[Digital subtraction dacryocystography and sialography].

Fourteen examinations of the tear ducts and 21 sialograms were performed using digital subtraction and conventional techniques. In nine of the former and 15 of the latter, digital subtraction produced better results, in the remainder conventional technique was equally good. The advantages of digital subtraction are due to the removal of superimposed structures of the skull. Dose measurements at the level of the lens of the eye showed a greatly reduced radiation dose when using digital subtraction, as compared with conventional dacryocystography. Digital subtraction techniques with pulsed radiation and the ability to perform pixel shift is an optimal method for performing radiological examinations of the tear ducts and salivary glands.

[Digital subtraction dacryocystography].

A comparison of 17 radiological examinations of the lacrimal pathways performed on twelve patients using digital subtraction and conventional film technique showed that digital subtraction dacryocystography yields images of superior quality. Digital subtraction makes enhanced imaging of the opacified tear duct as well as elimination of super-imposing skeletal structures possible, which with conventional techniques frequently impair diagnostic accuracy. In measurements of the radiation dose sustained by the crystalline lens, the radiation level with digital subtraction was found to be considerably lower than with the conventional method.

[Digital subtraction sialography].

In 22 patients we performed 33 sialographies of salivary glands using digital subtraction technique as well as conventional technique (100 mm camera). In the majority of cases the digital subtraction technique was superior to the conventional technique regarding diagnostic image quality. Additionally, digital subtraction technique generally needs a lower number of contrast media application. Hence we recommend digital subtraction technique for radiological imaging of salivary glands and ducts.

[Arterial bolus dynamics of ionic and nonionic contrast media in intravenous administration].

A more favourable intraarterial pattern of bolus dynamics can be expected in DSA on applying nonionic contrast media via the intravenous route, as can be concluded from the results of numerous experimental studies on the different effects of ionic and nonionic contrast media on cardiovascular function. However, bolus measurements of ionic and nonionic contrast media in intraindividual comparison of 28 patients via serio-CT did not yield any significant difference. The influences of various factors on measurement results--quantities of contrast media, time and method of measurement--are discussed as possible causes of this discrepancy.

[Initial experiences with PTA of hemodialysis shunts].

Our experience of using PTA for stenoses of haemodialysis fistulae in 11 cases is described. PTA is the procedure of choice for re-establishing adequate shunt function. Intraluminal pressure measurements on the venous limb of well functioning Cimino shunts produced values of 7.5 to 44.5 mmHg, average 24.5 mmHg. These values are the basis for judging the success of a dilatation. Shunt angiography with DSA with a highly diluted non-ionic contrast medium injected via a remaining dialysis needle is an effective procedure with minimal discomfort to the patient.

[Pass-through technic for translumbar catheter angiography].

A new introducer has been developed for translumbar catheter angiography using a 4-F catheter. This combines the advantages of catheter angiography and translumbar aortic puncture. CT examination following the angiogram showed retroperitoneal haematomas of about the same size as result from conventional translumbar aortography.

Cardiac gating in intravenous DSA.

ECG-gating of a pulsed radiation exposure during end-diastole using digital subtraction angiography shows significantly better results in the examination of the aortic arch vessels and pulmonary arteries and may be expected to decrease the rate of false positive diagnosis of stenosis when examining renal arteries. Different types of misregistration artifacts due to a smear effect by integrating several television frames using non-gated DSA are described and schematically explained. Furthermore, a short description of the difficulties which may be encountered with ecg-gated DSA--mainly rapid changes of heart frequency--is given.

Utility of DSA in the evaluation of haemodialysis access fistulas.

Results of 39 intravenous and intraarterial DSA studies of haemodialysis access fistula are presented. Advantages of DSA versus conventional fistulography and xeroangiography are discussed. Digital subtraction angiography represents an accurate, time-saving and low complication approach to the evaluation of failing dialysis access fistulas.

[Comparison of Doppler ultrasound, intravenous DSA and conventional film angiography in diagnosing stenosing changes in the area of the carotid bifurcation].

The findings on Doppler ultrasound, intravenous DSA and conventional film angiography were compared in 66 examinations of the carotids with regard to their specificity, sensitivity and accuracy. Comparisons with the literature show that intravenous DSA equals Doppler ultrasound only if good or excellent results of the latter are considered. If poorer DSA results are included, diagnostic precision of this method is 5 to 10% less than that of Doppler ultrasound. Mural ulceration can be diagnosed by means of intravenous DSA in only about 50% of cases, whereas Doppler ultrasound is unable to diagnose this. Improved image quality of intravenous DSA, with better diagnostic precision, could be achieved by using ECG gating.

[Intravenous DSA following kidney transplantation].

Intravenous DSA is a simple angiographic technique in the follow-up of patients with renal transplants. It is of value in the post-operative anuric phase, for investigating renal function which is deteriorating and for demonstrating, or excluding, stenosis of the renal artery at the anastomosis during the development of hypertension. Changes in the peripheral arteries are not usually demonstrable. Reduced opacification of the parenchyma is observed during rejection and if there has been damage to the transplant, but is not seen if the transplant is functioning normally.

[Computerized tomography of the elbow joint].

An essential pre-condition for the interpretation of computer tomographs of the elbow joint is a knowledge of the normal anatomy. The complexity of the joint requires a definite protocol for the examination, since only in this way is it possible to distinguish all anatomical structures and pathological processes. A clinical example is used to show that CT of the elbow joint is superior to all other radiographic methods for demonstrating osteochondritis dissecans. It is therefore an important additional method for demonstrating the elbow joint.

[ECG-directed DSA of the pulmonary arteries--technic and initial clinical experiences].

ECG-triggered DSA of the pulmonary arteries enables far-reaching avoidance of image artifacts which are due to pulsation. However, trigger delay and image duration must be adapted subtly to the individual heart rate. Whereas adaptation of trigger delay can be effected via programming of the new generation of equipment, further improvements will be necessary to achieve an adequate image life or duration of the pulsed irradiation. Experience collected so far has shown that ECG-DSA of the pulmonary arteries yields results that are as valid as those obtained via perfusion scintigraphy, so that this method can be used in initial examination to establish the diagnosis of pulmonary embolism. The use of ECG-DSA is not indicated in absolute cardiac arrhythmia.
