A clinical evaluation of two methods of caries prevention in newly-erupted first permanent molars.

A two-stage topical treatment regime, AgF followed by SnF2, was used as a caries preventive measure in newly-erupted first permanent molars in a group of children. Another group of children, from similar geographic and socio-economic backgrounds was treated by a one stage topical application of SnF2 only. The teeth treated were observed at six-monthly intervals for a period of 18 months, and showed a significantly higher incidence of caries developing when treated with SnF2 only.

Scanning electron microscope evaluation of wear of stainless steel and high carbon steel curettes.

Root planing depends on quality instrument cutting edges. The sharpness and wear of stainless steel (SS) and high carbon steel (HCS) dental curettes were compared before and after standardized root planing procedures. Forty working edges of dental curettes, 20 of each alloy, were examined by scanning electron microscope (SEM) prior to any root planing (control), after 15 strokes, and after 45 strokes. Root planing was carried out on freshly extracted, single-rooted teeth, mounted in natural positions in mannikin jaws. Working areas measuring 3 x 5 mm were marked on root surfaces previously exposed by periodontitis. Each working edge was examined at points 1 mm and 2 mm from the tip under the same magnification (x 240). Microscopic observations showed that edge deformation increased from the control (factory sharpened) group, to the "15 stroke" group, and from the "15" to the "45 stroke" group, in both SS and HCS curettes. SS curettes, however, demonstrated significantly more wear than HCS curettes. After 45 strokes, only 5% of the SS curettes were sharp, compared to 20% of the HCS curettes. While 65% of the SS curettes showed severe edge deformation by the end of the study, only 20% of the HCS curettes were severely deformed (P less than 0.01). The results indicate that HCS curettes are more resistant to wear than SS curettes. Further study is indicated to evaluate the relationship between actual bevel dimensions and root planing effectiveness.

Biobehavioral treatment of essential hypertension: a group outcome study.

In a group outcome and follow-up study of 77 patients with essential hypertension, significant reductions were seen in systolic and diastolic blood pressure (BP) and in hypotensive medication requirement. A multimodality biobehavioral treatment was used which included biofeedback-assisted training techniques aimed at teaching self-regulation of vasodilation in the hands and feet. Of the 54 medicated patients, 58% were able to eliminate hypotensive medication while at the same time reducing BP an average of 15/10 mm Hg. An additional 19 (35%) of the medicated patients were able to cut their medications approximately in half while reducing BP by 18/10 mm Hg. The remaining 4 (7%) medicated patients showed no improvement in either BP or medication requirement. Similar reductions in BP were seen in initially unmedicated patients. Seventy percent of the 23 unmedicated patients achieved average pressures below 140/90 mm Hg, with an additional 22% of these patients making clinically significant reductions in pressure without becoming normotensive, and with 8% unsuccessful at lowering pressures to a clinically significant extent. Follow-up data available on 61 patients over an average of 33 months indicated little regression in these results with 51% of the total patient sample remaining well-controlled off medication, an additional 41% partially controlled, and 8% unsuccessful in lowering either medications and/or blood pressures to a clinically significant extent.

Impact of the medical record credential on data quality.

This research study was funded by the Foundation of Record Education of the American Medical Record Association. The Department of Medical Record Administration at the University of Illinois, Chicago, was awarded the grant in the summer of 1984. The purpose of the study was to evaluate the quality of coded and abstracted medical record data and to determine if a relationship existed between data quality and the professional credentials of the individuals who manage and/or supervise the collection of these data. The study consisted of three phases: a telephone interview conducted of 83 hospitals, recoding and reabstracting of medical records performed by the staffs of 59 hospitals, and 34 random on-site visits for a reliability check. Data was collected and analyzed from each of the three phases of the study.

The effect of decalcified bone matrix on the osteogenic potential of bone marrow.

Rabbit bone marrow cells were cultured in diffusion chambers with or without decalcified bone matrix. The chambers were assayed after 28 days for alkaline phosphatase activity, deoxyribonucleic acid (DNA), calcium, and phosphorus contents. Morphologically, marrow cells incubated with or without matrix differentiated to form bone and cartilage. With bone matrix, the calcium and phosphorus contents of chambers were significantly higher than control chambers. Alkaline phosphatase activity and DNA content were not influenced by inclusion of bone matrix. These results indicate that bone matrix constituents exert a stimulatory effect on bone formation from marrow cells. This osteogenic stimulation could be due to the influence of an osteoinductive factor and/or to stimulation of osteoprogenitor cells known to be present in the marrow.

Assessing the efficacy and safety of q. d. theophylline therapy: a multicenter study.

One hundred thirty-two asthmatic patients with bronchial asthma were studied by 20 independent investigators during this 49-day study. One hundred sixty-four patients entered into this study, and up to 132 were judged evaluable for statistical purposes. Criteria for entry included current q 12-hour therapy with an available theophylline product (eg, Theo-Dur, Slo-Bid, etc). Use of beta-agonist aerosols PRN was permitted, as was use of steroid aerosols for short periods of time. Chronic treatment with oral corticosteroids or other oral bronchodilators was prohibited. Baseline peak and trough serum theophylline concentrations (STCs) and performance of pulmonary function tests (PFTs) were determined during q 12-hour treatment. Patients were then switched within one dosing interval to an equivalent single daily dose of Theo-24 and maintained on this therapy for the duration of the study. STC and PFT measurements were repeated on days 8, 21, and 49 of the Theo-24 study period. Throughout the 49-day study, patients measured their peak expiratory flow rate at home, at 8 AM, and at 8 PM using a Mini-Wright Peak Flow Meter. These data, along with asthma symptomatology and frequency of inhaled bronchodilator aerosol usage, were recorded daily in diary form. Three days after the start of Theo-24 treatment, the mean peak STC (11.18 micrograms/mL) was not significantly different from the mean peak STC with q 12-hour treatment (11.4 micrograms/mL) in 121 patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Review of clinical experience with amdinocillin monotherapy and comparative studies.

Activity against gram-negative bacilli and frequent synergism with other beta-lactam antibiotics were demonstrated by amdinocillin in urinary tract infections, urosepsis, and in a variety of other infections. In a prospective study, 299 patients were assigned at random to receive amdinocillin or another antibiotic considered standard treatment for the infection. The majority of infections were of the urinary tract, and 58 of 59 patients treated with amdinocillin responded clinically, with cures in 49. Of the 52 patients treated with tobramycin or other comparative agents, 49 responded, and 42 were cured. Escherichia coli and other Enterobacteriaceae were the usual pathogens. Immediately after treatment, 90 percent of urine samples were negative in both treatment groups. At four to six weeks follow-up, relapse or reinfection rates were about 20 percent in either group. Miscellaneous infections were treated with either amdinocillin or a comparative agent. Eleven of 15 infections responded favorably to amdinocillin, and seven were cured. Adverse effects were usually mild and characteristic of the penicillins.

Use of a written simulation to evaluate optometric clinical proficiency.

A written simulation of an optometric patient examination was designed and administered to 3rd-year optometric students. Test scores from the written simulation were compared with those from a clinical proficiency evaluation and no statistically significant correlations between the scores for the two testing methods was found. The results suggest that the two types of tests measure two different areas of clinical competence: problem-solving and case-analysis skills by the written simulation, and clinical performance skills by the clinical proficiency. This conclusion is supported by the subjective comments of students and faculty to whom the written simulation was administered and indicates that written simulations may be a useful tool in optometric curricula to teach and evaluate problem-solving and case-analysis skills.

Narrative strategies of aphasic and normal-speaking subjects.

A Picture Story Test for eliciting narrative speech was administered to five patients in each of the subgroups of Broca's and Wernicke's aphasic subjects and matched controls. While Wernicke's subjects and normal-speaking subjects did not differ significantly total output, the proportion of significant target lexemes was four times as great for normal-speaking subjects as for Wernicke's aphasic subjects. Broca's aphasic subjects, in spite of their telegraphic output, also had a smaller proportion of target lexemes than normal speakers. The proportion of nouns to verbs was elevated in the speech of Broca's asphasic subjects and depressed in the speech of Wernicke's aphasic subjects. Grammatical complexity was reduced in Wernicke's aphasic subjects, who used simple concatenation much more often than normal-speaking subjects. The Picture Story Test is suggeted as a clinically useful technique.
