Electric depolarization of photosensitized cells: lipid vs. protein alterations.

We have monitored several photosensitized reactions in proteins, liposomes and cells under similar conditions. We found that the depolarization of K(+)-diffusion potential of liposomes or the leakage of an entrapped molecule, calcein, progress at a much slower rate than the photosensitized damage to proteins and the photosensitized killing of bacterial and leukemic cells. X-ray microanalysis revealed that upon light exposure of HP-treated leukemic cells and bacteria, they totally lost their cellular potassium. We deduce that the direct photosensitized oxidation of lipid components cannot cause the depolarization of cells, which in turn could be responsible for their death. A photosensitized damage to protein sites in the cell, probably in the membrane, is a more likely reason for the depolarization, the loss of potassium ions and cell death that is caused in light-activated photodynamic processes.

A Candida albicans homolog of CDC25 is functional in Saccharomyces cerevisiae.

We have cloned, by functional complementation of the cdc25-2 mutation of Saccharomyces cerevisiae, a homolog of CDC25 from the pathogenic yeast Candida albicans. The new gene, named CSC25 codes for a 1333-amino-acid protein. The full length gene, as well as a truncated form coding for 795 amino acids, suppresses the thermosensitive phenotype of cdc25ts mutants. Biochemical analysis has shown that Csc25 activates the Ras/adenylyl cyclase pathway in S. cerevisiae at a rate two to three times faster than Cdc25, under the same conditions. The C-terminal domain of Csc25 is highly similar to the C-terminal domain of Cdc25, to almost the same extent as the C-terminus of the endogenous Cdc25 homolog Sdc25. We show that polyclonal anti-Cdc25 antibodies interact with Csc25 expressed in S. cerevisiae. In addition to the full length protein (approximately 150 kDa), we have found a approximately 50-kDa polypeptide which seems to include the C-terminus of the CSC25 gene product.

Determination of lipoprotein(a): enzyme immunoassay and immunoradiometric assay compared.

Lipoprotein(a) (Lp(a)) concentration in plasma is a strong independent risk factor for pre-mature atherosclerosis. Lp(a) closely resembles LDL. Its protein moiety contains apolipoprotein (apo) B-100 and apo(a). Two enzyme immunoassays (EIAs) for Lp(a) have been developed. In both, polyclonal antibodies for apo(a) are used as capturing antibodies. In the first, Lp(a) is detected with anti-apo(a) (apo(a)-EIA). In the second, detection is carried out with anti-apo B (Lp(a):B-EIA). Neither plasminogen nor LDL cross-reacted in the assays. Lp(a) was also measured using a commercial sandwich immunoradiometric assay (IRMA). This assay uses two monoclonal antibodies for apo(a). One of them, the solid phase antibody, cross-reacted with plasminogen. However, at physiological plasminogen concentrations there was no competition for solid phase binding sites. A quantity of plasma samples (201) were assayed for Lp(a) with the three methods. The best correlation was obtained between the IRMA and the Lp(a):B-EIA (r = 0.909). Correlations between the apo(a)-EIA and the IRMA or the Lp(a):B-EIA were 0.763 and 0.695, respectively. As compared to the EIAs, the IRMA overestimated Lp(a) by about 30%. It is concluded that both the Lp(a):B-EIA and the IRMA reflect the concentration of Lp(a) particles in plasma. In contrast, the apo(a)-EIA measures apo(a) antigen and may therefore be susceptible to the size polymorphism of apo(a).

[A sickle cell homozygote with transfusion deadlock. Favorable outcome with hydroxyurea treatment].

Alloimmunization following multiple transfusions, a frequently observed complication in sickle cell anemia patients, may make subsequent transfusion attempts extremely hazardous. The case of a young woman with sickle-cell anemia is reported; she was hospitalized for the treatment of extensive invalidating leg ulcers. The transfusion program that was initiated as a prerequisite to skin allografts had to be stopped, due to rapid occurrence of multiple alloimmunization. Broad spectrum alloantibodies precluded further selection of compatible blood units. In an attempt to get round this impossibility, a treatment with Hydroxyurea was initiated in order to boost synthesis of F-hemoglobin. After one year under this treatment, the patient's clinical status has clearly improved without requiring any new blood transfusion.

Ecological studies on the brown dog tick Rhipicephalus sanguineus (Acari: Ixodidae) in southern Israel and its relationship to spotted fever group rickettsiae.

An outbreak of spotted fever group rickettsiae (SFGR) was investigated by studying free-living and parasitic stages of ticks in two settlements of equal size and population located 20 km apart in the Negev Desert. Although high morbidity from SFGR was found in one of the settlements (Kibbutz Ze'elim), no clinical cases were observed in the second (Kibbutz Re'im). Using flagging and CO2-trapping, approximately 9 times more ticks were collected in Ze'elim than in Re'im. Rhipicephalus sanguineus (Latreille) was the dominant species in Ze'elim, whereas in Re'im R. turanicus Pomerantzev was the most abundant species. Several physical factors that may account for these differences were investigated. Significantly higher maximum soil temperature as well as ambient temperature above the soil were found in Ze'elim. Differences in soil composition in the two sites were also observed. Tick numbers were especially high during April through October in Ze'elim, whereas in Re'im, ticks were found mainly from April to July. A significant positive correlation was found between temperature and tick population size in Ze'elim. Dogs, sheep, goats, Mus musculus, and Meriones crassus, were more heavily infested with ticks in Ze'elim than in Re'im. The percentage of mice and dogs seropositive to SFGR was the same in both sites. In Ze'elim, 7.1% of dog owners acquired Mediterranean spotted fever during the period 1984-1989 compared with only 1.4% of people without dogs.

Singlet oxygen generation by porphyrins and the kinetics of 9,10-dimethylanthracene photosensitization in liposomes.

Two new sensitizers are introduced for a potential use in photodynamic therapy: Zn(2+)- and MG(2+)-tetrabenzoporphyrin (ZnTBP and MgTBP). A comparative study of the quantum yields of singlet oxygen generation (phi delta) of hematoporphyrin derivative (HpD), Photofrin II (PF-II), Zn(2+)-phthalocyanine tetrahydroxyl [ZnPC(OH)4] and the newly introduced sensitizers ZnTBP and MgTBP in liposomes, as well as the kinetics of a photochemical reaction sensitized by them, was made by employing the fluorescent membrane probe 9,10-dimethylanthracene (DMA). We followed the photosensitization of DMA in real time by monitoring its fluorescence decrease at 457 nm and found that DMA's photosensitization is oxygen mediated. The kinetic traces of the photosensitization reactions were fitted to an analytical function, and the phi delta values were evaluated. At 10 microM sensitizer in an aqueous suspension of 2 mg/mL egg phosphatidylcholine (EPC), HpD was found to have the largest value of phi delta (0.215), followed by PF-II (0.191), ZnTBP (0.023), MgTBP (0.019) and ZnPC(OH)4 (0.005). As a test of the method, phi delta for methylene blue in ethanol was measured and found to be 0.45 as compared to 0.52 reported in the literature. Due to difference in the sensitizers' absorbances at the laser's wavelength, the reaction photosensitized by ZnTBP was the fastest with a time constant of 6.7 min, followed by MgTBP (8.7), PF-II (11.9), HpD (17.1) and ZnPC(OH)4 (31.2), all at equal sensitizers' concentrations and laser intensities. The binding constants of the sensitizers to EPC liposomes are also reported.

Disseminated intravascular coagulopathy caused by acetic acid ingestion.

A case of disseminated intravascular coagulopathy following ingestion of 100% acetic acid is presented. The patient was successfully treated with fresh frozen plasma and cryoprecipitate. Complete resolution of the bleeding diathesis and the disturbed coagulation tests was achieved.

[The AKA-2 sutureless compression anastomosis of the colon and rectum].

According to own experimental studies with anastomoses without any foreign material and based on the first clinical experience with the sutureless compression anastomoses (AKA-2) in colorectal surgery on 140 patients, this anastomotic technique was applied on further 147 patients following resections of the distal colon and rectum. Excluding the 24 patients with primary protective colostomy the leakage rate was 4.0% and out of the 45 patients with an anastomosis of 10 cm or less 2 patients developed fistulae (4.4%). 3 patients showed clinically inapparent fistulae by the routinely performed gastrographin enema. On endoscopy between 2nd and 3rd postoperative week no epithelial defects were found. Due to the biological advantages regarding morphological and functional criterias and a possible oncological advantage the sutureless compression anastomoses (AKA-2) can be recommended.

Psychological tasks for bereaved children.

The grief process in bereaved children is conceptualized as a series of psychological tasks that must be accomplished over time. Early tasks include understanding and self-protection; middle-phase tasks include acceptance and reworking; late tasks pertain to identification and development. This timing model has practical implications for the conduct of psychotherapy at different times during the grief process.

Anti-Cdc25 antibodies inhibit guanyl nucleotide-dependent adenylyl cyclase of Saccharomyces cerevisiae and cross-react with a 150-kilodalton mammalian protein.

The CDC25 gene product of the yeast Saccharomyces cerevisiae has been shown to be a positive regulator of the Ras protein. The high degree of homology between yeast RAS and the mammalian proto-oncogene ras suggests a possible resemblance between the mammalian regulator of Ras and the regulator of the yeast Ras (Cdc25). On the basis of this assumption, we have raised antibodies against the conserved C-terminal domain of the Cdc25 protein in order to identify its mammalian homologs. Anti-Cdc25 antibodies raised against a beta-galactosidase-Cdc25 fusion protein were purified by immunoaffinity chromatography and were shown by immunoblotting to specifically recognize the Cdc25 portion of the antigen and a truncated Cdc25 protein, also expressed in bacteria. These antibodies were shown both by immunoblotting and by immunoprecipitation to recognize the CDC25 gene product in wild-type strains and in strains overexpressing Cdc25. The anti-Cdc25 antibodies potently inhibited the guanyl nucleotide-dependent and, approximately 3-fold less potently, the Mn(2+)-dependent adenylyl cyclase activity in S. cerevisiae. The anti-Cdc25 antibodies do not inhibit cyclase activity in a strain harboring RAS2Val-19 and lacking the CDC25 gene product. These results support the view that Cdc25, Ras2, and Cdc35/Cyr1 proteins are associated in a complex. Using these antibodies, we were able to define the conditions to completely solubilize the Cdc25 protein. The results suggest that the Cdc25 protein is tightly associated with the membrane but is not an intrinsic membrane protein, since only EDTA at pH 12 can solubilize the protein. The anti-Cdc25 antibodies strongly cross-reacted with the C-terminal domain of the Cdc25 yeast homolog, Sdc25. Most interestingly, these antibodies also cross-reacted with mammalian proteins of approximately 150 kDa from various tissues of several species of animals. These interactions were specifically blocked by the beta-galactosidase-Cdc25 fusion protein.

Flow cytometric analysis of colorectal mucosa from patients with Crohn's disease, ulcerative colitis and cancer.

Samples of colorectal mucosa from patients with Crohn's disease, ulcerative colitis and cancer were analyzed by means of flow cytometry. S- and G2-phase fractions were determined and mean values were calculated for different groups of patients. Almost identical results were obtained for inflamed and normal appearing mucosa from patients with Crohn's disease as well as inflamed mucosa from patients with ulcerative colitis. The mean S- and G2-phase fractions in normal appearing mucosa from cancer patients, however, were significantly higher. This seems to be due to the fact that patients with Crohn's disease are between 15 and 45 years old, while cancer patients are mostly over 45. A detailed analysis of the S- and G2-phase fractions in different age groups revealed a slight, but significant increase in colorectal proliferation between 25 and 75 years.

[Diathermy-induced gas explosion in the intestinal tract].

Surgical diathermy is widely used in modern operating rooms, although explosion in the gastrointestinal tract is a known serious complication. We report a case of colonic explosion following use of diathermy. Based on our experience and the available data, it is recommended that in cases of gastrointestinal tract obstruction, poor bowel preparation or following use of oral mannitol for bowel preparation, diathermy should be avoided in opening the gastrointestinal lumen.

Congenital myopathy with oculo-facial abnormalities (Marden-Walker syndrome).

Marden-Walker syndrome present in the neonatal period is characterized by oculo-facial abnormalities, congenital myopathy, and contractures. A newborn infant with a similar pattern of anomalies is reported, and further evidence for autosomal recessive inheritance as well as new neuropathological findings are included.

Dithranol-induced down-regulation of 12(S)-hydroxyeicosatetraenoic acid [12(S)-HETE] receptors in a human epidermal cell line.

The effects of dithranol and its therapeutically inactive oxidation product, danthrone, on 12(S)-HETE binding to the human epidermal cell line SCL-II were studied. Dithranol (0.25-1 microgram/ml), in contrast to danthrone, induced a substantial decrease in 12(S)-HETE binding in a dose-dependent manner. The inhibition occurred after a latency period of 6 h, reached its maximum at 18-24 h and slowly declined thereafter. At a concentration of 1 microgram/ml, the drug led to an approximately 50% decrease in the number of specific high-affinity 12(S)-HEFE receptors (Bmax), whereas receptor affinity (Kd) showed no change. The down-regulation of 12(S)-HETE receptors on epidermal cells by dithranol may contribute to its antipsoriatic action.

A double-blind, vehicle-controlled paired comparison of halobetasol propionate cream on patients with plaque psoriasis.

The efficacy and safety of halobetasol propionate 0.05% cream, an ultra high-potency corticosteroid preparation, was evaluated in a double-blind, vehicle-controlled, paired comparison study. Patients' psoriatic lesions were evaluated before treatment and after 1 and 2 weeks of twice-daily treatment with halobetasol propionate and vehicle. Response measures (plaque elevation, erythema, scaling, and pruritus) were evaluated with a 4-point severity scale whereby the sum provided a total score. Patient self-assessment measures were obtained at the 2-week visit by categorizing his or her global responses to queries about each treatment's "effectiveness" and "overall rating." All efficacy parameters, as judged by the physician, showed statistically significant (p = 0.0001) treatment differences favoring halobetasol propionate at both week 1 and week 2 evaluations. Patient global responses for "effectiveness" and "overall rating" favored halobetasol propionate 0.05% cream over vehicle after 2 weeks of use. No systemic adverse drug effects were reported during the study. No patient was discontinued from the study because of an adverse event, and there was no evidence of skin atrophy after 2 weeks of treatment with either agent. Patient reports of "stings" or "burns" were equally distributed between the active and vehicle treatment groups. This trial demonstrates that halobetasol propionate 0.05% cream is clinically beneficial and without evidence of significant risk in the treatment of plaque psoriasis.

Inhibition of 12(S)-hydroxyeicosatetraenoic acid [12(S)-HETE] binding to epidermal cells by ultraviolet-B.

12-hydroxyeicosatetraenoic acid (12-HETE), the main eicosanoid in skin, is assumed to have both pathophysiologic effects in inflammatory skin diseases such as psoriasis and atopic eczema and a physiologic role in the biology of cutaneous reparative processes. Because 12-HETE exerts its effects via specific high-affinity epidermal receptors, and ultraviolet-B (UV-B) is capable of modulating various cell-surface molecules, the effects of single and repeated UV-B irradiations on the 12(S)-HETE binding sites in a human epidermal cell line, SCL-II, were studied. UV-B (100-300 J/m2) induced a large decrease in 12(S)-HETE binding in a dose-dependent manner. The inhibition occurred after a latency period of 6 h, reached its maximum at 18 h and slowly declined thereafter. A single UV-B dose of 300 J/m2 or repeated irradiation with 50 J/m2 of UV-B resulted in a 70% decrease in the number of binding sites (Bmax), whereas receptor affinity remained unaffected. The modulation of epidermal 12-HETE receptors by UV-B may partly explain the therapeutic effects of UV-B, but possibly also contribute to photodamage to skin.
