Evidence for cohort or generational differences in the drinking behavior of older adults.

Data from a survey of 928 older Bostonians show that there is a negative correlation between age and alcohol consumption among older adults: i.e., "old-old" people- those who are seventy-five years old or older-are more likely to abstain and less likely to drink in any quantity than the "younger-old"-those age sixty-seventy-five years old. Since the study collected only cross-sectional data, it is not clear whether there are developmental trends in alcohol consumption-that people drink less as they grow older-or whether there are cohort or generational effects. However, several retrospective data suggest that there are cohort or generational patterns of drinking behavior by older adults. Very old respondents, who now drink less than their younger counterparts, report consistently different patterns of life-long drinking habits; they report different parental drinking habits, and they have consistently different attitudes toward the effects of alcohol consumption upon health. The survey provides no insight into the nature of the historical factors which account for the differences in alcohol use. However, Prohibition, Repeal, and the Depression are likely to have played important roles.

Value of DNA analysis with multiple DNA probes for the detection of hemophilia A carriers.

Detection of hemophilia carriers is an important issue and should be addressed with great care. The allelic frequencies of three intragenic probes (Bcl I for probe p114.12, Xba I for probe p482.6, and Bgl I for probe C) and one linked probe (Bgl II for probe DX 13) are reported, together with their diagnostic yield singly and in combination. In this series, 725 individuals (405 females) in 156 families were analyzed for restriction fragment-length polymorphisms. A total of 255 females (63%) were found to be informative for their carrier state with one or more probes. The most informative intragenic probe was p482.6 (useful in 49% of informative females). The most informative probe was DX 13 (useful in 59% of informative females), but this is a linked probe that carries a 5% risk of cross-over. By the use of probes p114.12, p482.6, and DX 13, almost 98% of all the informative females could be detected. In about 71% of families with a family history and a known carrier, prenatal diagnosis was feasible.

Respiratory mechanics in upper airway obstruction.

Respiratory mechanics were studied in 7 patients with vocal cord paralysis (Group A), 7 with laryngeal or sublaryngeal obstruction (Group B) an in 3 with intrathoracic tracheal obstruction (Group C). Group A showed decrease in FIV1.0, PIFR and VI50 when compared with corresponding expiratory parameters. Group B only showed a decrease in PIFR compared with PEFR. In all patients the high flow area near TLC in expiratory V/V curves was replaced by a flat flow pattern, extending variably into lower lung volumes. Predominant inspiratory flow reduction was present in Group A. Usually the normal expiratory plateau (IVPF curves) was absent even at very low lung volumes (about 25% VC) in the 8 patients studied (Group A, B, C). In Group B, inspiratory and expiratory resistances (IVPF curves) were similar up to about 50-60 cm H2O Palv; above this level inspiratory resistance was higher. The flow-pressure pattern (IVPF curves) of a normal subject with a relatively low resistance added at the mouth (9 mm i.d.) was very close to that of the resistance alone throughout inspiration. Above 60 cm H2O Palv expiratory resistance appeared to be lower, as in Group B. Calculated pressure drop due to convective acceleration across the resistance coincides with its pressure-flow relationship. These results do not support the hypothesis of inspiratory reduction in tracheal caliber as a contributing cause to inspiratory flow decrease. The difference PEFR-PIFR at highest Palv was inversely related to the degree of obstruction. Consequently, flow-volume curves of moderate sublaryngeal lesions may show disproportionate reductions in VI (like Group A) despite its fixed nature. Results obtained in six patients submitted to surgical treatment are discussed.

Genetic divergence of Asiatic Bdellocephala (Turbellaria, Tricladida, Paludicola) as revealed by partial 18S rRNA gene sequence comparisons.

Polymerase chain reaction (PCR) and direct sequencing of small ribosomal RNA genes were used for analysis of genetic differences among Asiatic species of freshwater triclad genus Bdellocephala. Representatives of four species and four subspecies of this genus were used to establish homology between nucleotides in the 5'-end portion of small ribosomal RNA gene sequences. Within 552 nucleotide sites of aligned sequences compared, six variable base positions were discovered, dividing Bdellocephala into five different genotypes. Sequence data allow to distinguish two groups of these genotypes. One of them unites species from Kamchatka and Japan, another one unites Baikalian taxa. Agreement between available morphological, cytological and sequence data is discussed.
