Rapid increase in LHRH mRNA levels following NMDA.

To test the hypothesis that changes in LHRH mRNA levels are tightly linked to changes in LHRH secretion, intact male rats were infused with NMDA, a substance which increases LH release by a hypothalamic action. NMDA significantly elevated cellular levels of LHRH mRNA at 15 and 60 min. Similar changes in LH levels were induced by NMDA. These effects of NMDA on LHRH mRNA levels were not due to a generalized excitatory effect in the brain because levels of mRNAs encoding proopiomelanocortin (POMC) and tyrosine hydroxylase (TH) in the arcuate-periarcuate region were not altered by NMDA and levels of TH mRNA were decreased significantly at 15 and 60 min in neurons of the zona incerta. These data strongly support the hypothesis that changes in cellular levels of LHRH mRNA are tightly linked temporally to changes in LHRH secretion.

Management of tibial fractures with the orthofix dynamic external fixator.

We report our experience with the use of the Orthofix external fixator for the management of closed and open tibial fractures. One hundred fractures were treated in 96 patients between May 1985 and December 1989. There were 47 closed and 53 open fractures. Of the open fractures, 11 were grade I, 16 were grade II and 26 were grade III. Forty-five closed fractures and 49 open fractures went on to solid bony union in an average of 15.2 and 20.5 weeks, respectively. Non-union requiring operative intervention occurred in two closed and four open fractures (6%). The other main complications were pin track infection (30% of cases) and malunion which occurred in 14% of closed fractures and 32% of open fractures. We concluded that dynamic axial fixation is a useful method of treatment for open and difficult closed tibial fractures. Our experience indicates that malunion and pin track infection remain common problems and have still to be overcome.

UB-82 forms offer wealth of information, misinformation.

UB-82s, those ubiquitous green forms hospitals rely on for proper reimbursement, also are being used as a research tool, a purpose for which they weren't designed. While the data have some valid applications, flaws such as inconsistent coding, distorted data and omitted information make their use unreliable for broad studies in such critical areas as quality of care and utilization.
