Fibrolipoma of the cheek in a child.

Fibrolipomas in the oral cavity are infrequent neoplasms. Their occurrence in children is even more unusual. A case of this tumor involving the check in a child is reported, and the histogenesis discussed. These lesions represent a variant of lipoma and derive their name from their relative amounts of adipose and fibrous tissue. Knowledge of fatty tumors in this region is important to all surgeons who operate in and about the oral cavity.

Social and psychologic correlates of problem drinking in old age.

A study of the drinking behavior of 928 noninstitutional residents of Boston, aged 60 or older, showed that 53 per cent drinks; and 6 per cent, two or more drinks. Nine subjects (1 per cent) were self-reported problem drinkers, and another 31 (3 per cent) reported that drinking had diminished the quality of their lives. Problem drinkers and those with alcohol-related problems were likely to be young-old native-born men. They were less likely to be satisfied with their relationships with family members, spouses, and close friends. All problem drinkers had long-term drinking problems. Most had sought treatment, but only one was being treated at the time of the study. The data suggest that current theories do not adequately explain late-life problem drinking and that current initiatives do not adequately address the needs of the elderly population at large.

Differential utilization of leucyl-tRNAs by Escherichia coli.

The utilization of the isoaccepting species of leucyl-tRNA in protein synthesis has been examined in E. coli. Two minor leucyl-tRNA species, isoacceptors tRNA3Leu and tRNA4Leu, are the predominant species found bound to ribosomes during exponential growth in minimal medium. In rich medium, an increased proportion of tRNA1Leu is found bound to ribosomes. One species, tRNA5Leu, is always absent from ribosomes. In a mutant strain in which normal tRNA3Leu and tRNA5Leu are reduced or absent, tRNA1Leu is a major ribosome-bound species in minimal medium. Protein synthesis in vitro with RNA from E. coli further suggests that tRNA5Leu is rarely used for total protein synthesis; however, this species is active with RNA from MS2 phage. We propose that tRNA1Leu can substitute for tRNA3Leu under rapid growth conditions, and that tRNA5Leu is used minimally in total protein synthesis.

Environmental air and airborne infections.

The results of a study on the epidemiology of airborne (aerobic) surgical infections are presented. The first phase of the study was carried out in a surgical suite which contained no environmental or traffic control systems. The second phase of the study took place within a modern "up to date" operating room suite containing multiple air screens as well as an elaborate ventilation system utilizing HEPA type filters which provided the operating room with clinically sterile air. One hundred and fifty-six patients were also studied. All patients underwent major procedures. The ratio of clean, clean-contaminated, and dirty cases was the same in both groups. Preoperatively, a nasal swab, clean voided urine (or vaginal swab) and a rectal swab were obtained on each patient. Daily nasal cultures and cultures of suspected sites of infection were obtained postoperatively. Daily nasal cultures and "glove sweat" cultures were obtained on all personnel attending the patient. Environmental cultures of the operating room, the operating room hallway, recovery room and patients' rooms were also taken. All samples were checked for the presence of staphylococci, streptococci, Escherichia coli, proteus species, enterobacter, klebsiella, and pseudomonas. In all, 15,000 cultures were taken during the study. The rate of infection was essentially the same in both phases of the study. Environmental air only occasionaly served as the source of infecting organisms. The results of the study support the conclusion that the most common source of infecting organisms in surgical infections is thepatient or those around him. The most common time of contamination is during the surgical procedure itself. Surgical infections can best be minimized by meticulous observation of fundamental principles of antisepsis rather than by dependence on elaborate and costly ventilation and air control systems.

Impact of the "Rand report" on alcoholics, treatment personnel and Boston residents.

Interviews with patients and staff at six alcoholism treatment centers, respondents in a random telephone sample, and a sample of homeless men considereded to need alcoholism treatment revealed little change in treatment program goals, no increase in patients seeking treatment, and few changes in drinking attributable to the "Rand report."
