Diet and weight loss: their effect on norepinephrine renin and aldosterone levels.

The effect of low calorie diet on blood pressure, catecholamines, plasma renin activity (PRA) and aldosterone was examined in 22 obese subjects (19 hypertensives and 3 borderline hypertensives). A significant decrease in blood pressure (P less than 0.05) was observed in all patients after 10 days on the diet. Twenty subjects showed a significant decrease in norepinephrine (NE) levels (P less than 0.05) and two patients showed an extreme increase in NE level. On reexamination six months later these two patients presented with high NE levels and predict blood pressure levels despite having lost 15 and 18 kg respectively. The study group showed a significant decrease in mean PRA (P less than 0.05). A correlation was observed between changes in NE levels and PRA (r = 0.42, P less than 0.05) and between changes in PRA and aldosterone (r = 0.54, P less than 0.05). The reduction in blood pressure associated with caloric restriction in these obese patients may be a result of reduced sympathetic nervous system activity.

The efficacy of cellulosic ophthalmic inserts for treatment of dry eye.

Fourteen patients with dry eye signs and symptoms were treated with a cellulosic ophthalmic insert and conventional artificial tear drops in a cross-over study. Each subject rated symptoms before the study and after each treatment period. At the conclusion of the study, subjects chose which of the two treatments they preferred. Thirteen people preferred the artificial tear drops, and more symptoms were alleviated by them. Tear breakup time was not significantly changed by either treatment. The insert was not well received due to subjective blurring of vision and ocular irritation. Other studies have shown patient preference for the insert, but this may be due to patient selection.

Clinical use of captopril in Takayasu's disease.

Three patients with Takayasu's disease (TD), initially observed with hypertension and/or congestive heart failure, were treated with captopril, an orally active inhibitor of converting enzyme. As arteritis in this disease is diffuse and extensive, surgical correction of renovascular hypertension is often delayed or impossible. Our experience enables us to recommend captopril for conservative treatment in TD.

A comparative study of infective endocarditis.

A survey of hospitalizations at the Chaim Sheba Medical Center, covering two nonconsecutive decades (1951-60 and 1970-79), included 213 patients (228 episodes) with infective endocarditis. The increase in the age of patients and in the frequency of atherosclerotic heart disease, as well as the introduction of techniques such as heart catheterization and open-heart surgery, are responsible for the differences in clinical course, microbiology and therapy. Despite the changed pattern, rheumatic heart disease remained the principal underlying disease in both periods and Streptococcus viridans remained the most common pathogen. Not a single case of drug addiction appeared in our material, despite its increasing frequency in Israel.

Computerized tomography in the diagnosis of congenital hepatic fibrosis.

A case of congenital hepatic fibrosis (CHF), a relatively rare disease, was diagnosed preoperatively on the basis of clinical and radiological findings. The diagnosis was confirmed by computerized tomography and later proven by a liver biopsy performed during cholecystectomy. Increased awareness of this entity may reveal heretofore unidentified cases of CHF.

Use of oral converting enzyme inhibitor, captopril for lateralizing renal venous renin activity.

Captopril was administered prior to renal vein renin sampling in an attempt to select patients amenable to surgical treatment for renin dependent hypertension. Renal venous blood for plasma renin activity was taken only after captopril stimulation. Sampling from the antecubital vein before and after this provocation showed a marked rise in renin, thereby confirming the efficacy of the test. Elimination of the initial selective renal vein sampling shortens the catheterization period without affecting the accuracy and dependability of the procedure.

Observations of denture-supporting tissue during six months of denture adhesive wearing.

The regular use by denture wearers of an appropriate denture adhesive is not associated with any increased incidence in mucosal irritation. The use of such an adhesive can, in fact, reduce the likelihood of the occurrence of tissue irritation and, at the same time, provide the denture wearer with specific benefits--both physical and psychological. The use by 111 denture wearers of a natural gum or a synthetic polymer adhesive for six months was not accompanied by any increase in mucosal irritation of denture bearing tissues. Mucosal irritation present in certain of the subjects at the start of the study was eliminated in almost all cases with the continuing use of the assigned adhesive during the six-month observation period.

The effectiveness of dental floss in reducing gingival inflammation.

Unwaxed and waxed dental floss were clinically evaluated for effectiveness in reducing gingival inflammation. Two different scoring methods were employed. Both types of floss, when administered by a dental hygienist, were very effective, but no significant differences between them could be found. Both scoring methods led to the same conclusions, but one was more sensitive and provided more detailed information.
