[Surgical treatment of erythromelalgia].

Two children (an 11-year-old boy and a 15-year-old girl) with erythromelalgia and uncontrollable pain in the lower extremities were treated by stereotaxic destruction of VPL and CM and (in one case) by partial rhizotomy. Pain in the extremities and the clinical manifestations of erythromelalgia disappeared after surgical treatment (follow-up periods of 5 and 2 years, respectively). The mechanism of the therapeutic effect of these operations is still not clear. Similar cases are not described in the literature.

[Combined surgical treatment of associated lesions of the major arteries of the head].

Concurrent affection of the main arteries of the head is the cause of vascular cerebral insufficiency in more than half of the patients, which makes necessary combined operations on the extra- and intracranial arteries. The sequence of the operations is determined by the degree of the threat of occlusion of the stenosed artery or by the side on which the cerebrovascular insufficiency is manifested. Information on the regional cerebral blood flow and the results of ultrasonic dopplerography and computed tomography are additional criteria for determining the sequence and the terms of the operations. Experience in 61 combined operations allows the authors to consider them to be an effective method for the surgical management of concurrent affections of the main arteries of the head.

[Stereotaxic removal of intracranial hematomas].

The high lethality and frequent postoperative complications in surgical treatment of intracerebral hemorrhages stimulate the search for new methods of their management. The introduction of computer tomography into clinical practice provided the grounds for elaborating a stereotaxic method for sparing removal of intracerebral hematomas. Experience in the clinical use of the method (54 operations on 51 patients) allows its results to be considered encouraging. Lethal outcomes amounted to 24%, while the possibilities of functional restoration were higher than in the other types of surgical and nonoperative treatment.

[Treatment of hemihyperkinesis by stereotaxic surgery on the basal ganglia].

Hemihyperkinesia develops in childhood as a rule as a consequence of meningoencephalitis. The authors operated on 60 patients, performing 76 stereotaxic interventions on the basal ganglia of the brain. It is established that these operations are the only effective method of treatment of the disease. A marked therapeutic effect was noted in over 70 percent of patients, which persists for many years. There were neither fatal outcomes nor stable complications. The best results were produced in one-stage stereotaxic cryodestruction of VL and subthalamic area.

[Destruction of the entry zone of the posterior roots as a method of treating chronic pain in traumatic damage to the brachial plexus].

To relieve a severe pain syndrome caused by trauma of the brachial plexus, the authors performed operation for destruction in places of traumatic avulsion of the posterior roots from the spinal cord in the projection of the posterior lateral sulcus ("destruction of the entry zone of the posterior roots"). Those roots whose representation corresponded to the spread of the pain syndrome were torn away as a rule. In cases of its wider spread along the dermatomes destruction was combined with residue-tomy of the preserved neighboring roots located above and below the avulsion. In 20 of 24 patients the result of the operation was good or quite satisfactory--the pain syndrome was relieved completely or by 75% and more. The pain diminished approximately by 50% ("mild improvement") in 3 cases and approximately by 25% ("poor result") in one case. The insufficient effect of the operation may be due to a coarse cicatricial-adhesive process in the region of the avulsion of the roots in the posterior lateral sulcus of the spinal cord. Surgical treatment of the pain syndrome in traumatic injuries of the brachial plexus by means of destruction of the entry zones of the posterior roots is a justified and effective method.

[Surgical treatment of deformities of the extracranial division of the vertebral artery].

Bends and twisting are encountered most frequently in the first segment of the vertebral artery (VA), annular and spiral torsions are rare findings. In insufficiency of circulation in the vertebrobasilar channel, operation is indicated in VA bends at an angle of no less than 90 degrees and twisting and torsion with the arc radius of 5 mm and less. Surgery includes removal of the fibrous strands, arteriolysis, desympathization, scalenotomy, and immobilization of the artery in a cuff of muscular or fat tissue. As the result of surgery the deformities may be corrected completely or changed to elongations with the arc radius measuring 10 mm and more. Operative interventions were performed for deformity of the first VA segment on 25 patients with good immediate and late results.
