Ante-and postnatal evaluation of the Dandy-Walker syndrome.

In three cases an antenatal diagnosis of isolated Dandy-Walker cyst was made at 22-28 weeks of gestation by ultrasound examination. Biparietal growth and the relative severity of the anomaly remained constant, so that all cases could be safely delivered at term. Postnatally, the diagnosis was confirmed during the first days of life by ultrasound and computed tomography (CT). Early neonatal shunting procedures were performed. Dual shunts were inserted in all three cases, and mental and physical development were observed to be normal during a follow-up of from 1.5 to 4.5 years. Early antenatal diagnosis of Dandy-Walker cyst may improve the prognosis and reduce the mortality, which has been reported to be 30%-48%. Dual shunting of both the cyst in the posterior fossa and the lateral ventricles has proved to be a safe and reliable method during the neonatal period.

Shunting procedures in the management of intracranial cerebrospinal fluid cysts in infancy and childhood.

Widely diverging opinions on the optimal therapy for intracranial cerebrospinal fluid cysts (CSF), mainly arachnoid cysts and the Dandy-Walker cysts, exist. Excision of the cyst walls in the treatment of the Dandy-Walker cyst has been replaced by shunting procedures, but the recommended method for primary treatment of arachnoid cysts in childhood is still cyst wall excision. Membrane excision is, however, often complicated by recurrence, subsequently requiring shunting-procedures. In a series of 19 cases primary shunting of intracranial CSF cysts proved to be a reliable method. In those cases where hydrocephalus (ventricular dilatation) is present at the time of the primary operation the ventricles should be shunted as well as the cyst. The catheter from the ventricle and that from the cyst should be connected to the same valve, otherwise an increased risk of intracranial herniation exists. The prognosis for infants and children suffering from intracranial CSF cysts is in general good; in 17 out of 19 cases mental development was normal and in 15 out of 19 motor development was normal. The risk of permanent motor damage seems to be particularly high when an arachnoid cyst is located on the quadrigeminal plate.

Ventriculoatrial or ventriculoperitoneal shunts in the treatment of hydrocephalus in children?

The data on all 881 primary or revision shunt operations performed on 158 paediatric patients treated in Gothenburg, Sweden from 1967 to 1984 and 101 patients treated in Oulu, Finland from 1968 to 1983 were pooled for the purpose of comparative evaluation of the function of ventriculoatrial (VA) and ventriculoperitoneal (VP) shunts. Ventriculoperitoneal shunting was the method of choice in Gothenburg and ventriculoatrial shunting in Oulu. The results of the 723 operations (305 VA and 418 VP shunts) were evaluated as the other 158 operations were for ventriculostomas, shunt removals and other procedures. 80 children had exclusively VA shunts and 133 children had exclusively VP shunts. Irrespective of the method of analysis the VP shunts were more frequently infected. The estimated relative risk for obstruction of the shunt (Meyer-Kaplan method) was shown to be significantly higher in VA shunts, but only at a low level of statistical significance (p less than 0.1). All other shunt complications were distributed uniformly in both groups. There was, however, a trend towards a higher mortality among children with exclusively VA shunts. Therefore it was concluded that despite the higher risk for infection in VP shunts, these still should be considered a safer choice, as the complications of VA shunts present greater risks.

Postural sway during standing and unexpected disturbance of balance in random samples of men of different ages.

Postural sway behavior while standing and during and after an unexpected disturbance of balance was examined in men aged 31 to 35, 51 to 55, and 71 to 75 years. The groups studied were random samples of men living in the town of Jyväskylä in 1981. Postural sway while standing was more pronounced in the older age groups, whereas after a disturbance of balance postural sway was nearly the same in all age groups. The functioning of the postural control system was correlated with the vibratory threshold on the ankles, grip strength, as well as with aerobic and anaerobic capacity within the age groups. Further, in the youngest age group the functioning of the postural control system was poorer among those who had been subject to noise at their work. The correlation between health status and postural sway was negligible. The results may indicate the partly parallel and similar effects of aging, poor physical fitness, and harmful environmental factors on the functioning of the postural control system.

Myopia, use of eyes, and living habits among men aged 33-37 years.

As a part of a more extensive research project on health and functional capacity among a representative sample of men, the living habits and backgrounds of myopic men between 33 and 37 years of age living in the town of Jyväskylä were studied in comparison with non-myopic men of the same age. The prevalence of a negative spheric equivalent, which was considered as a criterion of myopia, was 25%. It was found that the myopic, on the average, had been more interested in reading from childhood onwards, their educational and occupational status were higher, and their body structure was lighter. As children they had taken less physical exercise, but as adults no difference was found in physical exercise between the myopic and non-myopic.

Group A streptococcal peritonitis in infancy.

Primary peritonitis in infancy is a rare condition. It is usually associated with a severe pathological process, such as nephrotic syndrome. When it affects previously healthy infants it causes diagnostic and therapeutic difficulties. Pneumococci, streptococci and Haemophilus influenzae are possible causative agents, other causes are rare. The clinical course of two infants with group A streptococcal peritonitis is described.

Collagen of slow twitch and fast twitch muscle fibres in different types of rat skeletal muscle.

The appearance of collagen around individual fast twitch (FT) and slow twitch (ST) muscle fibres was investigated in skeletal muscles with different contractile properties using endurance trained and untrained rats as experimental animals. The collagenous connective tissue was analyzed by measuring hydroxyproline biochemically and by staining collagenous material histochemically in M. soleus (MS), M. rectus femoris (MRF), and M. gastrocnemius (MG). The concentration of hydroxyproline in the ST fibres dissected from MS (2.72 +/- 0.35 micrograms X mg-1 d.w.) was significantly higher than that of the FT fibres dissected from MRF (1.52 +/- 0.33 micrograms X mg-1 d.w.). Similarly, the concentration of hydroxyproline was higher in ST (2.54 +/- 0.51 micrograms X mg-1 d.w.) than in FT fibres (1.60 +/- 0.43 micrograms X mg-1 d.w.), when the fibres were dissected from the same muscle, MG. Histochemical staining of collagenous material agreed with the biochemical evidence that MS and the slow twitch area of MG are more collagenous than MRF and the fast twitch area of MG both at the level of perimysium and endomysium. The variables were not affected by endurance training. When discussing the role of collagen in the function of skeletal muscle it is suggested that the different functional demands of different skeletal muscles are also reflected in the structure of intramuscular connective tissue, even at the level of endomysial collagen. It is supposed that the known differences in the elastic properties of fast tetanic muscle compared to slow tonic muscle as, e.g., the higher compliance of fast muscle could at least partly be explained in terms of the amount, type, and structure of intramuscular collagen.

Mechanical properties of fast and slow skeletal muscle with special reference to collagen and endurance training.

The mechanical properties of the slow soleus and the fast rectus femoris muscle under passive stretching were studied in endurance trained, untrained and lathyritic rats, aged 3 months. The soleus muscle with more abundant and cross-linked collagen had higher ultimate tensile strength and tangent modulus compared to the fast rectus femoris muscle which, on the other hand, had higher maximum strain. The inhibition of collagen cross-linking by lathyrism resulted in decreased tensile strength and stiffness, especially in the soleus muscle, whereas endurance training showed the opposite effects. It is supposed that the properties of collagen partly explain the capacity of slow muscles to maintain posture and to perform prolonged dynamic work. The effects of training on the tensile properties further indicate the close relationship between intramuscular collagen and the endurance capacity of muscles.

Mineral density of calcaneus in men at different ages: a population study with special reference to life-style factors.

The mineral density of the calcaneus was measured in representative samples of men, aged 31-35, 51-55, and 71-75 years, utilizing a 125I-gamma-ray attenuation method. There was a gradual decrease in bone mineral density from the youngest to the oldest age group. Those men who participated in regular physical activity or who were non-smokers showed higher values for bone mineral density when compared to sedentary men or smokers, respectively. Furthermore, bone mineral density correlated positively with physical performance capacity and negatively with the prevalence of chronic diseases.

Functional capacity of men born in 1906-10, 1926-30 and 1946-50. A basic report.

This interdisciplinary study aims at describing the functional capacity of men belonging to different generation cohorts. The development of methods for the assessment of functional capacity is also the purpose of the study. Functional capacity is defined as a hierarchy comprising physical, mental and social areas into which individuals can be placed depending on the levels of the assessed functions. The study also aims at analyzing environmental and individual factors which are assumed to cause variation in the levels of functions. The purpose of this first report is to describe the framework of the study, the methods and main results concerning differences between cohorts and occupational groups. The sampling frame consisted of the population register of Jyväskylä town in January 1981. The basic populations were men in three age groups, born in 1946-50, 1926-30 and 1906-10. From each age group a systematic random sample of 250 men was made. Persons who had moved or died by the end of August 1981 or were living in institutions (6 men) were removed from the samples. The sample sizes were further randomly reduced so that the final samples consisted of 183, 188 and 176 men. At the time of the study the ages of the cohorts were 31-35, 51-55 and 71-75. The study included postal questionnaires, interviews and laboratory examinations. The basic questionnaire dealt with the background information, living conditions, life history, occupational history, social contracts and social participation, close human relations, use of time, health status and living habits. The second postal questionnaire dealing with psychic capacity included the following main items: life changes, self-realisation, personal trait anxiety, social fears, coping, self-respect, and the purpose of life. The basic questionnaire was checked in an interview when the subjects came for the laboratory examinations. The entrance interview also included questions about the most important events of previous 24 hours. In addition the subjects filled in a questionnaire on situational anxiety. The laboratory examinations included the following tests: cognitive capacity (4 tests), audiometry, speech understanding, visual tests, vibration threshold, posture control, reaction and movement time, tapping rate, leg extension velocity, anthropometric measurements, skin measurements, bone mineral density, physiotherapist's examination, physician's examination, blood analyses, isometric strength of muscles (5 muscles), anaerobic power, aerobic power and anaerobic threshold.(ABSTRACT TRUNCATED AT 400 WORDS)

Cardiac tamponade caused by a mediastinal teratoma.

Mediastinal masses in children younger than 2 years often cause symptoms from tracheal compression, but clinical indications later in childhood are less frequent. Mediastinal teratomata arise in the anterior region, and when they produce symptoms these are usually manifestations of tracheal compression, pain, pleural perforation or secondary infection of the tumour. Pericardial perforation is a rare and life-threatening complication. A case of sudden pericardial perforation and cardiac tamponade caused by a mediastinal teratoma is described.

Morphology of the great arteries in chronic experimental coarctation in dogs.

Vascular changes of the great arteries could be one explanation for the serious complications and persistent hypertension after coarcectomy among patients with coarctation. This possibility was studied by evaluating the morphology of the coronary artery, aorta and middle cerebral artery in 7 experimental 9-month-old dogs in which a coarctation of the thoracic aorta had been made at the age of 2 months. A classical coarctation syndrome with high blood pressure and intense collateral circulation developed during growth. Five sham-operated dogs living in the same laboratory environment served as a control group. Morphometric measurements showed medial thickening of the coronary artery wall in the coarctated dogs (135 vs 103 microns, P less than 0.05). The relative amount of glycosaminoglycans of the media was also greater in the study group (14.8% vs 8.1%, P less than 0.02). The relative number of medial nuclei was significantly greater in the coarctated dogs (P less than 0.025), reflecting medial hyperplasia. Vacuolated smooth muscle cells and fragmented elastic membranes were found in the media of the aortas of coarctated animals. No significant differences between the groups were found in the cerebral arteries. It is suggested that re-evaluation of the optimal age of patients with coarctation for surgical repair is obviously necessary.
