[Pellagra--a forgotten disease].

In the civilized world the traditional causes of pellagra such as hunger, malnutrition are disappeared, but the disease has not disappeared after all. In the well developed countries the current etiologic causes are the following: alcoholism, psychiatric disorders, diseases causing cachexy, malabsorption and some drugs. In the case reported by the authors the malnutrition, accompanying the chronic alcoholism was the cause of the diarrhoea, dementia and dermatitis. The correct diagnosis was established after four years of repeated psychiatrical and medical treatment.

[Inverted sigmoid diverticulum simulating polyps].

The authors write about the inversion, a rare complication of the diverticulum of the sigmoid colon. The colour, surface of the diverticulum, 15 and 8 mm in diameters, were the same as the colon mucosa, it was soft and mobile by tacting. Biopsy was performed which caused a serious complication, a perforation to the free abdominal space, requiring a surgical operation. The inverted diverticulum can be distinguished from a polyp not by endoscopy, but by double contrast barium enema. Detecting a long, soft, mobile mass in the colon, must be thought about an inverted diverticulum and the biopsy must be omitted.

[Follow-up of cases of diabetes mellitus existing over 25 years].

The life prospects of patients grew parallel with the improving possibilities of the treatment of diabetes mellitus. In 36 insulin-treated patients with diabetes existing longer than 25 years, obesity, smoking and alcohol consumption were infrequent, microangiopathic complications were moderate and displayed protracted course. Lability of carbon hydrate metabolism does not characterize the long lasting diabetes. Higher insulin requirement was found only with patients whose sulphonylurea resistance made later necessary the insulin therapy. Organic manifestations of macroangiopathy, hypertension and hypercholesterinemia were often observed. Two patients remained free of complications even after 28- and 31 years long diabetes. Long-term insulin dependent or after oral administration insulin treated diabetes mellitus continuing even in old age is a relatively benign slowly progressing disease.

[The effect of a fiber-rich diet on the postprandial blood sugar level in diabetics].

The effect on the postprandial blood sugar level of a müzli prepared from dry fruits and rough ground grain flakes of rich ballast material content has been examined in insulin non-dependent and dependent diabetic patients. In random order the patients consumed in the morning for 3 days test food of rich and for 3 days of poor roughage content. In insulin non-dependent patients the blood sugar concentration was significantly lower 1-2 hours following the meal than after the consumption of a traditional food of poor roughage content. No significant change of the blood sugar concentration was observed in insulin dependent diabetic patients. Food of rich roughage content is suitable for the moderation of the postprandial blood sugar peak in insulin non-dependent diabetes.

[Hypotension in insulin hypoglycemia in diabetics with autonomic neuropathy].

Hypotension was observed in 11 cases of hypoglycemic condition of 7 diabetic patients suffering from autonomic neuropathy. Following the iv. administration of glucose the hypotension became stronger. Upon glucagon the blood pressure kept decreasing. Neuropathy of the symphathetic nervous system, the osmotic effect of glucose and the vasodilating effect of glucagon were the causes of the hypotension. Hypotension in hypoglycemia is a symptom referring to autonomic neuropathy. Hypotension aggravates the neuroglycopenic disturbance. Slow injection of glucose solution moderates the decrease of the blood pressure. The use of glucagon in the hypoglycemia of diabetic patients with autonomic neuropathy must be subjected to consideration.

[Combination treatment with insulin and sulfonylurea. Results of a 5-year study].

For establishing the mode of treatment 55 non-insulin-dependent diabetic patients with later sulfonylurea resistance were examined with respect to insulin and insulin-sulfonylurea sensitivity. The test is helpful in reducing the time of clinical evaluation, in the easy and rapid selection of the suitable medication. 37 patients with sulfonylurea resistance were also resistant to insulin. In 21 of them the combined insulin and sulfonylurea test showed a significant fall of blood sugar. The latter received combined insulin and sulfonylurea therapy for 5 years. During this observation period treatment proved to be effective, the fasting, postprandial and daily values of blood sugar, glycosuria and HbA1 concentrations showed constant decrease. With the combined insulin and sulfonylurea therapy and low insulin doses carbohydrate metabolism may be normalized. In 2/3 of patients repeated daily injection of insulin was not required. Severe hypoglycemia did not occur in the course of treatment.

[Differentiation of stress-induced hyperglycemia from diabetes mellitus in cerebrovascular strokes by hemoglobin A1 determination].

The author observed high sugar level in 22 patients who had cerebrovascular diseases (cerebral hemorrhage and cerebral infarction). They tried to diagnose the cause of the increased sugar level with HbA1 test after the registration of the patients to the hospital within 24-48 hours. They found normal, less than 8% HbA1 concentration in 12 patients, increased, more than 8% values in 10 cases. They made oral glucose tolerance test in 19 survived patients after the acute symptoms had finished. The oral glucose tolerance test with increased HbA1 concentration showed in 5 patients diabetes, in 3 cases impaired glucose tolerance. The author observed from all hyperglycemic patients with low HbA1 level only 2 cases impaired glucose tolerance. Hyperglycemia with increased HbA1 concentration indicates previous carbohydrate intolerance.

[Disorders of electrolyte balance and metabolism in hormone-secreting bronchial cancer].

Four cases of bronchial carcinoma were observed which showed the picture of an ectopic ACTH-hormone syndrome. References to an endocrinopathy resulted from the hypopotassaemia and the metabolic alkalosis. Signs of Cushing's syndrome appeared only in one case. By disturbances of the electrolyte and acid-base-balance the functions of the skeletal and smooth musculature as well as of the heart muscle were much impaired. In two cases the clinical symptoms and the laboratory findings still before the manifestation of the bronchial carcinoma, in the third case simultaneously referred to the correct diagnosis. Due to the misleading cardiac findings and the fulminant course of the disease in the fourth case the diagnosis was clarified only by autopsy. the disturbances of the ion balance and of the metabolism had contributed to the respiratory insufficiency, had evoked disturbances of the cardiac rhythm and had unfavourably effected on the course of of the disease.
