical application of a modified method of stereotaxic biopsy of deeply located brain gliomas according to the results of computer tomography. In some patients with gliomas of subcortical localization stereotaxic biopsy was combined with cryodestruction of the tumour. The method makes it possible to extend the possibilities for precise diagnosis of deeply located three-dimensional processes in the brain, to clearly define the limits of their operability and to cause an effect on them by means of the cryosurgical method.

[Prognostication of the outcome of surgical treatment of hemorrhagic strokes using a computer].

A new mathematical method is suggested for making the prognosis of the results of surgery for haemorrhagic strokes due to essential hypertension and atherosclerosis. Twenty-two signs characterizing the patient's state are fed into the computer trained on the basis of 124 case reports of patients, both surviving and dying after surgery. The computer operating in accordance with the "Kopa-3" programme works out a vital prognosis for both the medicamental and surgical treatment. The prognosis proved correct in 90% of the cases. The described method permits an objective determination of the indications for surgery in hemorrhagic stroke.
