[Determination of the S-phase fraction of myeloid-determined stem cells (CFU-GM) using the cytosine arabinoside suicide technic in patients with acute myeloid leukemia in complete remission following polychemotherapy].

S-phase fraction of granulocyte-macrophage colony forming cells (CFU-GM) was assessed by cytosine arabinoside suicide technique in five normal volunteers and eight patients with acute myeloblastic leukaemia in complete remission after the cessation of polychemotherapy. An increased proliferation of CFU-GM (S-phase-fraction: 55-80%) was found in bone marrow of patient in complete remission up to the 27th day after polychemotherapy. Reduction of this increased proliferation was seen at day 30 after the cessation of polychemotherapy (S-phase fraction: 22 and 33%). Because of increased proliferation of CFU-GM in the early phase after polychemotherapy cytostatic drugs of the antimetabolite-type seem to be unsuitable for purging bone marrow in remission.

[Effect of platelet lysates on the proliferation of human bone marrow cells].

We tested the effect of a lysate from intact human platelets on human granulocyte-macrophage colony forming cells (GM-CFC) using the agar culture technique according to Pike and Robinson. Platelet lysate (PL) was prepared by freeze-thaw procedures of fresh and washed platelets. The results demonstrate the ability of PL to inhibit proliferation of GM-CFC up to about 60-80%. The inhibition is dose-dependent (a hyperbolic curve) and not caused by the influence of PL on CSF-production of feeder layer cells. Direct addition of CSF to the over layer (without feeder layer) maintains the inhibition of colony forming in human bone marrow by PL. The effect of PL may be related to physiological conditions of used platelets. Removal of granulocytes from the experimental system changes the inhibition characteristic of TL in a complex manner. In this case, the inhibition by TL on GM-CFC was found at low concentration only.

Megakaryocytic cells in mixed haemopoietic colonies (CFU-GEMM) from the peripheral blood of normal individuals.

The cellular composition of mixed haemopoietic colonies CFU-GEMM from the peripheral blood of normal individuals was investigated using both an improved in vitro culture assay and a monoclonal antibody reactive with the earliest megakaryocytic precursor cells. The frequency of CFU-GEMM was found to be 22 +/- 7/ml blood (mean +/- SE; n = 8) and 4.9 +/- 1.8/10(6) mononuclear cells. Double-labelling of single colonies with monoclonal antibodies against megakaryocytic as well as granulocytic cells revealed that only 18% of mixed colonies from the peripheral blood contained megakaryocytic cells as compared to 87% of mixed colonies from the bone marrow, while no difference existed with regard to granulocytic cells. It is concluded that circulating multilineage progenitor cells either require different culture conditions to express their megakaryocytic component or, alternatively, that the circulating pluripotent stem cell pool mainly consists of bipotent rather than multipotent progenitor cells.

[Bone marrow capacity and reaction].

The formation of the blood cells leads via proliferation, differentiation and maturation processes, in which cases the bone marrow capacity expresses the functional capacity of this comprehensive system of cell renovation. Nowadays clues to the quantitative behaviour of proliferation of the various compartments (stem cell compartment, indicator cell compartment and morphologically characterizable cell compartment) are already existing also for man, in which cases various proliferation-kinetic tests give adequate informations. Via a quantitative determination of the granulocytes of the bone marrow an estimation of the postmitotic bone marrow storage of the granulocytopoiesis is at present possible. The responsiveness of the neutrophil granulocytes after stimulation (glucocorticoids, endotoxin) may be used in the clinic as bone marrow functional test with restricted statement. The diagnostic possibilities of the agar-colony-technique for the judgment of the bone marrow capacity are represented on the basis of an instance.

[Risk of leukemia in aplastic anemias].

Nowadays an increased risk of leucaemia in patients with aplastic anaemias (insufficiency of the bone marrow) is regarded as certain. If the literature of the last 10 years is analysed, the data concerning a leucaemic transformation amount to values between 0 and 16%. Among our clinical material of 108 patients, 38 of them living, 10 patients (correspondingly 9%) showed a leukaemic transformation. The temporary latency between diagnosing of the aplastic syndrome and the leucaemic transformation was maximally 20 years. By cytomorphological investigations, particularly of the judgment of anomalies of erythroblasts, no certain prognosis concerning a possible development of leucaemia. Also cultivation methods of myelopoietic stem cells by means of diffusion chamber and agar culture technique do not seem to allow a certain evidence on prognosis.
