Five new insulin-producing cell lines with differing secretory properties.

Five cell lines have been derived from a rat transplantable islet cell tumour using two different methods. The lines differ in morphology and contain and release different amounts of insulin and glucagon (insulin content, 1-90 pmol/10(6) cells; insulin release, 6-250 pmol/10(6) cells per 24 h; glucagon content, less than 0.005-35 pmol/10(6) cells; glucagon release, less than 0.05-10 pmol/10(6) cells per 24 h). All the lines responded to the presence of the secretagogues leucine (20 mmol/l) plus theophylline (5 mmol/l) by increasing the rate of release of insulin approximately twofold. A high extracellular concentration of potassium (40 mmol/l) caused a three- to tenfold calcium-dependent increase in release of insulin and a parallel release of glucagon. Increasing the concentration of glucose from 2.8 to 16.7 mmol/l did not alter the rate of insulin release by any of the cell lines.

Reversal of recurrent laryngeal nerve palsy following VP16-213 combination therapy in a patient with metastatic teratoma of the testis.

Two years after para-aortic node radiotherapy following removal of a non seminomatous germ cell tumour (NSGCT) of the left testis, M.R., a 32 year old caucasian male developed haemoptysis and a hoarse voice. Chest x-ray revealed a left hilar mass and serum alphafoetoprotein (AFP) and beta-human chorionic gonadotrophin (beta HCG) levels were elevated. In spite of VB3 [1] therapy plus cis-platinum and mediastinal radiotherapy his voice remained hoarse, although serum markers returned to normal, and the hilar mass was reduced in size. Four months after stopping therapy serum AFP levels began to rise, although no clinical evidence of recurrence could be discovered in abdomen, brain or chest. Chemotherapy was restarted using VP16-213 + bleomycin + cis platinum [2]. After the first course of therapy the serum AFP level fell to normal and his hoarse voice disappeared. The patient received five courses of the above regimen, following which complete remission has been maintained for 7 months to date. This case suggest that VP16-213 combined with bleomycin + cis-platinum has great activity against NSGCT of the testis and may have a different spectrum of activity from vinblastine, bleomycin, and cis-platinum, making it a valuable addition to the drugs available for the management of these tumours. In this patient it would seem the complete eradication of the metastatic mass allowed complete and immediate recovery of function of the trapped nerve in spite of it having been paralysed for 15 months.

Radioimmunoassay of serum creatine kinase-BB as a tumour marker in breast cancer.

Brain type creatine kinase-BB (CPK-BB) was measured by radioimmunoassay in the serum of 113 women with breast cancer and 354 female controls. 80% of women with metastatic breast cancer had levels above 3 ng/ml (control range 0.5--3.7 ng/ml); the highest level was 23 ng/ml. 60% of women with local disease but no evidence of distant metastases showed levels above 3 ng/ml, the highest being 9.0 ng/ml. Of women who had presented with stage I, II, or III disease and postoperatively had no evidence of persistent disease 30% had levels above 3 ng/ml. Serial measurements in 31 patients indicated that the serum CPK-BB correlated with clinical response to treatment.

Metastases outside the central nervous system from a presumed pineal germinoma. Case report.

A case of a presumed pineal germinoma in a 17-year-old boy is presented. Metastases in the left loin musculature developed 3 years after presentation, and on biopsy were found to be typical of a seminoma. Bone marrow and chest films showed no evidence of disease. It is therefore considered likely that spread occurred via the lumbar foramina by direct extension from known spinal metastases rather than via the ventriculoatrial shunt. The testes remained normal at all times, and a small testicular primary tumor giving rise to cerebral symptoms from mestastases is considered unlikely, since cerebral metastases from testicular tumors are rare without prior metastasis to the chest. Four previous cases of pineal tumors with chest metastases are reviewed.

A computer-based radiotherapy clinical record system using mark-sense forms.

A computer-based system for radiotherapy clinical records is described. Its main features are the use of a single mark-sense form for input of data at all stages of the patient's progress, and the provision of a plain language print-out of the patient's record after each updating. The programs are written in Fortran and are implemented on a Rank Xerox Sigma 6 computer.

Criteria for setting quantitative estimates of acceptable intakes of chemicals in food in the U.K.

The U.K. Committee on Toxicity of Chemicals in Food, Consumer Products and the Environment (COT) provides advice to government on the safety-in-use of additives and on the possible health hazards from contaminants in food. In the past the COT has given advice on additives in descriptive rather than numerical terms; chemicals were either acceptable, temporarily acceptable or not acceptable for food use. From 1989 onwards the Committee will set acceptable intakes for new additives or existing additives under review and tolerable intakes for contaminants. These will provide guidance on levels of intake which generally should not be exceeded, and will be an aid to decisions on maximum levels of use of particular additives in specified foods. The criteria used by the COT to set quantitative estimates of acceptable and tolerable intakes are discussed.
