[Effect of perioperative antibiotics on septic complications following emergency interventions on the small and large intestine].

The effect of perioperative antibiotics on septic complications in emergency bowel surgery was studied by reviewing the charts of patients. The rate of septic complications was essentially reduced in patients, who had received antibiotics pre-, intra- or immediately postoperatively compared with patients without any antibiotic medication (without antibiotics [n = 25] 6 abdominal wound infections, 5 lethal anastomotic disruptions, 2 urinary infections; with antibiotics [n = 24] 0 abdominal wound infections, 2 lethal anastomotic disruptions, 1 urinary and 1 pulmonary infections). Because of the significant reduction of abdominal wound infections which could be achieved in treating patients (X2 = 6,56; p less than 0,05) with antibiotics, these medicaments should be given in emergency bowel surgery with a further prospective controlled study being not necessary.

[Experimental gallstone formation. Etiological significance of beta-glucuronidase producing bacteria and biliary obstruction].

Bile pigment gallstones were produced in six of six mongrel dogs by narrowing the cystic duct with ligature after a postoperative interval of seven days. beta-Glucuronidase producing group G Streptococcus and Staphylococcus aureus were found in the bile, the gallbladder wall, and the liver. In another trial similar gallstones were produced in four of eight dogs after mere injection of beta-glucuronidase producing Escherichia coli into the spleen resp. in six of eight dogs after injection of E. coli into the colon without ligature of the cystic duct. In an additional series gallstones were produced in six dogs after injection of beta-glucuronidase producing E. coli into the colon plus ligature of the cystic duct. The injected strain of E. coli was found in the bile, the gallbladder wall, the liver, and even within the produced gallstones. In a control group in none of six dogs gallstones were present. beta-Glucuronidase producing group G Streptococcus was found in the gallbladder of one dog, Pseudomonas aeruginosa was found in the gallbladder of another dog and Staphylococcus aureus in the liver and gallbladder wall of a third dog. We conclude from our experiments that merely bacterial infection of the biliary tract can lead to gallstone formation. The bacterial colonization of the liver and the biliary tract is promoted by biliary tract obstruction. beta-Glucuronidase producing bacteria increase the amount of beta-glucuronidase in the bile thus leading to calcium bilirubinate precipitation and gallstone formation by deconjugation of bilirubindiglucuronide.

[Electrical and motor activity of the rectum following low anastomosis].

Electrical and motor activity of smooth muscle of the canine rectum was registered by transanal recording following low rectum anastomosis. The relaxation reflex of the anal canal had disappeared distal of the anastomosis in the first postoperative week, and electrical slow waves and pressure waves were desynchronized proximal and distal of the anastomosis. The resting pressure of the rectal wall had increased above preoperative values. After wound healing had accomplished the relaxation reflex and the synchronization of electrical and mechanical activity proximal and distal of the anastomosis did occur again. It is concluded that the rectal wall provides the pathways for the relaxation reflex of the anal canal. After wound healing has finished these pathways are restored across the anastomosis.

[Continuous arteriovenous hemofiltration in acute postoperative and post-traumatic kidney failure].

Acute renal failure of 40 patients with polytrauma or abdominal surgery was treated by continuous arteriovenous hemofiltration. The excretory function of the kidney could be replaced by the method. Spontaneous urine excretion recurred in five patients after an interval of 11.3 days. The best survival rate was in patients with acute renal failure after bleeding shock. Contrary to the conventional hemodialysis no special personal or apparatus besides the hemofilter is necessary. Thus, continuous hemofiltration is not withheld from patients that can not be moved to dialysis.

[Intraoperative mural pressure measurements in the papilla of Vater (author's transl)].

The mural pressure in the papilla of Vater was measured in fifteen patients with benign stenosis of the papilla of Vater and in fifteen patients with normal papillary function by intraoperative water manometry. There was a zone of high pressure in the papilla of a few millimeters. The mural pressure in normal papillary function ws much higher than in papillary stenosis. There was only a tiny difference of pressure in functional and in fibrotic stenoses. Functional stenoses showed a non-linear pressure-diameter relationship when extended by probes of increasing diameter. Fibrotic stenoses could not be probed or could only be probed minimally. It is concluded that high mural pressures in the papilla of Vater do not represent pathologic papillary function. Functional and fibrotic stenoses can only be differentiated by probing.

[Route of infection of the biliary tract: experimental evidence for an enterohepaticobiliary bacterial cycle (author's transl)].

Suspensions of Escherichia coli bacteria were injected into the colon of ten mongrel dogs. Primarily, the cystic duct and the distal choledochal duct had been ligated. Choledochal bile was continuously collected with a ductal catheter. The bacteria could be cultured from the portal vein blood, the liver, and the gallbladder wall 5 h after injection, but not from choledochal or gallbladder bile. Suspensions of tritiated Escherichia coli bacteria were injected into the portal vein of another 20 mongrel dogs. Now the bacteria could be cultured from the liver, the gallbladder wall, the choledochal and the gallbladder bile 10 min after injection. Elevated radioactivity counts could be found in the tissue and bile samples. In the liver and gallbladder wall, the tritiated bacteria could be seen in phagocytes and capillary ducts by autoradiography. We conclude that there is an enterohepaticobiliary bacterial cycle which could explain the pathogenesis of infections in biliary tract diseases.

[Preoperative prophylactic antibiotics reduce septic complications of colorectal operations (authors' translation)].

On the preoperative day before colorectal operations (n = 36), 3 g neomycine and 3 g erythromycine were administered as oral prophylactic antibiotics. No antibiotics were administered to the controls (n = 24). The number of postoperative septic complications was reduced after preoperative prophylactic antibiotics. There were two abscesses of the abdominal wound, one fecal fistula, and two anastomotic disruptions in the control group but only two abscesses of the abdominal wound in the antibiotic group. No toxic effects were observed during this trial.

[Pressure-diameter-relationship of the papilla duodeni. An experimental study (author's transl)].

While extending the papilla duodeni of dogs by probes of increasing diameter, mural pressure was measured as an equilibration of the hydrostatic pressure within the probes along a distance of 12 mm. Pressure-diameter-curves at the duodenal end of the papilla show the mural pressure promptly increasing to a maximum when partially extended, then declining and rising again when extended extremely. Proximal to the papillary end the mural pressure first declines and then continuously increases with larger probe diameters. The pressure-diameter-curve of the papillary end resembles the length-tension-diagram of isolated smooth muscle in vitro, and represents the normal physiological action of the sphincter zone.

Functional analysis of organic sphincter systems by elastance-measurement. Evaluation of a new method.

The function of organic sphincter systems cannot be described by measuring a static pressure, but only by applying a dynamic procedure of measurement that takes into account the characteristics of smooth muscles. Therefore, the elastance-method has been developed. Experiences with this method and its clinical value are demonstrated when one applies this procedure to measurements in the lower oesophagus, the duodenal papilla and the anal canal.

[Colorectal polyps. Pathologico-anatomical and statistical studies on 3037 polyps].

Report on 3037 colorectal polyps from the Deutsche Klinik für Diagnostik Wiesbaden that were observed within about 6 years and submitted to microscopic examination. The polyps were classified according to the WHO nomenclature, 1976. Adenomas and metaplastic (hyperplastic) polyps were the most frequenty types. The terms 'carcinoma in situ,' 'focal carcinoma,' 'adenoma with severe cellular atypia,' and 'malignant polyp' are explained and discussed in view of possible false interpretations by the surgeon. It is stressed that only the term 'adenoma with severe cellular atypia' (WHO classification) should be retained. In 286 cases, primary multiplicity of polyps was observed. There were recurrent polyp(s) in 154 cases (7%). Most frequently the recurrent polyp(s) corresponded to the type that had been found during the first examination. Finally, some remarks are made concerning the techniques of biopsy and of pathological examination.

[Observations of the surface of the articular cartilage after treatment with hyaluronidase (author's transl)].

1. The articular surfaces of the femoral head of rats have been studied by scanning electron microscopy after in vitro treatment with hyaluronidase. 2. The cartilage surface has been digested mildly by the buffer medium (control). 3. The matrix of the cartilage has selectively been removed by the hyaluronidase. 4. Chondrocytes have been seen on the surface after treatment with hyaluronidase for a longer period. 5. We discuss the result of enzymatic digestion.
