[Prevalence of diabetes mellitus and glucose intolerance in adult Ethiopian immigrants].

339 adult immigrants from Ethiopia were surveyed 1 year after arrival. They were screened for diabetes mellitus and impaired glucose tolerance by measurement of serum HgbA1C levels. The levels were higher than normal in 35 subjects (10.3%), and were significantly higher in immigrants consuming the Israeli diet, which is richer in proteins and carbohydrates, than in those on the Ethiopian diet. Oral glucose load was abnormal in 7 subjects. 3 had overt diabetes mellitus with absent serum insulin response to glucose load. 4 had glucose intolerance with normal insulin secretion. However, clinical appearance of diabetes was incidental and diabetes ketoacidosis was not detected. The overall prevalence of diabetes mellitus and glucose intolerance in this ethnic group (2.1%) is similar to that in the general population of Israel. Longitudinal studies would indicate the exact prevalence and type of diabetes mellitus among the Ethiopian immigrants.

[Thyroid dysfunction and goiter among immigrants from Ethiopia].

1131 immigrants from Gonder, Ethiopia were surveyed a year after arrival in Israel. The overall prevalence of goiter was 46.1%; the lowest rates (6.7%) were in infants (aged 1-2 years) and the peak prevalence in males was 56.7% (in those aged 12-14 years) and in females, 77.7% (in those aged 35-40). A significant decrease in goiter prevalence was observed after the age of 60 for both men (7.2%) and women (35%). The prevalence of hypothyroidism was 1.1%, 2% in children and 0.2% in adults. That of hyperthyroidism was 1.7%, 0.8% in children and 2.4% in adults. The high prevalence of goiter and hyperthyroidism with low prevalence of hypothyroidism probably resulted from the combined effects of food goitrogens and iodine deficiency in Ethiopia, with the latter playing only a minor role. Neither factor was in effect after arrival in Israel. Genetic and hormonal factors may contribute to the low prevalence of both goiter and hypothyroidism in the adult males. In view of the high prevalence of hyperthyroidism, iodine enrichment is not recommended for Ethiopian immigrants.

Recurrent nephrotic syndrome associated with Kimura's disease in a young non-Oriental male.

Kimura's disease (KD) is an angiolymphoid proliferative disorder of soft tissue with eosinophilia, with a predilection for head and neck regions in young Oriental men. Renal disease is rarely associated with it. We describe a young non-Oriental male with KD and relapsing steroid-responsive minimal-change nephrotic syndrome. This case of KD associated nephrotic syndrome is unique in the relapsing nature of the nephropathy and the non-Oriental origin of the patient. We comment on KD and nephrotic syndrome and on treatment strategy.

Bilateral testicular adrenal rests in a patient with 11-hydroxylase deficient congenital adrenal hyperplasia.

A 13-year-old boy was referred for investigation of hypertension, precocious puberty and giant testicles. Hormonal studies established the diagnosis of congenital adrenal hyperplasia due to 11-hydroxylase deficiency. Testicular biopsy revealed that the scrotal masses were adrenal rest tumors entirely composed of adrenocortical tissue. Primary testicular tissue was absent. The size of the tumors regressed following replacement steroid therapy together with restoration of normal blood pressure. This observation of a testicular adrenal rest tumor in a patient with 11-hydroxylase deficient congenital adrenal hyperplasia is unique.

Seroprevalence of antibodies against Toxoplasma gondii among two rural populations in northern Israel.

The prevalence of antibodies against Toxoplasma gondii was measured in two rural populations in northern Israel--Jewish kibbutz members and Arab villagers. The respective prevalences in these two populations were 22.2% and 55.8% (P < 0.001). No correlation was found between the presence of antibodies and sex, occupation, contact with cats, a history of fever and/or lymphadenopathy, eye disease, abortions or delivery of children with congenital malformations. In contrast to Jewish children who were not found to have antibodies in the first decade of life, 20.5% of Arab children tested positive. A gradual increase in the prevalence of antibodies with age was seen in both groups, with the Jews reaching a prevalence of 42.6% at age 60+ and the Arabs reaching 74% at age 40. The difference between the two groups probably stems from different eating habits, namely ingestion of raw meat and unpasteurized milk and milk products.

[Pyogenic liver abscess caused by Streptococcus milleri].

Streptococcus milleri is a facultative anaerobic pathogen of the viridans group associated with serious suppurative infections, mainly abscesses. We saw 3 cases of liver abscess due to this organism in 1990 in women aged 43, 59 and 67 years, respectively. The coccus was grown from blood cultures in 2 of the cases and from abscess drainage in the 3rd. The importance of prompt diagnosis of liver abscess by means of CT and ultrasonography is stressed. Conservative treatment consists of CT-guided percutaneous drainage, followed by prolonged antibiotic treatment. Such conservative measures avoid complex and sometimes hazardous surgical procedures.

[Antibodies against double-stranded and single-stranded DNA in the aged].

Sera from 183 healthy, elderly subjects and 92 young, control subjects were tested by the Elisa method for antibodies against double-stranded (ds) and single-stranded (ss) DNA, and sera from the elderly were tested for antinuclear antibodies as well. Significantly higher levels of anti-ss-DNA were found in the elderly, but levels of anti-ds-DNA did not differ significantly; no sera positive for anti-ds-DNA were found. levels of antibodies to DNA were similar in men and women. In those 85 and over, mean levels of anti-ds-DNA were lower than in those 65-84 (63 vs. 44, p less than 0.05). 13 of those over 65 (7.1%) tested positive for antinuclear factor, 2 of whom had positive titers of anti-ss-DNA. The results of this study support the hypothesis that elevated levels of anti-ss-DNA are age-related and not disease-related.

Clindamycin in the treatment of an outbreak of streptococcal pharyngitis in a kibbutz due to beta-lactamase producing organisms.

During the winter of 1987-88, an epidemic of streptococcal pharyngitis (SP) affected almost 25% of the 800 inhabitants of a community settlement (kibbutz), and failed to respond to treatment with penicillin and/or erythromycin, despite the fact that the streptococci were highly sensitive to penicillin (minimum inhibitory concentration: MIC less than 0.001 micrograms/ml). We suggested that the concomitant presence of beta-lactamase producing organisms in the pharyngeal cavities of the affected individuals with SP could inactivate the antibiotics, rendering the streptococci resistant to penicillin. Throat swabs were taken and tested for aerobic and anaerobic beta-lactamase producing organisms and for beta-lactamase activity and clindamycin treatment was given to 110 patients with SP. Beta-lactamase activity was detected in 90 cases and the cultures were positive for Staphylococcus aureus (25 cases), Haemophilus sp. (22 cases) and anaerobes, which we did not identify in 68 cases. Ninety-eight patients completed the ten-day course of treatment with clindamycin and 96 were cured clinically and bacteriologically. The epidemic was controlled.

[Tuberculous meningitis].

2 cases of tuberculous meningitis are presented, 1 in a 54-year-old woman who had immigrated to Israel from Turkey 36 years before, and the other in a 10-year-old girl who recently immigrated from Ethiopia. Since diagnosis is difficult and prompt antituberculous treatment is life-saving, treatment may be instituted before the bacteriological diagnosis is established. A short term treatment of 9 months and a combination of rifampicin and isoniazid were successful in our patients.

Determination by ELISA of anti-DNA antibodies in patients with familial Mediterranean fever.

Positive titers of antibodies against double-stranded (ds) and single-stranded (ss) DNA were found in the sera of 4 and 6 patients, respectively, of 18 who had familial Mediterranean fever (FMF). While anti-dsDNA antibodies were found only in patients with active disease, there was no correlation between the presence of anti-ssDNA antibodies and disease activity. The antibody titers were lower than those found in patients with active systemic lupus erythematosus. This may be due in part to the fact that all the FMF patients were treated with colchicine.
