[Congenital duodenal obstruction, Malrotation of the intestine associated with annular pancreas (author's transl)].

A newborn presenting duodenal stenosis by small bowel malrotation and annular pancreas is reported. Major diagnostic problems in duodenal obstruction of newborn are considered in keeping with literature review and personal experience. In this presentation clinical appearance has been long aspecific (apnea and bradycardia spells) in spite of a rather typical radiological picture. Really, the true diagnostic nature of midgut obstruction is often intraoperative. Face to any duodenal obstruction in a newborn the possibility of many malformations of the gut should be raised and consequently looked for accurately by the surgeon for a good postoperative prognosis. The surgical procedure of choice is frequently a duodeno-jejunostomy or a duodeno-duodenostomy; the annular pancreas should, on the contrary, never be sectioned.

Cyclic neutropenia and severe hypogammaglobulinemia in a patient with excess of CD8-positive T lymphocytes: response to G-CSF therapy.

Cyclic neutropenia is a rare hematological disorder characterized by periodical severe granulocytopenia. A stem cell defect and/or immunological abnormalities are considered to play a role in this disease. Here we describe the case of an adult woman who was diagnosed as having both cyclic neutropenia and severe hypogammaglobulinemia. Her clinical history revealed that one or both abnormalities had been present since childhood. Normal in vitro growth of the patient's bone marrow CFU-GM was observed, while immunological analysis revealed the presence of a persistent excess of activated (HLA-DR+) CD8+ T lymphocytes in both bone marrow and peripheral blood. These T lymphocytes have been shown to be polyclonal by DNA analysis, and their role in determining the clinical picture of our patient remain uncertain since they could not be shown to produce inhibitors of in vitro CFU-GM growth. Intermittent low doses of human recombinant G-CSF were able to improve neutropenia and completely prevent infectious symptoms, thus confirming the efficacy of this cytokine in cyclic neutropenia patients.

[The incidence of Raynaud's phenomenon in 1920 residents in Milan].

We use the term Raynaud Phenomenon (RP) to define episodic digital ischemia with colour changes involving the digits of the hands and feet, induced by cold or emotional stresses. Surprisingly little information is available concerning the incidence of RP in the general population. In Italy, studies of this purpose have never been carried out and the few investigations, we find in literature, are discording, showing an incidence between 0.83% and 27%. The interest in knowing the incidence of RP is, beside the usual reason which get to an epidemiological study, RP indeed may be associated with severe and disabling clinical conditions as connective tissue diseases. The RP may be the initial complaint on an otherwise silent systemic illness and may even influence the course of these clinical conditions, assuming the role of prognostic index. It seemed useful for us to investigate on a model of population resident in our city the incidence of RP and the frequency of the idiopathic and secondaries forms. We have prepared a set of questions which was distributed to some students attending the IV year of the School of Medicine and to specialized doctors. The students randomly interviewed as many people as they could, while the doctors had to fill the questionnaire for companions of patients coming to three different Sanitary Units in Milan. All those subjects that at first instance were diagnosed as RP were sent to our ambulatory where more experienced doctors provided to collect a precise history.(ABSTRACT TRUNCATED AT 250 WORDS)
