Excitatory pattern of gamma-interferon on natural killer cell activity in senile dementia of the Alzheimer type.

Spontaneous natural killer (NK) cell activity and NK-induced cytotoxicity after interferon-gamma (IFN-gamma) were measured in healthy elderly subjects and in patients with senile dementia of Alzheimer type (SDAT) and multi-infarct dementia (MID). Normal basal and IFN-gamma-stimulated NK cytotoxicity were found in healthy old subjects and in patients with MID. On the contrary higher NK cytotoxicity after IFN-gamma (650 IU) was demonstrated in SDAT patients than in MID and healthy subjects (p < 0.001). A significant inverse correlation between the percent increase of NK cytotoxicity after IFN-gamma and the Mini Mental State Examination score (p < 0.001) was also demonstrated in patients with SDAT. Our data might suggest a cytokine-dependent mechanism of NK activation in SDAT associated with the neuroimmune hypothesis of the disease.

Enhanced cytotoxic response of natural killer cells to interleukin-2 in Alzheimer's disease.

Experimental data suggest an involvement of immune cellular components in the development of Alzheimer's disease (AD). Against this background, the spontaneous natural killer (NK) cell activity and the NK-induced cytotoxicity after interleukin-2 (IL-2) were studied in healthy elderly subjects and in patients with dementia of Alzheimer type (SDAT) and multi-infarct type (MID). Higher NK cytotoxicity (expressed as total lysis and percent increase) at different IL-2 concentrations (50 and 100 IU/ml/cells) was demonstrated in patients with SDAT than in healthy elderly subjects (p < 0.001) and MID patients (p < 0.001). NK cell activity of MID patients was similar to that of healthy elderly and healthy young subjects. A negative correlation between the percent increase in NK cytotoxicity after IL-2 and the Mini Mental State Examination Score was also found in SDAT patients (p < 0.01). Alterations of IL-2-mediated NK cytotoxicity may therefore support the neuroimmune hypothesis of AD.

Ab-interno trabeculo-canalectomy: surgical approach and histological examination.

PURPOSE: To evaluate, on eye bank eyes, a new surgical approach aimed at removing a quadrant of the trabecular meshwork (TM), with an ab interno approach. METHODS: Gonioscopically controlled ab interno removal of the TM was done with a subretinal forcep on six human bank eyes. Serial histological sections were obtained from the treated and untreated part of each globe to assess the effect of the technique on intraocular tissues. RESULTS: Under the gonioscope, the TM was easily removed in strings of varying length. Histological examination showed, unexpectedly, that this resulted in a well-defined deep furrow in the middle of the trabecular region involving both the TM and the inner wall of Schlemm's canal. The operation created a direct communication between the anterior chamber and Schlemm's canal lumen without any evident damage to the outer canal wall and adjacent ocular structures such as the iris base and corneal endothelium. CONCLUSIONS: Our small series on human bank eyes showed that the procedure involves both the TM and the inner wall of Schlemm's canal and is therefore called ab interno trabeculocanalectomy (AITC). The intraoperative findings and the histological evidence are encouraging, and suggest that the procecedure could have potential clinical application.

Efficacy of collagen shields after extracapsular cataract extraction.

The efficacy of presoaked porcine collagen shields was compared with subconjunctivally injected corticosteroids and antibiotics in reducing ocular inflammation after ECCE with IOL implantation, and signs of reduced oxygen delivery to the cornea were assessed. Fifty eyes of fifty patients were recruited. At the end of surgery 25 eyes (group 1) received a 24-h shield presoaked with 0.1% betamethasone disodium phosphate and 0.5% netilmycin. The other 25 eyes (group 2) received 1 mg betamethasone disodium phosphate and 250 mg piperacillin as a subconjunctival injection. The eyes were then taped closed. Inflammatory parameters (anterior chamber flare, fibrin, folds of Descemet's membrane, corneal edema) and tolerability of the delivery system (pain, itching) were evaluated 24 h after surgery. None of the patients lost the shield and the collagen devices were well tolerated. Groups 1 and 2 showed no significant differences in the parameters considered. There was, however, transient, slight diffuse superficial corneal edema in 24% of group 1 and 4% of group 2, raising the question of inadequate oxygen transmission through collagen shields under closed lids. The results suggest that collagen shields used with the right solution have no significant adverse effect and may be a less invasive alternative to subconjunctival injection.

Retinal pigment epithelium tear following toxemia of pregnancy.

A 28-year-old woman with toxemia during the 34th week of pregnancy complained of visual loss in one eye. Two weeks after abruptio placentae and delivery a retinal pigment epithelium (RPE) tear which presumably followed a RPE detachment (PED) was found in the macular region of the right eye. Visual acuity was 0.8 at the time of diagnosis but was 1.0 in both eyes one year after the delivery. Some authors have described peculiar choroidal lesions associated with central serous chorioretinopathy (CSC) in pregnancy and ischemic exudative choroidal lesions during severe toxemia. However, the findings we describe are highly unusual, indicating that the mechanisms of choroidal and RPE pathology are still poorly understood. We also demonstrate that a pathological tear following toxemia of pregnancy is compatible with good acuity in spite of the anatomical damage.

Academic education's contribution to the nurse-patient relationship.

AIM: To explore how nursing students perceive the role of academic education in enabling them to understand factors that can influence the relationship between nurses and patients. METHOD: An exploratory qualitative design, with data collected through unstructured interviews with four pre-registration students enrolled on a degree course in adult nursing. FINDINGS: Participants emphasised the role of reflection in making academic education relevant to practice and in promoting understanding of the relationships they developed with patients. They also valued experiential learning methods used in seminars, for example, case studies, role play and group discussion. CONCLUSION: Academic education plays an important role in fostering understanding of the nurse-patient relationship by promoting a learning approach based on reflection and self-awareness.

[Idiopathic thrombocytopenic purpura and food allergy: a study on possible correlations].

UNLABELLED: Eight children affected by idiopathic chronic thrombocytopenia were studied for history of food allergy, anti-platelet antibodies, total IgE, prick test for food antigens, AGA and EMA. They were free of treatment for thrombocytopenia since at least six months before. Three children had in the first year of life intolerance to cow's milk proteins with atopic dermatitis and/or poor growth. None had AGA or EMA positivity. 6/8 had positivity for anti-platelet antibodies. A 15-days oligoantigenic diet was instituted in all of them. Platelet count was unaffected in all but one, in which a sharp increase was noted (from 19x 10(9)/l. to 150x 10(9)/l.). He was a 12-year-old boy with a previous history of cow's milk intolerance and actual skin prick test positivity for casein and lactoglobulin. A one-year follow up for this case was instituted and he was put on two further 15-days periods of oligoantigenic diet, but no change in platelet count was noted any more. CONCLUSION: we were not able to correlate chronic thrombocytopenia to food allergy.

Carboplatin, cisplatin and paclitaxel in the treatment of patients with epithelial ovarian cancer.

BACKGROUND: Prognosis of advanced ovarian cancer is unsatisfactory. Chemotherapy can be intensified combining active drugs at their highest possible doses. PATIENTS AND METHODS: In this phase I/II trial, 77 untreated patients received escalating doses of paclitaxel (135, 155, 175, 195 and 215 mg/m2, infused over 3 hours) with carboplatin (AUC 3.6) and cisplatin (60 mg/m2). Nine, 16, 13, 8 and 3 patients were treated at the five levels, respectively. A further 28 patients were treated at the maximum tolerable dose (MTD). RESULTS: Dose-limiting toxicities (one WHO grade 3 constipation, one grade 2 prolonged peripheral neurotoxicity and one grade 3 cardiac toxicity) occurred at 215 mg/m2 in 3 out of 3 patients. MTD was reached at level 4 paclitaxel dose (195 mg/m2). Response was evaluated in 62 patients. A complete response was achieved in 23 patients (37.1%-95% CI 25.2-50.3), including 16 (25.8%) pathological and partial response in 28 (45.2%), for an overall response rate of 82.3% (95% exact CL: 70.5%-90.8%). The probability of response was affected by the degree of initial debulking (p = 0.002) and not by the paclitaxel dose. In patients with stage III-IV disease, median progression-free survival was 17 months (95% CI 14-25). After a median follow-up of 28 months, median survival had not been reached; 2-year estimated survival was 67%. CONCLUSION: Paclitaxel can be safely given at the dose of 195 mg/m2 in combination with carboplatin (AUC 3.6) and cisplatin (60 mg/m2). This combination is active and safe and could be considered in clinical settings requiring intensive short treatment.

[Dopaminergic neurons and clinical neuropsychiatry].

The neurobiologic acquisitions in the realm of the dopaminergic neurons are analyzed and discussed in connection with related clinical problems. Both "neurological" (extrapyramidal; autonomic; neuro endocrine) and "psychiatric" (bradifrenia; dissociation; depression of mood) symptoms and syndromes--whose pathophysiology and pharmacotherapy involves dopaminergic transmission--are discussed. The peculiar physiological meaning of dopaminergic neurons between the monoaminergic adaptive systems is stressed.

Intravenous administration of lisuride in the treatment of neuroleptic malignant syndrome.

The neuroleptic malignant syndrome (NMS) is a very rare but life-threatening complication of neuroleptic treatment. The mortality of NMS has been estimated at 8-30% and the most common cause of death is respiratory failure. Signs and symptoms of NMS are attributed to impairment of dopaminergic neurotransmission in the central nervous system. We describe two cases of NMS successfully treated with intravenous lisuride in combination with oral L-Dopa.

"Organic" and "functional" diseases of the nervous system.

The aim of this paper is to discuss whether in the realm of diseases of the nervous system the concept of "dysfunctional" versus "organic" disorders is still useful. The knowledge on the Common Brain Stem System (CBSS) described by Hess is reviewed in order to underline its role as a control centre integrating all the homeostatic and adaptive nervous activities, and in this context, the nervous control of the cardiovascular system is examined, particularly in relation to higher nervous activities. The neurogenic syncopes are chosen as examples of the either "organic" or "dysfunctional" conditions whose semeiotic and pathophysiological distinctive features are analysed. In conclusion the distinction between "organic" diseases and "dysfunctional" disorders seems to be still justified, mainly for the nervous affections involving the homeostatic-adaptive properties of the CBSS.

[Changes in the blood rheology and an audiometric deficit in a group of patients exposed to occupational noise pollution].

Several hemorheologic and plasma proteic features were analyzed in workers exposed to acoustic defilement. Whole-blood viscosity was significantly increased (at every shear-rate considered) in patients with severe sensorineural hearing impairment; whereas plasma, serum and relative viscosity remained unchanged. These variations were independent of erythrocyte deformability and aggregation, and of plasma fibrinogen levels, resulted indeed in the normal range. On the contrary plasma fibronectin was significantly increased in patients with sensorineural hearing impairment, suggesting a microvascular endothelial damage in these subjects. The exposition to acoustic defilement during work activity may be considered as aetiological factor for the development and progression of sensorineural hearing impairment, and more extensively for the occurrence of cardiovascular complications.

[Effects of a vagolytic substance on the circadian rhythm of the ACTH-secreting system in man].

The circadian rhythm of plasma cortisol and urinary 17-hydroxy-corticosteroids has been studied in 6 normal volunteers both in basal conditions and after the administration of a single dose of 30 mg of a banthine derivative (the beta methyl-beta-isopropylaminoethyl ester bromide of xantene-9-carbonic acid, "Pervagal") given orally once a day at different hours (midnight, 4 AM, 8 AM, noon, 4 PM, 8 PM). The vagolytic drug inhibits the cortisol secretion only when administered at 4 PM, 8 PM or midnight, whereas it is ineffective when given at different hours. If don't exist circadian variations of the bio-availability of the drug employed, our results suggest that also in human beings as well as in experimental animals, the cholinergic mechanisms are effective, over all if not exclusively, in starting the circadian activation of the hypothalamo-pituitary-adrenal system.
