[Benign gastric ulcer and ulcerated gastric carcinoma. Considerations on various clinico-endoscopic cases].

Clinical data, X-ray, findings, and the endoscopic and cytologic diagnosis of 94 patients admitted to hospital for gastric ulcer have been reviewed. The purpose of the study was to establish the frequency of malignancy and the reliability and comparative merits of clinical factors, radiology, endoscopy and multiple biopsies in arriving at exact diagnosis. The results show that endoscopy is the most accurate in differentiating benign from malignant gastric ulcers.

Prognostic features of endometrial cancer in estrogen users and obese women.

In a case-control study to evaluate risk factors for endometrial cancer in Italy, use of noncontraceptive estrogens was associated with a moderately increased risk, whereas obesity appeared to be the most important single risk factor related to cancer of the endometrium. This report considers the estrogen and obesity-related relative risks with regard to various prognostic features of tumors (clinical stage, histologic grade, extent of myometrial invasion, lymph node involvement). In both estrogen users and obese women, the increase in relative risk was greater for earlier than for more advanced tumors. These findings, therefore, provide further support for a specific role of both exogenous and endogenous estrogens in endometrial cancer.

Oestrogens and obesity as risk factors for endometrial cancer in Italy.

In a case-control study to evaluate risk factors for endometrial cancer, obesity, history of various diseases, reproductive and menstrual characteristics, marital status, education, and lifetime use of female hormones were examined in 173 histologically proven endometrial cancers and 347 controls. Obesity, non-contraceptive oestrogen use, late menopause, low parity and history of uterine fibromyomas were associated with an increased risk of endometrial cancer. The relative risks of obesity and oestrogen use seem to fit well for an additive model. Histological differentiation was positively correlated both to oestrogen use and to level of overweight, supporting the hypothesis of a specific role of oestrogens in endometrial cancer.

[Echocardiographic patterns of discrete subaortic stenosis. Report of two cases (author's transl)].

The echocardiographic patterns of the discrete subaortic stenosis are reported. They consist of: narrowing of the left ventricular outflow tract, presence of extra echoes in the septal-aortic passage, abnormal movement of leaflets aortic valve, uniformal thickness of the interventricular septum and posterior ventricular wall. The cross-sectional echocardiography consent to specify the entity, the correct spatial orientation of the stenosis, and to discriminate the discrete membranous type from the fibromuscular diffuse obstruction. The Authors present two cases respectively of subvalvular aortic stenosis of membranous type and fibromuscular type, which are examined by M-mode and cross-sectional echocardiography. Cardiac catheterisation demonstrate, in the first of the cases, also the presence of dinamic obstruction. The specific diagnostic utility of the echocardiography in the investigation of these diseases is confirmed.

[Fiberoptic endoscopic examination in the diagnosis and therapy of digestive hemorrhages of the supra-mesocolic tract. Retrospective analysis of 46 cases].

Forty-six cases of upper gastrointestinal haemorrhage examined during three years have been revised. The patients were 39 men and 7 women, from 22 to 73 years old: twenty-six of them were first examined by endoscopic technique, while in the other twenty-ones the first examination was a bariummeal X-ray. All the twenty-six patients of the first group were examined within 48 hours from admission to hospital while the twenty patients of the second group could be examined not before the third day from admission. The evaluation of our results has confirmed an overt superiority of endoscopic versus radiologic technique, mainly in detecting non-ulcerative mucosal alterations responsible of bleeding.

[Epicutaneous electrical recording for immediate diagnosis of the site of intestinal occlusions and subocclusions].

64 patients have been studied by means of an original abdominal epicutaneous electric recording technique. Cases of intestinal paralysis did not show up waves of particular importance while cases of intestinal occlusion showed waves of three basic types. A close relationship was found between types of waves and occlusion localization. The physiopathological and clinical implications of this diagnostic approach are discussed.
