Effects of prostaglandins and PAF on the contractility of the bovine ciliary muscle.

The present study has investigated the effect of prostaglandins and PAF on the contractility of the bovine ciliary muscle, a tissue involved in the control of aqueous outflow. The results show that the prostaglandins tested (PGI2, its stable analogue Iloprost, PGE2, PGE1, and PGF2 alpha) as well as PAF, were able to contract the ciliary muscle, although with different potencies and efficacies. PGI2 and Iloprost displayed parallel dose-effect curves with upper plateaus that did not differ significantly; however, PGI2 was slightly more potent than Iloprost. This is at variance with what is observed at the level of the platelet prostacyclin receptor. PGF2 alpha was equipotent with the PGEs tested, with a maximal effect not different from either PGI2 or PGEs. PAF was the most efficacious of the compounds tested.

Non-invasive haemodynamic study in hypertensive subjects after treatment with verapamil slow release.

Aim of this study is to investigate the haemodynamic effects, after the short and long-term antihypertensive treatment. After a wash-out period and a placebo treatment period, 30 hypertensive patients received verapamil SR (slow release, 240 mg o.d.) for 30 days. A significant decrease in systolic and diastolic blood pressure was obtained already 4 h after the first administration of verapamil; it was more evident and persistent throughout the study. No significant changes of heart rate or PR interval in ECG were observed. A significant decrease in total vascular resistances, both supine and upright, was evident already 4 h after the drug intake and observed throughout the study. The major effect was obtained after one month. No significant changes of cardiac output, cardiac index and stroke volume were recorded. Furthermore, plasma verapamil levels were measured to confirm that the haemodynamic effects are obtained by low drug concentrations. The present study provides evidence that the antihypertensive effect of verapamil, whose mechanism is the reduction of total vascular resistances, is progressive, long acting and achieved by low plasma levels, when slow release formulation is considered.

Pleomorphic adenoma and adenoid-cystic carcinoma of the salivary glands: comparative immunohistochemical patterns.

A series of 20 cases of pleomorphic adenoma and 19 cases of adenoid-cystic carcinoma of the salivary glands, and one case in the mammary location, were investigated regarding immunohistochemical reactivity for Tissue Polypeptide Antigen (TPA), Pre-Keratins, Vimentin, S-100 Protein, and their arrangement pattern of fibronectin. As a whole, the results support the hypothesis of morpho-structural and mainly, onto-histogenetic similarities between these tumours, but they also underline the need for great care in outlining their morpho-functional features, in relation to their different prognoses.

Correlation of an estrogen receptor-related phosphoprotein with histopathological features in breast cancer.

A series of 65 cases of different histological types of breast carcinoma was investigated for the immunohistochemical location of the estrogen receptor-related, 29 kD phosphoprotein using the ER-D5 monoclonal antibody. The ER-D5 response is heterogeneous in relation to some therapeutic limitations and is correlated with histopathological features of the tumors and survival. The main parameters for evaluation of breast cancers are reviewed, both those that are statistically correlated and those that are not apparently always correlated but are known to have considerable biological meaning, such as the ER-status of tumors.

Prognostic value of clinicopathologic variables and DNA ploidy in stage I cutaneous malignant melanoma.

We studied a consecutive series of 78 stage I cutaneous malignant melanoma in order to identify variables which might predict development of metastases. Anatomical site, sex, tumor thickness, Clark level, microscopic ulceration, growth phase, histologic type, cell type, and DNA ploidy were investigated. Lesions with tumor thickness 1.5 mm, Clark level IV-V, microscopic ulceration and DNA aneuploidy were at high risk for the development of metastases. This study showed the prognostic importance of DNA ploidy in stage I cutaneous malignant melanoma and the strong relationship between DNA ploidy and classic prognostic factors. This variable can be used in routine diagnosis for selecting a high-risk group of patients who may benefit from a more aggressive therapeutic approach.

Prognostic factors in oral squamous cell carcinoma. A review of the literature.

Despite diagnostic and therapeutic progress, the overall prognosis of oral squamous cell carcinoma remains poor and prognostic information is essential for the evaluation and optimal treatment of patients. The prognosis of oral carcinoma depends primarily on clinicopathological factors, that however, have showed a limited predictive value for the identification of patients with high risk of disease relapse. In recent years, the analysis of DNA ploidy, cellular proliferation and oncogenes amplification have been used in attempt to identify new prognostic indicators. The review of the literature shows that TNM stage, tumor grading evaluated at the deep invasive margins and maximal tumor thickness effect on the prognosis. Moreover, the assessment of DNA ploidy and proliferative activity can be used to obtain additional prognostic informations. In contrast, additional studies are needed to understand the prognostic value of oncogenes expression in oral carcinoma.

[The use of 1,1,1-trichloroethane (methylchloroform) in industrial operations: the neurotoxicity risk].

1,1,1-Trichloroethane (1,1,1-TE) is a widely used organic solvent which is thought to be relatively safe. The present review is mainly focused on the neurotoxic effects of 1,1,1-TE. Concentrations above 350 ppm are considered capable of negatively conditioning the performance of the exposed person, in particular reaction time and manual abilities. Peripheral neuropathies have also been described. More recently the potential neurotoxicity of 1,1,1-TE has been underscored by animal studies investigating the effects of this substance in neurobehavioral tests and on neurochemical parameters. 1,1,1-TE alters the motor activity in small laboratory rodents. In fixed interval responding in mice, the effects of 1,1,1-TE are qualitatively similar to those of other volatile (ethanol) and non volatile (benzodiazepines and barbiturates) CNS depressants. In addition, in drug discrimination procedures 1,1,1-TE shares discriminative stimulus properties with barbiturates, toluene, halotane, and oxazepam indicating that this solvent has behavioral effects in common with central nervous system depressant drugs. From a neurochemical point of view long-term exposure to 1,1,1-TE induces astrogliosis in the cerebral cortex of gerbils as indicated by an increased concentration of glial fibrillary acidic protein. In addition low dose inhalation of 1,1,1-TE for 3 months decreases DNA concentrations in several brain regions of the gerbil. 1,1,1-TE affects also brain concentrations of cAMP and cGMP as well as calcium ion control, possibly through an action on voltage-dependent calcium channels. It has been also suggested that intermediate metabolites of 1,1,1-TE may give origin to condensation products with dopamine, ultimately impairing dopaminergic transmission. Some of the neurochemical effects of 1,1,1-TE are shared by other chlorinated organic solvents as well as by CNS depressants. The literature analyzed indicates that 1,1,1-TE may exert significant neurotoxic effects suggesting that more caution in its use is needed and that attention should be paid to the possibility of an additive effect of 1,1,1-TE and CNS depressants (ethanol, barbiturates, benzodiazepines and other volatile solvents) giving origin to severe neurotoxicity.

[Plasmacellular neoplasms of the head and neck: clinical status and personal experience].

In this review, the authors examine the diagnostic, prognostic and therapeutic advances in clinical management of Solitary Plasmacytoma of bone, Extramedullary Plasmacytoma and their relationship to multiple myeloma. The clinical differences among these tumors are the subject of continued studies. Oral symptoms and signs of the neoplasias are relatively common. The literature is reviewed with respect to the prevalence of oral and maxillofacial manifestations and the presence of such manifestations as the first sign and/or symptom of disease. The case of a young adult patient, with a kappa-light-chain positive extramedullary plasmacytoma of the maxillary sinus is presented. The reported case illustrates the necessity of a multidisciplinary approach using relevant immuno-hematologic, radiographic and histologic investigations in the differential diagnosis of these tumors. Factors influencing the prognosis of these neoplastic lesions are also discussed.

Diagnosis of cervical lesions from human papova virus.

The impact of human papillomavirus (HPV) infection in genital pathology has been clarified relatively recently and its relevance appears to be parallel to the liberalization of sexual customs. Aside from some aspects of the infection grossly evident to the naked eye, an increasing number of sub-clinical lesions, that can be colposcopically identified as "flat condylomata" is commonly revealed by cytologic screenings. The Authors report their experience in the diagnosis of this frequent pathology in gynecologic practice, underlining the importance of more fine laboratory investigations, mostly ultrastructural and immunohistochemical.

[Micro-glandular hyperplasia of the uterine cervix. Histo-cytopathological evaluation, differential diagnosis and review of literature].

Microglandular hyperplasia is a lesion of the uterine endocervical mucosa, often associated with a story of contraceptive drugs or pregnancy. It was frequently confused with adenocarcinoma or premalignant glandular changes until its benign nature was recognized. In order to ascertain clinical presentation, hormonal or gestational status, histologic patterns with their variants and cytologic counter-part, we collected and reviewed 28 cases of this condition over a period of a decade. The age ranged from 23 to 54, with a mean of 37.2 years. 11 patients had been receiving oral contraceptives and 5 were pregnant. All samples came from uterine cervix. Cervical smears were available from 17 women. Four histologic patterns were identified, including glandular, reticular, trabecular and solid. Cytologic features varied from aspecific, inflammatory changes of the columnar endocervical cells to more particular findings, including clear cells, strips, sheets, papillae, rosettes and corolla-like aggregates. Moreover, differential diagnostic criteria from cervical neoplasms and various pseudoneoplastic conditions of the cervical glandular epithelium are discussed. An extensive review of the literature is also presented.
