Preoperative lateralisation of aldosteronomas by aldosterone/cortisol ratios in adrenal venous plasma.

Data from adrenal scintigraphy with 6 beta-[131I]-iodomethyl-19-norcholesterol (AS), computed tomography (CT), adrenal venography and adrenal venous sampling were compared for their accuracy in locating small aldosteronomas in 17 patients. Final confirmation of location was by surgery. Seventeen out of 18 adrenals whose aldosterone/cortisol (A/C) ratio was 5.0 x 10(-3) or higher were found to have aldosteronomas (94.4%). All 14 adrenals whose A/C ratios were less than 5.0 x 10(-3) contained no aldosteronomas (100%). AS successfully lateralised nine out of 17 aldosteronomas (52.9%), CT 3 out of 5 (60.0%), adrenal venography 12 out of 17 (70.6%), adrenal venous aldosterone concentration 12 out of 15 (80.0%). The most frequent problem with adrenal venous sampling was the varying degrees of mixture with non-adrenal venous blood, mainly from renal vein and inferior vena cava blood. The A/C ratio of adrenal venous plasma proved to be the most useful diagnostic tool, using cortisol concentration as an indicator of any non-adrenal venous mixture.

Serum concentration and effects of a single dose of enalapril maleate in patients with essential hypertension.

The antihypertensive effect of a non-sulfhydryl, long acting ACE (angiotensin converting enzyme) inhibitor, MK-421, was evaluated by administering a single dose of 10 mg to 13 patients with mild to moderate essential hypertension. The pharmacokinetic profile of MK-421 and its potent active metabolite, MK-422, was also assessed, together with the effect on the various components of the renin-angiotensin system. A single dose of MK-421 produced a significant fall in MBP from 2 to 24 hours post-drug. As could be expected, plasma ACE activity was suppressed up to 24 hours after MK-421. The half-life of MK-422, Cmax and [AUC]24(0) of MK-421 and MK-422 were measured. No significant change in plasma bradykinin or urinary excretion rate of kallikrein was observed, whereas a slight increase was observed in the urinary excretion rate of kinins after MK-421 in 8 patients. Significant correlations were observed between pretreatment PRA levels and the maximum fall in MBP.

Differences in the kinetic rate constants of normal and high molecular weight renin substrate from term pregnancy human plasma.

We have previously reported on the differences in physical and chemical characteristics between the high-molecular weight renin substrate (HMS greater than 150,000 daltons) and the normal substrate (NMS = 60,000). In this study, the kinetic constants were determined in both HMS and NMS which were prepared by gel exclusion chromatography from the plasma of pregnant women at term. Renin substrate (angiotensinogen) levels were expressed by radioimmunoassay of angiotensin I after incubation of samples with added semi-purified human kidney renin in the presence of angiotensinase inhibitors. The kinetic constants (Km and Vmax) were determined by the method of Line-weaver-Burk plots and also the method of Wilkinson. The Km for the HMS was 1.79 micrograms angiotensin 1 equivalents ( AIeq )/ml and the Vmax = 41.2 ngAIeq /ml/h, and the Km for the NMS was 3.52 micrograms AIeq /ml and the Vmax = 138 ng/ml/h. When adding small amounts of the HMS to the NMS, the production of angiotensin I was found to increase more than that in the NMS alone. It was also observed that the renin substrate reactivities of the plasma of pregnant women, which contained small amounts of the HMS, were higher than that found in the plasma of normotensive women not taking oral contraceptives. It is suggested that the existence of small amounts of the HMS may therefore contribute to the elevation in blood pressure under the influence of estrogens.

Chromosomal aberrations in normal human cells induced by the auger effect via Ca atoms.

PURPOSE: To quantify the Auger effect on chromosomal aberrations via Ca atoms in human cells. MATERIAL AND METHODS: Exponentially growing human normal fibroblasts (GM05389) were irradiated with 4.047 (CaK-P), 4.026 (CaK-L) and 4.067 (CaK-H) keV X-rays (corresponding to the resonance absorption edge of the Ca K-shell and slightly below and slightly above the edge, respectively) using synchrotron radiation at the photon factory (PF) of the High Energy Accelerator Organization located in Tsukuba. Chromosomal aberrations induced by the irradiation were analyzed by the premature chromosome condensation (PCC) method using calyculin A. The dependency of the chromosomal aberrations on the incubation time post 2 Gy irradiation was observed for each energy. Irradiation using 200 kVp conventional X-rays was also examined as a reference to CaK irradiation. RESULTS: (1) Soon after irradiation with 2Gy, the enhancement ratios of CaK-H X-rays to CaK-L X-rays were 1.21, 1.51 and 2.70 for breaks/gaps, isochromatid breaks and exchanges, respectively. The enhancement ratios of CaK-P X-rays to CaK-L X-rays were 1.82, 0.98 and 6.30, for breaks/gaps, isochromatid breaks and exchanges, respectively. (2) After a 6-hr incubation treatment post 2 Gy irradiation, the enhancement ratios of CaK-H X-rays to CaK-L X-rays were 1.59, 2.03 and 2.14 for breaks/gaps, isochromatid breaks and exchanges, respectively. The enhancement ratios of CaK-P X-rays to CaK-L X-rays were 1.69, 1.66 and 2.00 for breaks/gaps, isochromatid breaks and exchanges, respectively. (3) Soon after irradiation, the ratios of the efficiencies of CaK-P X-rays to those of 200 kVp X-rays were 1.74, 1.29 and 2.51 for breaks/gaps, isochromatid breaks and exchanges, respectively. And after a 6-hr incubation treatment, the ratios were 5.50, 1.93 and 1.81 for breaks/gaps, isochromatid breaks and exchanges, respectively. CONCLUSIONS: An effective enhancement of chromosomal aberrations, such as breaks/gaps, isochromatid breaks and exchanges, was caused by Ca K-shell ionization or excitation. Auger electrons emitted by Ca atoms in irradiated cells appear to have an important role in causing this enhancement. Comparing these efficiencies of chromosomal aberrations with those produced by 200 kVp conventional X-rays suggests un-repaired and complicated damage is induced by the X-rays around the Ca K-shell resonance absorption edge.

Significance of inguinal folds for diagnosis of congenital dislocation of the hip in infants aged three to four months.

We evaluated the usefulness of the appearance of asymmetrical inguinal folds in the frog leg position as a screening indicator for congenital dislocation of the hip (CDH) in the hope that we might be able to decrease the frequency of radiographic examinations of 3-4-month-old infants. Abnormal inguinal folds were observed in 499 (23.8%) of the 2,111 patients examined at the Asahikawa Health Center, and these 499 included all patients with complete dislocation or subluxation. Abnormal inguinal folds were also present in all 29 previously untreated patients with complete dislocation and in 11 with subluxation seen in our department. Because limited passive hip abduction is less sensitive than abnormal inguinal folds, we recommend inguinal fold assessment as a useful adjunct to other screening methods for CDH in 3-4-month-old infants.

Tangential view arthrogram at closed reduction in congenital dislocation of the hip.

Of 119 previously untreated hips with congenital dislocation (complete dislocation), a classification of tangential view arthrograms at closed reduction was attempted in 34 joints that could not be reduced with a Pavlik harness or by traction but could be reduced manually under anesthesia. Conservative or open reduction was selected according to this classification. None of the hips had total femoral head necrosis, and only two femoral heads showed mild permanent necrosis. These results suggest the usefulness of this classification for selection of treatment for congenital hip dislocation.

Reduced arterial baroreflex control of muscle sympathetic nerve activity in young borderline hypertensives.

Mean arterial pressure (MAP), heart interval (HI) and muscle sympathetic nerve activity (MSNA) were measured before and after a bolus injection of phenylephrine in 11 normotensives (NT) and 12 borderline hypertensives (BHT). (1) Basal MSNA was elevated in BHT compared to NT. (2) Both baroreflex slope for HI and that for MSNA were significantly less in BHT. (3) There was an inverse correlation between the slope for MSNA and the basal MSNA (r = -0.63). Thus, arterial baroreflex control of sympathetic activity may be reduced in BHT and the reduction may be involved in the elevated sympathetic nerve activity in these subjects.
