An immunohistochemical and ultrastructural comparison of the effects of 2-bromo-alpha-ergocryptine on intrasellar and transplanted rat pituitaries.

Following 2 weeks of administration of 2-bromo-alpha-ergocryptine, a marked decrease was observed in prolactin immunoreactivity of the grafted pituitaries, whereas no reduction was noted in the intrasellar pituitaries. No evidence of crinophagy was revealed by electron microscopy in prolactin cells of 2-bromo-alpha-ergocryptine-treated rats.

Growth hormone-producing pituitary adenoma with giant secretory granules.

Unusually large, oval and pleomorphic secretory granules were noted by electron microscopy in an acidophilic adenoma of the pituitary. The tumor, which was removed by surgery from a 42-year-old woman with elevated blood growth hormone levels and the clinical features of acromegaly, was found to contain growth hormone by the immunoperoxidase technique. This ultrastructural abnormality of secretory granules was not reported so far and was not seen among the 58 cases of growth hormone-producing adenomas investigated in our laboratory. The present case clearly shows that the cytogenesis and cellular composition of pituitary adenomas cannot be determined by solely examining the size and shape of secretory granules.

Autolytic changes in the rat adenohypophysis. A histologic, immunocytologic and electron microscopic study.

Thirty-nine adult female Long Evans rats were decapitated and the heads stored at room temperature. The pituitaries were removed at intervals from 30 minutes to seven days, fixed, embedded and studied by histology, immunocytology and electron microscopy. Histologically, changes were noticeable after two hours postmortem. Immunoperoxidase staining showed postivity for growth hormone, prolactin, FSH, LH and TSH up to seven days after sacrifice, appearing even stronger in the advanced stages of autolysis. Fine structural alterations were evident at 30 minutes and more conspicuous later. Changes included dilation, partial degranulation and whorl formation of RER, swelling of Golgi complexes and mitochondria, chromatin clumping, lysis, rhexis and pyknosis of nuclei, cytosegresome formation and disruption of cell membranes. Secretory granules remained well preserved throughout, although some exhibited fusion or reduced electron density. Dilation of capillaries with accumulation of erythrocytes, platelets and fibrin fibers were prominent findings. The severity of changes varied considerably from cell to cell indicating that the rate of autolysis is not the same among different cell types and is possibly affected by the actual functional state of the cell. It appears that increased membrane permeability and disruption of plasmalemma represent important steps in the autolytic process.

Farmer's lung disease: long-term clinical and physiologic outcome.

To determine the long-term effects of farmer's lung disease and the factors influening the outcome, 141 patients with farmer's lung disease were evaluated. At the time of the last follow-up, 29 patients had died and 92 (mean age, 54 years) were studied clinically, physiologically, and radiologically. The mean length of disease was 14.8 years (range, 2.25 to 40 years). Symptoms at the time of the last follow-up included complaints of cough (33 per cent of the patients), breathlessness while walking on the level (20 per cent), breathlessness on minor exertion (14 per cent), and breathlessness while at rest (3 per cent). Twenty-eight per cent had chronic bronchitis. Thirty-nine per cent (36 of 92 patients) had some evidence of interstitial changes on roentgenogram. Abnormal vital capacity was present in 11 patients (12 per cent), abnormal total lung capacity in 11 (12 per cent), and abnormal CO difussing capacity in 27 (30 per cent). The ratio of one-sec forced expiratory volume to forced vital capacity was abnormal in 23 patients (25 per cent), and arterial PO2 was abnormal in 39 (40 per cent). Patients with a history of 5 or more symptomatic recurrences had significantly smaller values (P less than 0.05) for vital capacity, total lung capacity, and CO diffusing capacity than did those patients with less than 5 recurrences. There was no significant relation between continued farming or length of disease and lung function. On the basis of several measurements of airway function, 34 of the patients (58 per cent) were found to have some abnormality, It is concluded that symptomatic recurrences may be the most important factor in determining the danger of progressive disease. Persistently positive precipitins were correlated with decreased CO diffusing capacity. Moreover, airway disease is relatively uncommon but does occur, and in some cases it is a possible consequence of farmer's lung disease.

Nuclear inclusions in the human adenohypophysis. A fine-structural study of nontumorous and adenomatous pituitaries.

Electron microscopy revealed the presence of nonviral nuclear inclusions in human nontumorous as well as adenomatous adenohypophysiocytes, regardless of cell type. Based on ultrastructural appearances, the inclusions have been classified as simple bodies, complex bodies and filamentous aggregates. Many transitional forms were noted between the simple and complex bodies, however, no relationship between the nuclear bodies and filamentous aggregates was found. It can be concluded that the three types of inclusions are normal nuclear constituents. Since no accumulation of these structures was observed in cells associated with enhanced secretion it appears that they are not related to hormonal hyperactivity in the human adenohypophysis.

Morphogenesis and significance of fibrous bodies in human pituitary adenomas.

In the course of light and electron microscopic studies of 142 surgically-removed human pituitary adenomas, 28 tumors were found containing fibrous bodies composed of type II microfilaments with an average width of 115A. These spherical structures, measuring up to 4-5 micrometer occur exclusively in sparsely granulated growth hormone cells and acidophil stem cells, but as revealed by the immunoperoxidase technique, contain no growth hormone. Fibrous bodies are located in the Golgi region and are consistently associated with Golgi membranes and smooth-surfaced endoplasmic reticulum. Their association with centrioles is thought to be anatomical rather than functional. Several adenoma cells possess spherical formations composed entirely of smooth-walled membranes or transitional forms between smooth tubules and type II microfilaments, suggesting that smooth membranes may play a key role in the production of fibrillar substance. Fibrous bodies appear to be reliable morphologic markers and are valuable in the differential diagnosis of pituitary adenomas.

Renal angiomyolipoma: ultrastructural study.

An angiomyolipoma from a twenty-six-year-old woman was studied by electron microscopy. The tumor was composed of a mixture of mature smooth muscle cells, fat cells, and abnormal blood vessels. MAny of the smooth muscle cells showed excessive intracytoplasmic accumulation of glycogen, while the abnormal blood vessels lacked a normal media.

Pituitary adenomas associated with elevated blood follicle-stimulating hormone levels: a histologic, immunocytologic, and electron microscopic study of two cases.

The histologic, immunocytologic, and electron microscopic features of pituitary adenomas surgically removed from two men with elevated levels of blood follicle-stimulating hormone (FSH) are described. In both cases, the high blood FSH levels were reduced after surgery. By light microscopy, the tumors corresponded to chromophobic adenomas, and the immunoperoxidase technique revealed the presence of immunoreactive FSH (beta-subunit) in the cytoplasm of the adenoma cells. By electron microscopy, the adenoma cells differed considerably from nontumorous FSH cells. They were smaller and angular and contained numerous microtubules as well as spherical secretory granules measuring 100 to 250 nm in diameter and often lining up along the cell membranes. The present findings are consistent with the view that FSH-producing adenomas may originate in the human pituitary.

Prolactin cell adenomas of the human pituitary. Morphologic features of prolactin cells in the nontumorous portions of the anterior lobe.

Twenty pituitary glands, harbouring prolactin cell adenomas, obtained from autopsy of male and female patients of various ages and dying of different diseases have been investigated by the immunoperoxidase technique in order to reveal the morphologic changes of prolactin cells in the nontumorous protions of the anterior lobes. For comparison, 40 nontumorous pituitary glands and 20 pituitaries lodging adenomas not consisting of prolactin cells have also been studied. In all four prolactin cell adenoma cases, the anterior lobes contained numberous prolactin cells outside the tumors, indicating a lack of involution of prolactin cells in the nontumorous portions of the pituitary glands.

Method for correcting laboratory model deep-well disposal system data for hydrostatic pressure effects.

A pressure chamber for determining the effect of increased hydrostatic pressure on growth and metabolic activities of groundwater bacteria is described. The chamber was used to show that moderate increases in pressure (to about 100 atmospheres) result in increased growth of mixed cultures of industrial-injection-well bacteria and in the more complete degradation of formate and nitrate by these bacteria, as compared with identical cultures at atmospheric pressure.

Ectopic ACTH syndrome: clinicopathological correlations.

Ten out of 164 cases of bronchogenic carcinoma showed pathological evidence at necropsy of the ectopic ACTH syndrome. All occurred in association with oat-cell carcinoma, constituting 19% of that group. The pathological features consisted of adrenocortical hyperplasia confined to the zona fasciculata and Crooke's hyaline change in the pituitary. Immunoperoxidase stainable ACTH was detected in the pituitary but not in the carcinoma tissue, a surprising finding, which may be due to the different nature of ACTH present in tumour tissue. The ectopic ACTH syndrome was diagnosed ante mortem in only four out of 10 patients on the basis of hypokalaemia and metabolic alkalosis. The lack of clinical pointers in all but terminal cases is discussed as well as possible measures for earlier diagnosis.

Fine structural changes in the liver of methotrexate-treated psoriatics.

Ultrastructure of 55 liver biopsies obtained from 52 patients, all but 1 with psoriasis and all but 5 treated with methotrexate (MTX), have been studied. In MTX-treated psoriatics, liver cell nuclei showed reduced electron opacity with frequent cytoplasmic invaginations and glycogen inclusions. In the mitochondria nonspecific changes, such as pleomorphism, gigantism, paracrystalline inclusions, compartmentalization, signs of division and sequestration via autophagosomy were noted. The pericanalicular ectoplasm exhibited widening with accumulation of microfilaments. Detachment of desmosomal plaques between hepatocytes was common. A striking finding was the hyperplasia of fat-storing perisinusoidal (Ito) cells. No definite correlation was found between morphology, duration of disease, treatment as well as cumulative dose of MTX.

Intrasellar paraganglioma associated with hypopituitarism.

A 37-year-old with a history of postpubertal arrest of sexual development and delayed growth was found to have an enlarged sella turcica. The clinical and biochemical features were consistent with hypopituitarism. A tumor was removed transsphenoidally that, through light microscopy, histochemistry, and electron microscopy, proved to be a paraganglioma. We report the possible origin of intrasellar paraganglioma, as well as the pathogenesis of hypopituitarism.
