[Two unedited letters from Baron Louis Suetin: their significance in the biography of a surgeon (1816-1817)].

The author reveals and comments two letters written by the surgeon Louis Seutin and discovered in 1994. They were sent in February 181 6 and July 1817 to lieutenant Ferdinand Doignon, a native from Tournai. Circumstances which created Seutin's friendship for lieutenant Doignon in 1813, during the campaign of Saxony of the Great Army, are described. These two letters are different by their tone. Consideration is given to the periods and places in which they were written. Their significance in Seutin's biography is enlightened through explanatory comments.

Major General Harry G. Armstrong: memories of a friendship.

Major General Harry G. Armstrong was a well known personality in aviation medicine. Most people, especially the younger generation, recall only his public image as a scientist wholly involved in his research and as a man of great authority. Dr. Evrard recounts some of his memories of this great man in an effort to bring the true personality of Major General Harry G. Armstrong to light.

[Herpes encephalitis treated using adenine-arabino-side and followed-up by densitometry (author's transl)].

In a 52-year-old man suffering from encephalitis, immunofluorescent study of cerebral tissue led to a diagnosis of herpes encephalitis 4 hours after biopsy. Cell cultures on the fourth day confirmed the diagnosis. Such a hypothesis had already been advanced before biopsy on the basis of the electroencephalogram which showed electrical changes with a periodic tendency. Following treatment with adenine-arabinoside, motor deficit and aphasia disappeared in one month whilst there was a parallel increase in antibody levels in the blood and cerebrospinal fluid.

[Cerebral infections due to anaerobic organisms (author's transl)].

Most of intracranial suppurations, especially those from E.N.T. origin, are secondary to anaerobic germs, which can be cocci Gram (+) or Gram (-), in association or not with aerobic bacteria. 97 cases have been observed between 1968 and 1979 at Lariboisière Hospital. When the bacteriological study was realised in correct conditions, anaerobic germs were found in more than 60% of cases, and less than 10 % of cultures were negative. A lower mortality (11 %) and morbidity (34 %) rates follow an improvement in bacteriological diagnosis. Anaerobic bacteria stay very susceptible to antibiotics, such as the association Penicilline-Metronidazole; now, we prescribe immediately such a therapeutic, and eventually modify it after having received the antibiogram. As a conclusion, the treatment of brain abscesses could be simplified: CT scan and a rigourous bacteriology make possible an accurate control and follow-up of the suppuration after simple punction.

[Optic chiasm gliomas (author's transl)].

A follow up study of 40 cases of optic chiasm gliomas is presented. Histological examination was obtained in 26 cases. All were bipolar spongioblastomas. Four of these were optic nerve gliomas, 17 chiasmatic gliomas and 19 invasive optohypothalamic tumors. The main clinical features are reviewed with special attention to endocrine disorders, including precocious puberty and diencephalic emaciation. Neurofibromatosis was present in 30%. Diagnostic difficulties are outlined, especially in chiasmatic tumors without optic nerve involvement: event with C T scan, the differential diagnosis with ectopic pinealomas, craniopharyngiomas and primary hypothalamic tumors, may be difficult. The operative mortality was 12% (3/25). All but one, had radiotherapy either alone or post operative. All four of the patients with unilateral optic nerve gliomas have a normal C T scan more than 10 years after treatment. Of the 17 patients with chiasmatic gliomas, 14 are alive with normal C T scan in 2 cases, and with reduction in size of the tumor in 7. Eleven of 19 patients with invasive gliomas are alive with diminished size of the tumor in 7. Improvement in visual acuity, reduction in size of tumor mass, persistent increase in size of non irradiated tumor and lack of recurrence of optic nerve glioma even with histological proven chiasmatic involvement is taken as evidence of the effect of radiotherapy. The sub frontal approach is emphasized for optic nerve and optic chiasm tumors: it provides direct surgical verification with the opportunity for complete removal, large biopsy, decompression of the optic foramina or cyst removal. Regarding secondary endocrine disorders, radiotherapy is discussed in children. If necessary it must be managed under careful evaluation of endocrine status, principally growth function.

Determination of Kováts retention indices with a capillary column and electron-capture detection: application to the assay of the enzymatic conversion of 3,4-epoxy-1-butene into diepoxybutane.

A method is described that allows for alternate operation of a "pin and cup"-type electron-capture detector (ECD) in the normal ECD mode and in the metastable argon ionization mode. By such means, marker n-alkanes can be run without a change of the gas or temperature parameters, and measurements can be made of the Kováts retention indices of unknown compounds that are detectable in trace amounts by ECD only. As an application, tests are reported which support the probable enzymatic conversion of 3,4-epoxy-1-butene into diepoxybutane during incubation of the former with rat liver microsomes.
