[Liver transplantation: anomalies of the hepatic artery and the liver in 80 donors].

The author has studied the main hepatic artery (HA) alterations and liver related disease (LRD) in 80 donors. The normal arterial anatomy with a single HA arising from the celiac trunk was seen in 61% of cases; a left hepatic artery (LHA) off left gastric artery (LGA) in 19% of cases and a right hepatic artery (RHA) arising from superior mesenteric artery (SMA) in 15% of cases. Two rare anomalies: an HA off anastomosis connecting celiac trunk to SMA and a LHA off LGA around the esophagus. A case of absence of celiac artery and the splenic artery (SA) was found arising directly from the aorta, and another one the SM arose from RHA. Six livers were turned down: 3 by cholecystitis, 2 by preoperative hypoxia and 1 by hepatic polycystic disease. Steatosis and hemangioma have been among the others LRD. In conclusion, HA and LRD have occurred in approximately 50% of donors, this fact underline the importance of searching the anomalies of the HA and LRD during the organ retrieval.

[Cellularity of distal pulmonary parenchyma in rats by the bronchoalveolar lavage method in acute pancreatitis].

The induction of pancreatitis by injecting the taurocholate acid 2.5% solution into the rat's hepatic duct. Ten rats formed the normal control group, nine rats the two hours pancreatitis group and eight rats the eight hours pancreatitis group. The comparison of the groups has shown a significant increase in the numbers of macrophages, neutrophils and lymphocytes in the broncho-alveolar lavage of pancreatitis group (p < 0.05) indicating an acute inflammatory pneumopathy.

Acute Disease↗

[Hepatic artery aneurysm: treatment with left hepatectomy. Report of a case].

A 71-year-old female with a rupture of left intrahepatic artery aneurysm with acute upper abdominal pain, weight loss and fever. The diagnosis was established with doppler ultrasound, contrasted abdominal computer tomography scanning and celiac and mesenteric artery angiography. Prompt recognition and left hepatectomy led to a favourable outcome.

[Main variations of the extrahepatic biliary system and their application to the so-called "split-liver" transplantation technique].

Sixty fresh adult livers were obtained from cadavers together with celiac trunk, head of the pancreas and superior mesenteric artery. We have found a single right hepatic duct in 13(21.6%) of the cases and a multiple right hepatic duct in 47(78.3%). A single left hepatic duct was found in 3(5%) of the cases and a multiple left hepatic duct in 57(95%). A median hepatic duct was found in 18(30%) of the cases, out of this 2(3.3%) was multiple median hepatic duct.

[Surgical treatment of solitary liver cysts].

Solitary hepatic cysts an uncommon disease in the past, which incidental diagnosis is increasing with the advent of ultrasound and computed tomography, are nowadays very frequent. Cysts that reach massive proportions often become symptomatic and necessitate surgical intervention. Surgical wide unroofing technique is a simple procedure advocated for the treatment of symptomatic patients. Ten patients with solitary liver cysts were submitted to wide unroofing with good immediate and late results.

[Annular pancreas. Report of a case and review of the literature].

The authors present a case of newborn pre-term with birthweight of 2,250 g. There was polyhydramnios. On the first day she developed jaundice suggested upper intestinal obstruction. A plain radiograph of the abdomen confirmed the previous finding of obstruction of the duodenum, which showed a "double bubble" image with no other gas shadow in the rest of the abdomen. Contrast rodiologic examination of the abdomen confirmed total obstruction of the duodenum second portion. On the 2nd day after birth, laparotomy revealed an annular pancreas. The reconstruction was made by a side-to-side proximal duodeno-jejuno-anastomosis. The postoperative course was uneventful. Barium radiography at one month postoperatively was good.

Duodenal Obstruction↗

[Mesenteric venous thrombosis after azygos-portal disconnection with splenectomy for the treatment of bleeding esophageal varices in mansonian schistosomiasis. Three cases reports].

The consequence of an acute mesenteric venous thrombosis following porta-azygos disconnection for the treatment of bleeding esophageal varices due to mansonian schistosomiasis has not been well defined in the literature. The clinical manifestations reported were fever, spasmodic abdominal pain associated with food intake. We treated three patients with thrombosis of the portal-mesenteric trunk following porta-azygos disconnection and adopted a conservative clinical approach in two patients while one had to have a surgical small bowel ressection.

Angiomyolipoma of the liver.

Angiomyolipoma, a form of mixed mesenchymal tumor, a not uncommon tumor of the kidney, is rarely found in the liver. We report a case of angiomyolipoma of the liver in a 34-year-old woman, in whom diagnosis was made by computed tomography (CT), magnetic resonance imaging, superior mesenteric and celiac trunk angiography and histological examination. The treatment of choice was a successful extended right hepatectomy.
