Adenosquamous carcinoma of the gallbladder with spindle cell features. A light microscopic and immunocytochemical study of a case.

A case of adenosquamous carcinoma of the gallbladder showing extensive spindle transformation is presented. By light microscopy, areas showing interwoven fascicles of fusiform, poorly differentiated cells closely resembling a sarcoma were seen to merge imperceptibly with areas showing more obvious glandular and squamous cell features. Immunocytochemistry utilizing tissue-specific antibodies against intermediate filaments demonstrated the exclusive presence of prekeratin antibodies in both components of the tumour, thus establishing the epithelial nature of this neoplasm. The importance of immunological phenotyping in the differential diagnosis of epithelial tumours of the gallbladder showing pseudosarcomatous features is underscored.

Sclerosing hemangioma of the lung. Immunohistochemical demonstration of mesenchymal origin using antibodies to tissue-specific intermediate filaments.

A case of pulmonary sclerosing hemangioma of the lung was studied by light microscopy and indirect immunofluorescence using tissue-specific antibodies against intermediate filament subunits. All the tumor cells stained positively and exclusively with antivimentin antibodies thus indicating their mesenchymal origin. In addition, positive staining with cytokeratin antibodies was observed in cells lining cystic spaces and elongated slit-like spaces were occasionally encountered throughout the tumor, disclosing residual epithelial elements. Using brightfield microscopy, the keratin-positive areas were identified as distorted alveolar spaces lined by hyperplastic respiratory epithelium entrapped within the tumor. It is proposed that these entrapped epithelial elements may account for the conflicting results obtained by different investigators in previous attempts to determine the histogenesis of this tumor.

Primary thymic carcinoma. An unusual case originating in a lymphocytic rich thymoma.

A case of thymic carcinoma arising within a lymphocyte rich thymoma is reported. The undifferentiated carcinoma contained cellular elements resembling choriocarcinoma but could be differentiated therefrom by positive staining for prekeratin antigen and an absence of staining for B-HCG antigen utilizing immunohistochemical techniques.

Sarcoidosis: a cause of bilateral hilar lymphadenopathy after excision of malignant melanoma of the arm.

Bilateral hilar lymphadenopathy developed in a patient one year after excision of a malignant melanoma of the left arm. There were no signs of distant metastases. Hilar lymph node biopsy specimen obtained by mediastinoscopy showed multiple noncaseating granulomas consistent with sarcoidosis, without evidence of tumor cells. Sarcoidosis developing in the course of a preexisting neoplasm is rare, but must be considered in order to establish a proper diagnosis and withhold unnecessary treatment.

Distinctive immunofluorescent labeling of epithelial and mesenchymal elements of carcinosarcoma with antibodies specific for different intermediate filaments.

A carcinosarcoma of the lung, as well as the paratracheal lymph nodes from the same patient, were subjected to immunofluorescent labeling with antibodies to tissue-specific intermediate-filament subunits, including desmin, vimentin, and prekeratin. Within the tumor mass two distinct populations of malignant cells were found: prekeratin-positive cells, corresponding to the carcinomatous component of the tumor, and vimentin-positive cells, corresponding to the sarcomatous elements. Tumor cells were also detected in lymph node metastases in which only the prekeratin-containing carcinoma cells were found. In view of the strict specificity of antivimentin and anti-prekeratin for cells of mesenchymal or epithelial origin, respectively, it is proposed that the two components of the carcinosarcoma are derived from distinct cell types and are not morphologic variants of the same tumor.

Use of antibodies to intermediate filaments in the diagnosis of metastatic amelanotic malignant melanoma.

Immunofluorescent staining of tissue from a lung tumor detected 12 years after excision of a primary malignant melanoma of the skin was negative for prekeratin and positive for vimentin, indicating that the tumor was not epithelial in origin and excluding carcinoma from the differential diagnosis. Complementary conventional staining with hematoxylin-eosin confirmed the melanocytic origin of the tumor, indicating that it was probably an amelanotic metastasis of the original malignant melanoma. The findings in this case demonstrate the potential usefulness of immunohistochemical microscopic characterization of specific intermediate filament proteins in the diagnosis of otherwise ambiguous cases of amelanotic melanoma.

The fate of patients with lung cancer: five-year review of 250 cases.

Two hundred fifty patients with bronchogenic carcinoma were followed for at least 5 yr after diagnosis. In 187 (75%), the tumor was diagnosed within 6 mo of the first clinical manifestation. Among 205 patients who were not operated on because of tumor spread or because of poor general condition, 139 (67.5%) died during the first year, 15 (7.5%) during the second year, and 51 (25%) were not available for follow-up and are presumed dead. Forty-five patients underwent curative resection, with one early death. Of the 44 survivors, 30 had lobectomy, 1 had sleeve lobectomy and 13 pneumonectomy. The cancer-free survival rate in this group was 32% at 5 yr and 30% at 7 yr. Except for small cell carcinoma, there was no correlation between tumor histology and prognosis. Due to the late appearance of symptoms, early diagnosis of lung cancer is impossible with present-day methods.

Inflammatory fibrous histiocytoma of the bronchus.

A 33 year-old woman underwent lobectomy of the middle and lower lobes of the right lung because of inflammatory fibrous histiocytoma of the bronchus. To our knowledge, this is the first description of such a tumor originating in the bronchus.

Regression of malignant thymoma with metastases after treatment with adrenocortical steroids.

A patient with a malignant lymphoepithelial thymoma and pleural metastases is described. Incidental treatment with adrenocortical steroids after an allergic reaction resulted in a marked regression of the masses for a period of 18 months. A second course of treatment resulted in complete radiological regession of the tumors, and the patient has remained symptom free with normal chestX-rays for a further period of 18 months.
