[Arthrodesis of the upper ankle joint. Indications, technique, results].

From 1984 to 1990 a total of 119 arthrodesis of the upper ankle joint were performed at the Hospital of Accident Surgery of the Trade Association in Frankfurt am Main. The results of 98 patients after an arthrodesis of the upper ankle joint are documented by an x-ray control series and patient files, including the expertise on the medical status of pensioners plus a follow-up examination of 34 patients. The indications and the results are discussed on the basis of the various procedures. The results confirm the method of a compressions arthrodesis with an extension screw in case of a posttraumatic arthrosis of the upper ankle joint, while a fixateur externe should be used in case of chronical osteomyelitis, osteoporotic bones, extensive tissue swellings and after a pilon or talus fragment fracture.

[Preoperative diagnosis of cystic parathyroid adenoma by ultrasonic-guided fine-needle puncture and parathormone determination].

In one patient with primary hyperparathyroidism, a cystic parathyroid adenoma was diagnosed pre-operatively by ultrasound-guided fine-needle aspiration biopsy and the detection of an elevated parathyroid hormone concentration in the aspirate.

[Intraoperative ultrasound studies in pancreatic surgery].

In 32 patients with benign or malignant disease of the pancreas direct ultrasound imaging of the pancreas was performed during the operative procedure. Benign could be distinguished from malignant lesions on the basis of different ultrasound patterns, changes in the pancreatic duct and size of the organ. A further improvement in diagnostic accuracy was obtained by intraoperative staging with ultrasound-directed small-needle biopsy of that part of the pancreas which looked suspicious macroscopically and by ultrasound. Previously unknown tumour infiltration was also revealed by ultrasound imaging. Intraoperative ultrasound is a new helpful method for the surgeon. Since it is easy it should be used in all doubtful instances, even though the interpretation requires considerable experience.
